A Case of Sub-Diaphragmatic Abscess after Injection of Botulinum Toxin to Treat Achalasia.

Achalasia is a disorder of the esophagus characterized by decreased lower esophageal sphincter (LES) relaxation and absence of esophageal peristalsis. Botulinum toxin (BT) injection targeting the LES has been used in the treatment of achalasia and other esophageal motility disorders, and it is considered to be safe and effective for short-term symptomatic relief. Serious adverse events of this procedure are rare. We report the case of an 83-year-old woman treated with BT injection for achalasia who subsequently developed a subdiaphragmatic abscess.

Gastric outlet obstruction with an elevated serum pancreatic lipase secondary to an infraumbilical hernia.

We report the case of a 68-year-old woman who presented with symptoms and signs of gastric outlet obstruction with a history of a ventral hernia. Clinical examination revealed a large ventral hernia with visible peristalsis of the herniated viscera. Initial serum biochemistry revealed a markedly elevated lipase level and deranged renal function. Computed tomography demonstrated an infraumbilical hernia with herniation of the stomach through the ventral defect and distortion of the pancreatic anatomy. The hernia was reduced operatively and repaired, leading to an uneventful recovery.

Achalasia.

This article reviews the diagnosis and treatment of achalasia, a rare esophageal motility disorder characterized by absent peristalsis and failure of the lower esophageal sphincter (LES) to relax. Various treatment options including management with sublingual nitrates or calcium channel blockers, injection of the LES with botulism toxin, pneumatic dilation of the LES, and pneumatic dilation are discussed. Laparoscopic Heller myotomy is minimally invasive with incumbent low morbidity and mortality rates, and combined with a partial fundoplication is a durable, safe, and effective treatment opti…

Inflammatory fibroid polyp or Vanek's tumour.

We report a case of a 65-year-old woman who presented with recurrent episodes of severe, postprandial abdominal pain followed by projectile vomiting. Gastroscopy revealed a large polyp in the prepyloric region. During peristalsis, the polyp was repeatedly 'passing' through the pylorus into the duodenal bulb, hence obstructing the lumen. The polyp was eventually removed in a piecemeal fashion. Histopathologic examination revealed an inflammatory fibroid polyp (known also as Vanek's tumour). A brief review on inflammatory fibroid polyps follows.

Primary oesophageal motility disorders: how primary are they?

The normal regulation of oesophageal peristalsis is complex. Nitric oxide-containing inhibitory neurons and cholinergic excitatory nerve fibres play the key roles. In the so-called primary oesophageal motility disorders, the coordination of oesophageal contractions and lower oesophageal sphincter function is disturbed (achalasia, diffuse oesophageal spasm), or the amplitudes of peristaltic contractions are abnormally high (nutcracker oesophagus). This article focuses on the pathophysiology of achalasia and nutcracker oesophagus. There is evidence that achalasia and nutcracker oesophagus should…

Discrepancy between the renal pelvic fluctuations and the pacemaker discharges at the pelvi-calyceal border.

Experiments were performed in vitro to explore the influence of isoproterenol on the canine pelvi-ureteral system. The rhythmic changes of intrapelvic pressure became no longer manifest and simultaneously recorded electromyograms revealed disappearance of peristalsis at sites distal to the upper region of renal pelvis following application of isoproterenol, while the pacemaker at the pelvi-calyceal border continued to discharge with an enhanced pace. This result has demonstrated that the renal pelvic pressure recorded during treatment with isoproterenol did not reflect the pacemaker activity o…

A novel dynamic in-vitro model of esophageal button battery ingestion demonstrating benefits of honey, but not acetic acid.

PURPOSE: Retained esophageal button batteries (BBs) cause significant morbidity in children. Current guidelines recommend oral honey administration in children over 1 year and esophageal irrigation with 0.25 % acetic acid (ACA) after BB removal, but supporting evidence is limited. We aimed to develop a dynamic in vitro esophageal model to evaluate these interventions. METHODS: Our novel model consisted of fresh porcine oesophagi suspended in an environment simulating body temperature, peristalsis, and saline perfusion. BBs or sham BBs were placed in the oesophagus. Honey was administered at s…

Supplementation with stigma maydis polysaccharide attenuates autism-like behaviors and improves gut function in valproic acid-induced autism model male rats.

Stigma maydis polysaccharide (SMPS) has regulatory effect on the intestinal microflora and promotes gastrointestinal peristalsis. Children with autism spectrum disorder (ASD) often experience gastrointestinal problems and dysbiosis in their gut microbiota. Our previous study revealed that SMPS interventions had an impact on the gut microbiota of valproic acid (VPA)-induced autism model rats. However, the effects of SMPS on the behavior and gut function of autism model rats remain poorly understood. Therefore, we gave different doses of SMPS intervention in the early stage of autism model rats …

Coordination of Pharyngeal and Esophageal Phases of Swallowing.

Although swallowing has been reviewed extensively, the coordination of the phases of swallowing have not. The phases are controlled by the b … rocesses. A significant inhibitory process is deglutitive inhibition. When one swallows faster than peristalsis ends, peristalsis is inhibited by the new pharyngeal phase. This process prevents the ongoing esophageal peristaltic wave from blocking the bolus being pushed into the esophagus by the new wave. The esophageal phase returns during the last swallow o …

Esophageal motility disorders in symptomatic patients and its relation to age.

BACKGROUND: Although swallowing disorders can impact individuals of any age, some are unique to the elderly, and others are frequent. Some disorders, such as achalasia, are diagnosed by esophageal manometry studies, which assess lower esophageal sphincter (LES) pressure and relaxation, peristalsis in the esophageal body, and contraction wave characteristics. This research aimed to evaluate esophageal motility dysfunction in symptomatic patients and its relation to age. METHODS: Conventional esophageal manometry was performed on 385 symptomatic patients who were divided into two groups; Group …

Re-orchestration of blood flow by micro-circulations.

Haemoglobin Video Imaging (HVI) demonstrates conjunctival and episcleral blood flow in man with the resolution of a single erythrocyte. Longitudinal muscle dysfunction in achalasia esophagus and its relevance.

Muscularis propria of the esophagus is organized into circular and longitudinal muscle layers. Goal of this review is to summarize the role of longitudinal muscle in physiology and pathophysiology of esophageal sensory and motor function. Simultaneous manometry and ultrasound imaging that measure circular and longitudinal muscle contraction respectively reveal that during peristalsis 2 layers of the esophagus contract in perfect synchrony. On the other hand, during transient relaxation of the lower esophageal sphincter (LES), longitudinal muscle contracts independently of circular muscle. Rece…

Perturbation of hoxb5 signaling in vagal neural crests down-regulates ret leading to intestinal hypoganglionosis in mice.

BACKGROUND & AIMS: The enteric nervous system (ENS) controls intestinal peristalsis, and defective development of this system results in hypo/aganglionosis, as seen in Hirschsprung's disease. In the embryo, vagal neural crest cells (NCC) migrate and colonize the intestine rostrocaudally then differentiate into the ganglia of the ENS. Vagal NCC express the homeobox gene Hoxb5, a transcriptional activator, in human and mouse, so we used transgenic mice to investigate the function of Hoxb5 and the receptor tyrosine kinase gene Ret, which is affected in many patients with Hirschsprung's disease, i…
