Treatment of postmenopausal osteoporosis with calcitriol or calcium.

BACKGROUND AND METHODS: Osteoporosis is a common problem whose management is controversial. To evaluate the efficacy and safety of calcitriol (1,25-dihydroxyvitamin D3) in the treatment of postmenopausal osteoporosis, we conducted a three-year prospective, multicenter, single-blind study in 622 women who had one or more vertebral compression fractures. The women were randomly assigned to receive treatment with calcitriol (0.25 micrograms twice a day) or supplemental calcium (1 g of elemental calcium daily) for three years. New vertebral fractures were detected by means of lateral roentgenograp

[Postmenopausal osteoporosis and treatment with sex hormones].

An animal model of postmenopausal osteoporosis was established and the animals were treated with sex hormones in the experiment. From a group of fifty female Wistar rats, forty were castrated and ten were subjected to sham operation served as controls. After fourteen weeks, ten rats each from the castrated (the Model group) and control rats were killed. Through bone histomorphometry and bone mineral analysis it was sure that a good model of postmenopausal osteoporosis was established with a high bone turnover rate at the tissue level. The bone loss occurred mainly at the trabecular surface thr

The role of calcitonin treatment in postmenopausal osteoporosis.

Synthetic salmon calcitonin (SCT) is a potent antiosteoclastic hormone with adjunctive stimulatory effects on osteoblastic function. It is capable of increasing or stabilizing bone mass in osteoporosis and thereby can lessen the risk of fractures. Treatment doses vary from 100 IU daily to 50 IU three times a week, and the duration of treatment is 2 to 5 years. SCT also exerts an analgesic effect on the skeleton that increases its beneficial effect. Side effects, which do not involve organ toxicity, are common but are usually mild and transient. More severe side effects can be managed by maneuv

Postmenopausal pneumococcal tubo-ovarian abscess.

Streptococcus pneumoniae is an uncommon cause of infection in the female genital tract. An unusual case of postmenopausal pneumococcal tubo-ovarian abscess is presented. The pathogenesis of this infection probably entailed the ascent of bacteria from the vagina to the internal genitalia; there was no evidence of infection of the bloodstream or gastrointestinal tract.

[Clinical problems related to postmenopausal osteoporosis].

Authors discuss current hypotheses related to the causes of osteoporosis and its possible therapy. One of the frequent menopausal disorders is osteoporosis, i.e. decrease in bone density, which may eventually lead to the pathological fractures and marked deformities of the skeleton. Etiology of the postmenopausal osteoporosis is not clearly explained. However, one may assume that hormonal disturbances, especially estrogen deficit, plays an important causative role. The treatment is difficult. It is usually aimed at preventing of bone loss.

Role of calcitriol in the treatment of postmenopausal osteoporosis.

Treatment with 1,25-(OH)2D3 (calcitriol) was compared with placebo in a double-blind, randomized, parallel clinical trial of 24 months' duration. Subjects were white women with postmenopausal osteoporosis. The study was completed by 15 patients who received placebo and 12 patients who received calcitriol. Positive slopes were observed in the active treatment group for total body calcium, bone mineral content of the radius, bone mineral density of the lumbar spine, and radiographic absorptiometry of the middle phalanges. In contrast, negative slopes were observed for the bone mineral measuremen

[Hysteroscopy in the diagnosis of postmenopausal uterine bleeding].

Postmenopausal bleeding is a special diagnostic problem in gynecology on account of inadequate specimen obtained during dilatation and curettage for pathologic examination. Instead, hysteroscopy proves to be an ideal procedure in defining the cause of bleeding through direct vision of the pathologic lesions in the uterine cavity. In this series, hysteroscopy was performed in 150 patients with postmenopausal bleeding. The causes of bleeding enumerated by the order of frequency of occurrence are atrophic endometrium, exogenous estrogen effect, unremoved IUD, endometrial carcinoma, submucous myom

A preventive approach of the postmenopausal bone loss.

In countries where osteoporosis in the ageing process tends to be a major problem, this phenomenon is of particular interest. The consequences of the osteoporosis in an ageing population, especially in the coming decades could create serious problems for the health authorities. It would be very interesting to see that in an early stage i.e. at menopause it could be influenced by prophylactic treatments. In a few countries the prophylactic approach of the menopausal osteoporosis has been already introduced. But it will take a long time before prophylaxis of postmenopausal osteoporosis could be

Alternative strategies for prevention of postmenopausal osteoporosis.

Osteoporosis is more readily prevented than treated, and early intervention with effective therapy would be expected to reduce significantly the impact of osteoporotic fractures among the aging population. For the postmenopausal female population, estrogen is the cornerstone of therapy, and multiple studies have demonstrated efficacy in reducing cortical and trabecular bone loss in the axial and peripheral skeleton. Alternative strategies for those who cannot, or will not, take estrogen and who can be documented to be at increased risk of osteoporosis, include calcium supplementation, progesto

Rationale for the use of calcitonin in postmenopausal osteoporosis.

Calcitonin has been used in the prevention and treatment of osteoporosis already for several years. The results have been promising. Especially patients with high-turnover osteoporosis respond satisfactorily to the treatment. In the immediate postmenopausal period calcitonin should be considered appropriate if estrogens are either contraindicated or refused by the patient.

Calcitonin for prevention of postmenopausal bone loss.

A 2-year randomised pilot study was conducted in 70 patients to see whether the osteoclast-inhibiting effect of calcitonin would reduce postmenopausal vertebral bone loss. An oestradiol-treated group was included in the study as a positive control since oestrogens are known to be effective. Calcitonin reduced vertebral bone loss in doses above 250 micrograms human calcitonin (50 international units) a week, and at this dose was as effective as oestradiol.

Diagnosis and management of perimenopausal and postmenopausal bleeding.

The menopausal years are characterized by a deficiency of progesterone and relative hyperestrogenism. This hormonal imbalance creates an environment favorable for the development of endometrial hyperplasia. The pathologic progression of hyperplasia to endometrial carcinoma can be arrested with progestogen therapy. A simple diagnostic approach for peri- and postmenopausal bleeding disorders is presented, along with a rational treatment regimen. Some of the risks and benefits of hormonal replacement therapy are discussed.

Lactose and calcium absorption in postmenopausal osteoporosis.

The prevalence of lactase deficiency and the relationship between lactose and calcium malabsorption in postmenopausal osteoporosis has been assessed in 46 subjects. Malabsorption of lactose occurred in 25 (54%) of the subjects and was associated with a significantly lower milk intake. Malabsorption of calcium occurred in 11 (44%) of the lactase-deficient subjects and in 11 (52%) of normal lactose absorbers. There was no relationship between lactose and calcium malabsorption. Vertebral and forearm mineral densities were not significantly different between normal lactose absorbers and lactase-de

[Non-hormonal treatment of postmenopausal osteoporosis].

Non hormonal treatments for postmenopausal osteoporosis are curative and preventive treatments that do not call for sex hormones. Curative treatments aim to reduce the frequency of relapsing vertebral fractures and they mainly consist in sodium fluoride (50 mg/day) combined with Calcium (1 g element/day). Preventive treatments are intended to prevent the occurrence of vertebral fractures. They consist in calcium, physical exercise, vitamin D and calcitonin. Calcium and physical exercise are simple and inexpensive means which may be convenient for every woman, with no need for any preselection,
