Because of the continuing morbidity and mortality resulting from pneumococcal infection, a program was instituted to redevelop polyvalent vaccines consisting of capsular polysaccharides of Streptococcus pneumoniae. Vaccines containing 50 microgram each of the capsular polysaccharides of as many as 13 pneumococcal types have been shown to be safe, antigenic, and 78.5% effective in the prevention of type-specific putative pneumococcal pneumonia and of type-specific pneumococcal bacteremia in adults. In a population in which pneumococcal pneumonia predominated, the total incidence of radiological

BACKGROUND: Streptococcus pneumoniae is a widely spread human pathogen that can cause infections as severe as meningitis. The pneumococcal vaccination is the optimal approach to prevent morbidity and mortality, but the vaccination coverage is low despite several attempts to improve it. This meta-analysis assesses the impact of the interventions to increase vaccination coverage.
METHODS: A meta-analytic review and a mixed-effects meta-regression on articles between 2013 and 2023 from PubMed, Web of Science and ScienceDirect that assessed the impact of interventions on pneumococcal vaccination

Pneumococcal conjugate vaccines (PCVs) are important preventive tools against invasive pneumococcal disease (IPD) in asplenic patients. This systematic review assesses current data and knowledge gaps on the immunogenicity of PCVs in asplenic patients. Twenty-seven studies were included assessing post-PCV IgG, opsonophagocytic functionality (OPA), or B-cell responses. We show that PCVs induce a strong serotype-specific IgG and OPA response in asplenic adults and children with various underlying conditions. Data in this review suggest that PPV23 has limited benefits for adults, but has been show

BACKGROUND/OBJECTIVES: Infection from Streptococcus pneumoniae can lead to serious complications, such as meningitis and pneumonia, in children under 2 years of age, older adults, and immunocompromised populations. Conjugate vaccines against the pathogen have been licensed for the prevention of invasive pneumococcal disease. Conjugate vaccine development is an involved process demanding extensive characterization of both the polysaccharide (PS) and protein (Pr) moieties in complex structures. One powerful tool in our analytical tool kit that can shed light on various analytical attributes of c

INTRODUCTION: Despite the worldwide usage of pneumococcal conjugated vaccines (PCVs), Streptococcus pneumoniae remains a major cause of morbidity and mortality among children. This study evaluated the effects of universal PCV10 vaccination on incidence, serodistribution and antimicrobial resistance among S. pneumoniae strains causing invasive pneumococcal disease (IPD) in Croatian children.
METHODS: The study analyzed consecutive, nonredundant invasive S. pneumoniae isolates obtained from children up to 18 years of age, collected through a network of microbiologic laboratories in Croatia from

An adolescent patient developed a severe local inflammatory reaction, acute kidney injury, extensive myofascitis, and profound immune thrombocytopenia resulting in vitreous hemorrhage and long-term permanent visual impairment after 23-valent polysaccharide pneumococcal vaccine administration. This severe adverse outcome has not previously been reported in the literature with this particular vaccination. This case highlights rare but serious potential adverse immunologic effects of polysaccharide vaccines.

Patients are known to be at an increased risk of sepsis post-splenectomy due to encapsulated organisms, such as Streptococcus pneumoniae. This is clinically important, as overwhelming post-splenectomy infection (OPSI) is very serious. Prevention, including vaccination, is recommended. Herein, we describe the case of a patient who survived a second attack of OPSI due to S. pneumoniae, serotype 34, that occurred 8 years after acute pericarditis related to S. pneumoniae-associated OPSI. The patient underwent a splenectomy due to malignant lymphoma and had been vaccinated with PPSV 23, but had not

OBJECTIVES: The Korea Disease Control and Prevention Agency (KDCA) identified an increasing prevalence of pneumococcal serotypes not covered by vaccines that are currently used for pediatric invasive pneumococcal disease. In response, the KDCA decided to introduce a 20-valent pneumococcal conjugate vaccine (PCV20), which offers broader serotype coverage.
METHODS: The Pneumococcal/Meningococcal Subcommittee and the Korea Expert Committee on Immunization Practices (KECIP) reviewed the appropriateness of introducing PCV20, focusing on disease characteristics, vaccine properties, cost-effectivene

