A 45-year-old-woman had plasma cell leukemia (PCL) and mild renal insufficiency. Renal biopsy findings were compatible with kappa-chain nephropathy. Our case adds PCL to the list of plasma cell dyscrasias associated with kappa- and/or lambda-chain nephropathy.

Neoplastic plasma cell disorders, multiple myeloma, extramedullary plasmacytoma and solitary plasmacytoma affect the head and neck with different manifestations. Multiple lytic lesions of the jaws and infrequent soft-tissue lesions characterize multiple myeloma's presence. The solitary plasmacytoma of bone is infrequent in the jaws. A patient is described who presented with a combined preleukaemic, lymphoproliferative and plasma cell disorder. These disorders were not related to cytotoxic therapy. The clonal nature of the lymphoid component was demonstrated by lymphocyte surface marker studies and the clonal nature of the plasma cell component by immunoperoxidase studies. Since the involved lymphoid and plasma cells contained different heavy and light chains, they were felt to originate from two separate B cell clones not related to the clone responsible for the preleukaemic component.

The role of haemolysis in producing deficient complement function in homozygous sickle cell disease was studied by measuring indices of complement activation and of haemolysis in 30 asymptomatic patients. Plasma concentration of C3d (an index of increased C3 turnover) was elevated in 40% of patients, and modest decreases in serum concentration of C3 and functionally (haemolytically) active factor B were found. A 54-year-old male was hospitalized because of lethargy, weight loss, an upper abdominal mass, hepatosplenomegaly and peripheral blood plasmacytosis of 66%. Sixt-two series of radiographs were analyzed. Interstitial plasma cell pneumonia (IPCP) was divided into six radiographic stages. A schematic description of these stages is given. Plasma cell leukemia (PCL) is a rare and aggressive form of plasma cell neoplasm, defined by the presence of 5% or more circulating plasma cells in peripheral blood in patients diagnosed with plasma cell myeloma.
