IgG4-associated inflammatory pseudotumor of ureter: clinicopathologic and immunohistochemical study of 3 cases.

Inflammatory pseudotumors are lesions characterized by proliferation of fibroblasts/myofibroblasts with variable chronic inflammatory cell i … ositive plasma cells; infiltration pattern of eosinophils and histiocytes; presence of obliterative phlebitis; and immunohistochemical profiles of smooth muscle actin, anaplastic lymphoma kinase, and CD68. Three patients, 45- and 47-year-old men and 84-year-old woman, all presented with flank pain and ureteral narrowing by a mass effect. Microscopic examina …

[Phylephebitis due to diverticulitis].

A case of a 52 year-old-male, with past medical history of renoureteral crisis and recurrent episodes of abdominal pain, is presented. The p … number of diverticulum. Surgery was performed in order to rule out an acute diverticulitis. A phylephlebitis, infective suppurative thrombosis of the portal vein and its branches (inferior mesenteric vein and splenic vein) was found due to an acute diverticulitis with neither perforation nor abscess. A ligature of the inferior mesenteric vein and a Hartman …

[Mondor's disease of the female breast].

This report is based on eight patients with Mondor's syndrome. Its main clinical findings is linear retraction of breast skin probably due to thrombophlebitis or lymphangitis. The final diagnosis of Mondor's syndrome implies exclusion of breast cancer by mammography. A differential diagnosis of this entity is discussed.

Polypoid granulomatous and sclerosing endophlebitis of spermatic cord: new pathologic type of schistosomal funiculitis.

A case of polypoid granulomatous and sclerosing endophlebitis of the spermatic cord due to Schistosoma hematobium is described. The lesion was not due to deposition of ova and is believed to have resulted from death of an incarcerated worm mass upstream. As such, the case represents a hitherto unknown form of schistosomal funiculitis. Vascular changes in schistosomiasis are reviewed and a clinicopathologic classification of schistosomal funiculitis is proposed.

IgG4-related disease: review of the histopathologic features, differential diagnosis, and therapeutic approach.

Immunoglobulin G4-related disease (IgG4-RD) is an uncommon disorder that demonstrates characteristic clinicopathologic features including sclerosing lesions with storiform fibrosis, increased IgG4+ plasma cells with an increased IgG4+/IgG+ plasma cell ratio, obliterative phlebitis, and often an increased serum IgG4 level. This review summarizes the characteristic histopathologic and clinical features of IgG4-RD with detailed discussion of the histopathologic characteristics of the most commonly involved anatomic sites. We also present recent advances in our understanding of the pathophysiologi…

[IgG4 related disease presenting as panhypopituitarism and perimacular scotoma].

IgG4-related disease (IgG4-RD) is a recently described systemic entity of unknown origin. It predominantly affects older men and has distinctive histopathologic features as storiform fibrosis, obliterative phlebitis, dense lymphoplasmacytic infiltrate with immunostaining for IgG4, and it may be associated with elevated serum levels of IgG4. Although any organ can be affected, pituitary gland is rarely involved. We describe the case of a 36-year-old man who presented with headaches, impaired vision, panhypopituitarism with diabetes insipidus and an infiltrative lesion mainly of infundibulum and…

Perivascular fibrosis and IgG4-related disease: a case report.

Immunoglobulin G4-related disease (IgG4-RD) is a newly recognized fibroinflammatory condition which can potentially involve any organ. Some characteristic histopathologic features with lymphoplasmacytic infiltrate, an increased number of IgG4+ cells, storiform fibrosis and obliterative phlebitis are the mainstay for diagnosis. Serum IgG4 levels often increase. We report the case of a patient with perivascular fibrotic lesions involving the aortic arch and the splenic hilum, with a surgical biopsy-proven diagnosis of IgG4-related disease. The patient is now undergoing a low-dose corticosteroid …

[Severe asthmatic crisis during general anesthesia in a patient with IgG4 related disease].

We experienced severe asthmatic crisis during general anesthesia in a 45-year-old man with IgG4-related disease, COPD and athma undergoing removal of submandibular gland. The ventilatiory failure was caused by the stimulation of the operation, sputum, and neostigmine. His serum IgG4 level was extremely high. IgG4 related disease is a recently emerging entity characterized by a diffuse or mass forming inflammatory reaction rich in IgG4-positive plasma cells associated with fibrosclerosis and obliterative phlebitis. It is associated with an elevated serum level of IgG4 and an allergic disease. W…

Overall clinical utility of the intramuscular preparation of imipenem/cilastatin.

Imipenem is the broadest-spectrum antibiotic currently available but requires frequent intravenous dosing for efficacy. A recently formulate … ration include: less frequent dosing, avoidance of the complications of intravenous administration (phlebitis, bacteremia, fluid overload, nursing time), ability to use the drug in settings where intravenous administration is either undesirable or impossible and, surprising to medical personnel, greater patient satisfaction.

[Milk fever].

Infectious complications following delivery were, in the past, attributed to "milk fever": these were milk congestion, milk deposits, rancid milk, etc., that were held responsible. The milk was reabsorbed into the blood of the patient and settled in the peritoneum ("milk peritonitis"), in the broad ligaments (pelvic abscess), in the thighs (phlebitis) and also in the breasts (breast abscess). This belief, originated by Aristotle, was accepted by excellent authors like Andre Levret (1703-1780), one of the most famous French obstetricians and Nicolas Puzos, at the same time. More recently, autho…

Studies of intravenous cefoxitin (MK306).

The pharmacokinetics of cefoxitin, a new cephamycin antibiotic, were studied in six patients who were undergoing total hip replacement, and … infections against sensitive organisms. It was well tolerated, and the only side effect was that of phlebitis in long-term therapy. Cefoxitin may be valuable for prophylactic use in bowel and orthopaedic surgery.

Pulmonary manifestations of IgG4‑related disease in a South African patient.

Immunoglobin 4-related disease (IgG4-RD) is an auto-immune, multisystem inflammatory disorder characterised by storiform fibrosis, lymphoplasmacytic infiltration and obliterative phlebitis on histology. Its pathophysiology is not well understood, but is thought to occur due to complex interactions between T helper 2 cells, their cytokines, chemokines, and B lymphocytes that become dysregulated and produce dysfunctional immunoglobulins. Here, we present a case report of a 54-year-old man who was initially suspected of having lung cancer on imaging, but was ultimately diagnosed with IgG4-RD on h…

The Use of Dalbavancin with a Dermal Substitute Application - a Case Report.

Skin and soft tissue infections (SSTIs) are a large group of diseases with a wide range of clinically different conditions, some of which can be immediately life-threatening. A number of bacteria play an important role in the etiology of SSTIs, especially gram-positive cocci Staphylococcus aureus and Streptococcus pyogenes. In this case report, a young woman with skin defects after a fasciotomy was treated using a dermal substitute application. Multiple infectious complications in the skin and soft tissues with a high risk of development of phlebitis and with significant intolerance to a varie…

A Canadian perspective on the safe administration of bendamustine and the prevention and management of adverse events.

Although bendamustine has been used to treat lymphoproliferative disorders for decades, it has only recently been approved for use in Canada … ydrating patients to minimize fatigue, and using free-flowing saline at the closest port to prevent phlebitis.

Static blood flow, phlebitis, and genetic and acquired hypercoagulability often contribute to venous thrombosis.
