Neonatal mortality and perinatal risk factors in rural southwestern Nigeria: a community-based prospective study.

BACKGROUND: Reliable data on births and deaths particularly at the community level are scarce yet they are urgently needed to inform policy and assess the improvements which may have occurred with recent interventions. OBJECTIVE: To determine neonatal mortality rate and identify perinatal risk factors associated with neonatal deaths. METHODS: In a community based prospective study, baseline data on births and deaths were collected as they occurred in a rural community of Southwest Nigeria from 1993 to 1998. Data on births and deaths were collected for the period. RESULTS: There were 972 liv

Examining autonomy's boundaries: a follow-up review of perinatal mortality cases in UK independent midwifery.

BACKGROUND: An earlier matched cohort study in the United Kingdom found a significantly higher perinatal mortality rate for births booked under an independent midwife compared with births in National Health Service units (1.7% [25/1,508] vs 0.6% [45/7,366]). This study examined independent midwives' management and decision making in the 15 instances of perinatal death that occurred at term. METHODS: Thematic analysis of independent midwives' case notes was performed in instances of perinatal mortality. Semi-structured interviews were conducted with the midwives concerned. RESULTS: Home birth…

Maternal-perinatal morbidity and mortality associated with adolescent pregnancy in Latin America: Cross-sectional study.

OBJECTIVE: This study was undertaken to determine whether adolescent pregnancy is associated with increased risks of adverse pregnancy outcomes. STUDY DESIGN: We studied 854,377 Latin American women who were younger than 25 years during 1985 through 2003 using information recorded in the Perinatal Information System database of the Latin American Center for Perinatology and Human Development, Montevideo, Uruguay. Adjusted odds ratios were obtained through logistic regression analysis. RESULTS: After an adjustment for 16 major confounding factors, adolescents aged 15 years or younger had high

Perinatal and infant mortality and low birth weight among residents near cokeworks in Great Britain.

With growing evidence of the adverse health effects of air pollution--especially fine particulates--investigators must concentrate on the fetus, neonate, and infant as potentially vulnerable groups. Cokeworks are a major source of smoke and sulfur dioxide. In the current study, the authors investigated whether populations residing near cokeworks had a higher risk of adverse perinatal and infant outcomes. Zones of 7.5-km radius around 22 cokeworks in Great Britain were studied, within which the authors assumed that exposure declined from highest levels within 2 km to background levels. Routinel

Perinatal mortality among pregnant undocumented migrants in Norway 1999-2020: A register-based population study.

BACKGROUND: Irregular legal status is a recognized health risk factor in the context of migration. However, undocumented migrants are rarely … born abroad), and 'non-migrants' (with an identity number and born in Norway). The main outcome was perinatal mortality, i.e., death of a foetus ≥ gestational week 22, or neonate up to seven days after birth. We used log-binominal regression to estimate the association between legal status and perinatal mortality, adjusting for several maternal pre-gestational and ge …

Extracardiac Doppler indices predict perinatal mortality in fetuses with Ebstein anomaly and tricuspid valve dysplasia.

OBJECTIVES: Ebstein anomaly and tricuspid valve dysplasia (EA/TVD) carry high perinatal mortality. Past studies have focused on cardiac predictors of mortality; we sought to describe the fetal echo (FE) extracardiac Dopplers in this cohort and determine their association with perinatal mortality. METHOD: Fetuses with EA/TVD at 23 centers from 2005-2011 were included for retrospective study. Doppler pattern and velocity of the umbilical artery (UA), umbilical vein (UV), ductus venosus (DV), and middle cerebral artery (MCA) were collected. Bivariate and multivariate analyzes were performed. The…

Perinatal mortality disparities between public care and private obstetrician-led care: a propensity score analysis.

OBJECTIVE: To examine whether disparities in stillbirth, and neonatal and perinatal mortality rates, between public and private hospitals are the result of differences in population characteristics and/or clinical practices. DESIGN: Retrospective cohort study. SETTING: A metropolitan tertiary centre encompassing public and private hospitals. Women accessed care from either a private obstetrician or from public models of care - predominantly midwife-led care or care shared between midwives, general practitioners, and obstetricians. POPULATION: A total of 131 436 births during 1998-2013: 69 0…

Perinatal Mortality Magnitude, Determinants and Causes in West Gojam: Population-Based Nested Case-Control Study.