Invasive pneumococcal disease (IPD) risk increases with age for older adults whereas the population size benefiting from pneumococcal vaccines and robustness of immunogenic response to vaccination decline. We estimate how demographics, vaccine efficacy/effectiveness (VE), and waning VE impact on optimal age for a single-dose pneumococcal vaccination. Age- and vaccine-serotype-specific IPD cases from routine surveillance of adults ≥ 55 years old (y), ≥ 4-years after infant-pneumococcal vaccine introduction and before 2020, and VE data from prior studies were used to estimate IPD incidence and w…

BACKGROUND: Infections are a common reason for hospitalization and death in multiple myeloma (MM). Although pneumococcal vaccination (PV) and influenza vaccination (FV) are recommended for MM patients, data on vaccination status and outcomes are limited in MM.
MATERIALS AND METHODS: We utilized data from the global, prospective, observational INSIGHT MM study to analyze FV and PV rates and associated outcomes of patients with MM enrolled 2016-2019.
RESULTS: Of the 4307 patients enrolled, 2543 and 2500 had study-entry data on FV and PV status. Overall vaccination rates were low (FV 39.6%, PV

BACKGROUND AND OBJECTIVES: Disease caused by Streptococcus pneumoniae is associated with considerable morbidity and mortality in children. Pneumococcal conjugate vaccines (PCVs) are well tolerated and effective at reducing pneumococcal disease caused by vaccine serotypes. VAXNEUVANCE (V114) is a 15-valent PCV containing 13 serotypes in Prevnar 13 (PCV13), plus serotypes 22F and 33F. This large phase 3 study evaluated safety and tolerability of V114 in infants.
METHODS: In total, 2409 infants were randomized to receive V114 or PCV13 at 2, 4, 6, and 12 to 15 months of age. Safety was evaluated

BACKGROUND: Nasopharyngeal colonisation by S. pneumoniae is a prerequisite for invasive pneumococcal infections. Influenza co-infection leads to increased susceptibility to secondary pneumonia and mortality during influenza epidemics. Increased bacterial load and impaired immune responses to pneumococcus caused by influenza play a role in this increased susceptibility. Using an Experimental Human Challenge Model and influenza vaccines, we examined symptoms experienced by healthy adults during nasal co-infection with S. pneumoniae and live attenuated influenza virus.
METHODS: Randomised, blind

The implementation of pneumococcal conjugate vaccines (PCVs) has led to a decline in vaccine-type disease. However, there is evidence that the epidemiology of non-typeable Haemophilus influenzae (NTHi) carriage and disease can be altered as a consequence of PCV introduction. We explored the epidemiological shifts in NTHi carriage using whole genome sequencing over a 5-year period that included PCV13 replacement of PCV7 in the UK's National Immunization Programme in 2010. Between 2008/09 and 2012/13 (October to March), nasopharyngeal swabs were taken from children <5 years of age. Significantly

Incidence of invasive pneumococcal disease (IPD) among HIV-infected children is 20-40 fold greater compared to HIV-uninfected children, including among HIV-infected children on antiretroviral therapy (ART). Also, HIV-exposed, uninfected children have 2.7-fold greater risk of IPD compared to HIV-unexposed children. Areas covered: We reviewed studies identified on Pubmed database with the terms 'PCV' and 'HIV'; studies involving adults only were excluded. Expert commentary: While ART and pneumococcal conjugate vaccines (PCV) have reduced IPD morbidity and mortality in HIV-infected children, ART-

BACKGROUND: Parapneumonic empyema, a serious complication of pneumonia, started increasing among U.S. children before the introduction of the 7-valent pneumococcal conjugate vaccine (PCV7) in 2000, and continued afterwards. This increase was due in part to pneumococcal serotypes not included in PCV7 that were included in the new 13-valent (PCV13) vaccine introduced in 2010. We assessed changes in the incidence of empyema hospitalizations among U.S. children after PCV13 introduction.
METHODS: We calculated annualized empyema hospitalization rates among U.S. children <18years using Nationwide I
