PURPOSE: The purpose of this study was to report on the treatment of high astigmatism after Descemet stripping automated endothelial keratoplasty with IntraLase (IntraLase, Inc., Irvine, CA) -enabled astigmatic keratotomy (IEAK).
METHODS: A 85-year-old patient with pseudophakic bullous keratopathy underwent an intraocular lens exchange and Descemet stripping automated endothelial keratoplasty surgery on his left eye. Four months after surgery, high astigmatism was treated with IEAK.

We present a patient with marginal corneal degeneration and corneal astigmatism of more than 10.0 diopters (D). A toric phakic intraocular lens (IOL) of 7.0 D cylindrical power was implanted in both eyes to correct the high astigmatism. Of 76 members of a large pedigree with familial aniridia, 61% of the 38 affected patients had visual acuity of 6/9 (20/30) or better and only 5% had acuity of 6/60 (20/200) or worse. Cataracts were present in 18% of affected patients; glaucoma in 13%; and strabismus in 34%. Sixteen percent of affected patients had strabismic amblyopia. No patient had nystagmus or corneal pannus. The good visual acuity in this family, as compared to that in others, indicates that the absence of iris tissue alone cannot account for the severe visual problems usually associated with aniridia.

We examined 25 patients from three pedigrees with dominant optic atrophy (Kjer type). Follow-up on 20 patients ranged from five to 40 years (mean, 16 years; median, 13 years). Pituitary tumors are a significant cause of visual morbidity. The medical records of 45 patients with histologically verified pituitary adenoma and visual defects (fields and/or acuity) were reviewed. We examined two patients who received contaminated corneas from the same organ donor during penetrating keratoplasty. Both developed Candida albicans endophthalmitis, which responded to surgical and antifungal therapy. On follow-up examination one patient had a visual acuity of hand motions, a pupillary membrane, and a macular scar. The other had a visual acuity of 20/100 and a clear graft.
