BACKGROUND: Increasing doses of ultraviolet (UV) radiation are tolerated in patients with vitiligo, due to photoadaptation. In this pilot study, five patients with Fitzpatrick skin phototypes IV-VI with vitiligo received six treatments of targeted UVB phototherapy over a 3-week period.
METHODS: To investigate photoadaptation, minimal erythema dose (MED) testing was conducted on treated and untreated vitiliginous and normal skin at baseline and after three and six treatments. One patient had unattainable MED values, and was hence excluded.

Two patients, a nine year old girl and a 59 year old woman, presented with extensive and recalcitrant lichen sclerosus et atrophicus (LSA). Tryptophan and nonesterified fatty acids (NEFA) are bound to serum albumin. NEFA displace tryptophan from their binding sites. Since NEFA decrease during phototherapy, we examined reciprocal variations of NEFA, total and free tryptophan in jaundiced newborn infants during phototherapy. After 24 h of phototherapy, we noticed a significant decrease of both NEFA and free tryptophan (p less than 0.001). Free tryptophan variations might affect the synthesis of cerebral neutrotransmitter 5-hydroxytryptamine, which depends on the passing of free tryptophan through the blood-brain barrier.

The biochemistry of bilirubin is reviewed with particular reference to newborn infants. The formation, properties, and metabolism of bilirubin are summarized and the importance of molecular shape, hydrogen-bonding, and polarity on the biologic disposition of bilirubin is emphasized. The chemical basis for the subtle influence of visible (blue) light on bilirubin structure and metabolism is explained, and recent concepts of the mechanism of phototherapy are presented. A glossary of current jargon is appended.

Thirty four term babies with physiological jaundice were subjected to continuous phototherapy and to two regimens of intermittent phototherapy. The difference in serum bilirubin kinetics between the three groups of treated babies was insignificant; a schedule of one in four hours of irradiation achieved the same treatment effect as continuous phototherapy.

Thirty-one patients with early mycosis fungoides (MF) and three patients with parapsoriasis en plaques were treated with ultraviolet phototherapy (280 to 350 nm) at home using a commercially available light source containing four Westinghouse FS40 lamps. When young rats are exposed to white fluorescent light the concentration of calcium in their serum decreases. This effect is prevented by shielding the occiput, by inhibiting corticosterone synthesis, and by exogenous melatonin. Furthermore, the expected hypocalcemic response to cortisol injection is prevented by melatonin. Light-induced hypocalcemia may result from increased calcium uptake by bone when the blocking effect of melatonin decreases after pineal inhibition by transcranial illumination.

The serum vitamin E levels of 11 full-term and 10 premature infants, jaundiced and subjected to phototherapy, were measured and compared wth 9 premature and 10 full-term jaundiced control infants. No differences were observed before or after phototherapy or 1 week after stopping it. The same negative results were noted in the two groups of infants regarding the values of microhematocrit, Hb and reticulocytes.
