Epidemiological mapping and risk profiling build on the idea that diseases are tied to social-ecological systems that govern the distribution and abundance of transmissible pathogens, vectors and hosts. This is the heart of the emerging field of ecohealth, which examines how biological, cultural, demographic, economic, physical, political and social environments change and how these changes affect the health and wellbeing of humans, animals and ecosystems and the services they provide. This paper is an overview of a special issue of Acta Tropica, whose 15 publications reflect a geographically

The principal strategy adopted by the African Programme for Onchocerciasis Control (APOC), for the control of onchocerciasis in the 19 countries of Africa that now fall within the programme's remit, is that of community-directed treatment with ivermectin (CDTI). Halfway through its 12-year mandate, APOC has gathered enough information on the main challenges to guide its activities in Phase 2. An analysis of reports and other documents, emanating from consultants, scientists, monitors and national and project-level implementers, indicates that there are three broad categories of challenge: mana

Delays in vaccinating communities of color to COVID-19 have signaled a need to investigate structural barriers to vaccine uptake, with mass incarceration demanding greater characterization as a potential factor. In a nationally representative survey from February-March 2021 (N = 1,157), exposure to the criminal legal system, defined as having been incarcerated in prison or jail or having had a family member or close friend incarcerated, was associated with higher odds for COVID-19 vaccine deliberation. Individuals with criminal legal system exposure reported lower confidence in physician recom

The 10th and largest Ebola virus disease epidemic in the Democratic Republic of the Congo (DRC) was declared in North Kivu Province in August 2018 and ended in June 2020. We describe and evaluate an Early Warning, Alert and Response System (EWARS) implemented in the Beni health zone of DRC during August 5, 2018-June 30, 2020. During this period, 194,768 alerts were received, of which 30,728 (15.8%) were validated as suspected cases. From these, 801 confirmed and 3 probable cases were detected. EWARS showed an overall good performance: sensitivity and specificity >80%, nearly all (97%) of alert

BACKGROUND: Although harm reduction is highlighted as an effective intervention for alcohol and drug use, a funding gap for harm reduction interventions has been identified, mainly in low- and middle-income countries. In these countries, tensions between abstinence and harm reduction models have impaired the shift from punitive practices to evidence-based interventions committed to guaranteeing the human rights of people who use drugs. Since 2015, the Brazilian government has adopted a more punitive and abstinence-focused drug policy that jeopardizes the care of people who use alcohol and othe

PURPOSE OF REVIEW: Health policy and political decision-making increasingly shape reimbursement, technology access, workforce capacity, and research funding in urology. Despite these growing influences, many urologists receive little formal training in advocacy. This review is timely given ongoing reimbursement reform, regulatory shifts, cancer policy initiatives, and cross-border health legislation in both the United States and the European Union (EU).
RECENT FINDINGS: Recent literature highlights declining procedural reimbursement, workforce shortages, and regulatory barriers to innovation

The 2025 United Nations Political Declaration on non-communicable diseases (NCDs) and mental health provides a critical opportunity to recognize inflammatory bowel disease (IBD) as part of the global NCD agenda. Once confined to the West, IBD now affects over 7 million people worldwide, with the steepest rise in South Asia, Latin America, and Africa. Its early onset, chronic relapsing course, psychosocial burden, and economic cost align it closely with major NCDs. Yet, policy recognition remains limited, particularly in low- and middle-income countries, where diagnostic delays, inequitable acc

Tobacco control policies are complex with the potential for significant health effects. The public health actors and regulators who design, influence, and implement these policies place a high value on the evidence base that underpins them, engaging in scientific and applied debates about how to evaluate the success of existing policies and justify new actions. To investigate the construction of this evidence base, we conducted interviews with 40 individuals involved in formulating, advising, and implementing flavour restrictions for tobacco products. Our interviews inform case studies of six

(1) Background: Ambulatory services are the main interface between populations and health systems, yet their benefits are unevenly distributed due to structural inequalities-systemic differences rooted in the social, economic, and political organisation of society. Inequities in access and quality are well documented, but patient safety remains the least examined. (2) Methods: Following Joanna Briggs Institute methodology and PRISMA-ScR guidelines, PubMed/MEDLINE and Scopus were searched for studies published from 2010 to 2026. In total, 44 studies were included, predominantly from the USA (n

This paper explores the urgent need for an inclusive model of leadership in public health, particularly in the context of accelerating social and political change. Drawing on lessons from the COVID-19 pandemic and other recent public health crises, the paper argues that traditional top-down leadership structures-often disconnected from the communities most impacted-are insufficient for responding to modern challenges. Instead, public health systems must invest in cultivating leaders who are embedded in, trusted by, and responsive to diverse populations. Citing workforce declines, public mistru