INTRODUCTION: In Ethiopia, even if a significant reduction in child mortality is recorded recently, perinatal mortality rate is still very high. This study assessed the magnitude, determinants and causes of perinatal death in West Gojam zone, Ethiopia. METHODS AND MATERIALS: A nested case control study was conducted on 102 cases (mothers who lost their newborns for perinatal death) and 204 controls (mothers who had live infants in the same year) among a cohort of 4097 pregnant mothers in three districts of the West Gojam zone, from Feb 2011 to Mar 2012. Logistic regression models were used to…

Factors associated with perinatal mortality in Kassala, Eastern Sudan: a community-based study 2010-2011.

This study investigated the factors associated with perinatal mortality in eastern Sudan from 2010 to 2011. Among 808 deliveries, there were 15 pairs of twins, giving 823 babies at risk of perinatal deaths. There were 761 live births and 62 perinatal deaths. Of the 62 perinatal deaths, 25 (40.3%) were stillbirths and 37 (59.7%) were early neonatal deaths. The stillbirth risk, early neonatal mortality risk and perinatal mortality rate were 30.9 per 1000 pregnancies, 48.6 per 1000 live births and 75.3 per 1000 births, respectively. In the logistic regression model, home delivery [odds ratio (OR)…

Project Pró-natal: population-based study of perinatal and infant mortality in natal, Northeast Brazil.

The Pró-Natal project is a collaborative initiative that aims to improve maternal and infant health in a deprived community in Natal, Northeast Brazil. To assess the perinatal and infant mortality in this population of 40,000, we have collected over a 2-year period a consecutive series of 39 autopsy examinations on deaths under 1 year of age. During this period there were 2212 live births in the study population. The 14 perinatal deaths are described using the Wrigglesworth classification, and the 25 infant deaths, using a clinicopathological system. The contribution of normally formed stillbi

Perinatal morbidity and mortality rates in twin pregnancies--a 15-year review study from Athens.

There were 150 deliveries of twin pregnancies over 26 weeks in our hospital. The incidence of twin pregnancies was 1.0% in a population cons … een 1,501 to 2,500 grs. Twenty two (9.57%) neonates had weights between 1,501 grs to 2,500 grs. The perinatal mortality when birth weights exceeded 1,500 grs was 7.32%. In total, 186 (62%) newborns were delivered vaginally and 114 (38%) by cesarean section. Forceps were used in 4.33% of the cases and breech extraction was performed in 2.33%. The mode of delivery and …

Perinatal mortality rate and associated risk factors in mogadishu, somalia: A hospital-based cross-sectional study.

INTRODUCTION: Perinatal mortality is a critical barometer of maternal and newborn health. In fragile states like Somalia, which grapple with prolonged conflict and systemic healthcare challenges, this indicator is particularly stark. Despite global progress, contemporary, localized data from this region remain scarce, hindering targeted public health initiatives. This study aimed to quantify the perinatal mortality rate (PMR) and identify key determinants within two major referral hospitals in Mogadishu. METHODS: We conducted a hospital-based, cross-sectional study with a seven-day prospective…

Placental abruption and perinatal mortality in twins: novel insight into management at preterm versus term gestations.

Twins suffer a disproportionately higher burden of adverse perinatal outcomes than singletons. However, the degree to which preterm delivery shapes the relationship between abruption and perinatal mortality in twins is unknown. Through causal mediation decomposition, we examine how preterm delivery mediates the effect of abruption on perinatal mortality among twins using the US-matched multiple birth data (1995-2000). We estimated the hazard ratio (HR) from Cox models with gestational age as the timescale. We decomposed the total effect (TE) into counterfactual natural direct (NDE) and natural…

Effectiveness of neonatal "near miss" audits in reducing perinatal morbidity and mortality: a systematic review protocol.

OBJECTIVE: This review will assess the effectiveness of neonatal near-miss audits in reducing perinatal mortality and morbidity, as implemented in health care facilities. INTRODUCTION: Every year, 2.5 million newborns die and 2.6 million more are stillborn worldwide. While many perinatal deaths are preventable, their reduction has been markedly slower than the decline of maternal or child mortality rates. It is hypothesized that neonatal near miss is part of the spectrum of stillbirth-neonatal death and that auditing these cases might reduce perinatal mortality and morbidity. INCLUSION CRITE…

Factors associated with perinatal mortality in Nepal: evidence from Nepal demographic and health survey 2001-2016.

BACKGROUND: Perinatal mortality is a devastating pregnancy outcome affecting millions of families in many low and middle-income countries including Nepal. This paper examined the more distant factors associated with perinatal mortality in Nepal. METHODS: A sample of 23,335 pregnancies > 28 weeks' gestation from the Nepal Demographic and Health Survey datasets for the period (2001-2016) was analysed. Perinatal Mortality (PM) is defined as the sum of stillbirth (fetal deaths in pregnancies > 28 weeks' gestation) and early neonatal mortality (deaths within the first week of life), while Extended…