Immunization is one of the greatest public health achievements of all times, preventing millions of deaths and making major improvements in child survival in Africa. Beyond protecting against infectious diseases, vaccines foster educational attainment, productivity, and poverty reduction, while mitigating antimicrobial resistance and economic shocks. However, each year more than 9.5 million children in the World Health Organization African Region remain under-vaccinated, resulting in recurrent outbreaks of vaccine-preventable diseases. In 2024 alone, 59 outbreaks were reported across the regio

Introduction: Sexual violence is a public health problem included in political and social agendas worldwide. It is a medical emergency seen in health services, which are the main source of record-keeping by means of public health surveillance systems. Objective: To analyze the sociodemographic characteristics of victims of sex offenses and their offenders in the municipality of Envigado, Antioquia, between 2011 and 2020. Materials and methods: This was a descriptive cross-sectional study that analyzed all the sexual violence records reported in the Envigado Public Health Surveillance System fo

OBJECTIVES: Most Vietnamese immigrants in the U.S. today arrived as political refugees due to the Vietnam War in the late 20th century. Refugees are disproportionally affected by health and mental health disparities as a result of experiencing distress and potentially traumatic experiences before, during, and after their migration processes. This study involved Vietnamese families facing dementia and used a qualitative approach to investigate participants' experiences before, during, and right after their resettlement in the U.S.
METHODS: In-person interviews were conducted with 11 Vietnamese

Government policies on abortion are a longstanding topic of heated political debates. The COVID-19 pandemic shook health systems to the core adding further to the complexity of this topic, as imposed national lockdowns and movement restrictions affected access to timely abortion for millions of women across the globe. In this paper, we examine how countries within the European Union and the United Kingdom responded to challenges brought by the COVID-19 crisis in terms of access to abortion. By combining information from various sources, we have explored different responses according to two dim

The genesis and global spread of coronavirus disease 2019 (COVID-19) has resulted in significant health concerns amid societies that were ill-equipped for such a formidable opponent. The COVID-19 pandemic has illuminated systemic health disparities and weaknesses within our health care and political systems. With aggressive but necessary interventions to thwart the instantaneous reproduction number, we have simultaneously introduced risk factors for new and exacerbated mental health concerns. Suicide, an ongoing pandemic whose rates are higher than they have been in the past 50 years, is one o…

South America exhibits some of the higher rates of antimicrobial resistance in Enterobactericeae worldwide. This continent includes 12 independent countries with huge socioeconomic differences, where the ample access to antimicrobials, including counterfeit ones, coexists with ineffective health systems and sanitation problems, favoring the emergence and dissemination of resistant strains. This work presents a literature review concerning the evolution and current status of antimicrobial resistance threats found among Enterobacteriaceae in South America. Resistance to β-lactams, fluoroquinolon

Prior to the German reunification, cancer survival was much lower in East than in West Germany. We compare cancer survival between Eastern and Western Germany in the early twenty-first century, i.e. the second decade after the German reunification. Using data from 11 population-based cancer registries covering a population of 33 million people, 5-year age-standardized relative survival for the time period 2002-2006 was estimated for the 25 most common cancers using model-based period analysis. In 2002-2006, 5-year relative survival was very similar for most cancers, with differences below 3% u

OBJECTIVE: To review and discuss the available literature for rabies control in Latin America.
METHODS: A literature search was conducted in PubMed through October 2008. Articles in English, Spanish and Portuguese were reviewed.
RESULTS: Recent reports indicate that the region is close to achieve the goal of eliminating human rabies transmitted by dogs even though there are some isolated cases reported. However, insectivorous and vampire bats continue to expose humans and animals. These cases have been reported with increasing frequency.
DISCUSSION: Many challenges to the successful eradica

AIM: As an increasing number of patients suffer from osteoporosis-related disorders worldwide, the medical as well as the socioeconomic impact of this problem is significant. Although evidence-based guidelines for diagnosis and treatment are available, their application in daily practice is insufficient. The aim of our initiative was to develop a strategy for supporting this transfer.
METHODS: An expert group of the German Society of Orthopaedics and Traumatology (DGOU) has analysed the current scientific as well as health-care data bases regarding diagnosis and treatment of osteoporosis. The

The global spread of highly pathogenic avian influenza (H5N1) clade 2.3.4.4b has evolved into a multispecies panzootic that disrupts conventional boundaries between human, animal and environmental health systems. Using Brazil's response as an illustrative case, this article argues that prevailing containment strategies-particularly mass culling-remain ethically insufficient and practically misaligned with the ecological complexity of H5N1 transmission. To clarify the normative foundations of an alternative approach, we introduce a theoretical framework grounded in zoonoethics and global ecolog
