Non-specific X linked mental retardation with aphasia exhibiting genetic linkage to chromosomal region Xp11.

A new type of non-specific X linked mental retardation is described in a three generation family. The three affected males had severe mental retardation (IQ 20 to 30), mutism, growth failure, frequent infections, seizures, and the following minor anomalies: brachycephaly, frontal hair whorl, square face, large mouth, thick lips, and prognathism. There was not a characteristic facies. Normal laboratory studies on the proband included a karyotype with fragile X screening, skeletal survey, blood amino acid, urine organic acid, and HGPRT levels. Linkage analysis was performed with 10 X chromosome …

Surgical Result and Identification of FGFR2 Variants Using Whole-Exome Sequencing in a Chinese Family With Crouzon Syndrome.

Crouzon syndrome is considered as one of the most common craniosynostosis syndromes with a prevalence of 1 in 65,000 individuals, and has a close relationship with variants in fibroblast growth factor receptor 2. Here the authors described a Crouzon syndrome case, which was asked for surgery treatment for the symptom of multisuture craniosynostosis. Mild midfacial retrusion, larger head circumference, proptosis, pseudo-prognathism, and dental malposition could also be found obviously. Then fronto-orbital advancement and cranial cavity expansion were performed to the child. After whole-exome se…

Crouzon syndrome in a fraternal twin: A case report and review of the literature.

BACKGROUND: Crouzon syndrome (CS; OMIM 123500) is an autosomal dominant inherited craniofacial disorder caused by mutations in the fibroblast growth factor receptor 2 (FGFR2) gene. CS is characterized by craniofacial dysostosis, exophthalmos, and facial anomalies with hypoplastic maxilla and relative mandibular prognathism. CASE SUMMARY: Our report involves a 6-year-old fraternal twin boy with many caries in the oral cavity who presented with characteristic features of CS based on clinical and radiographic examinations along with Sanger sequencing. The fraternal girl did not show any abnormal…

A Pediatric Case of Cowden Syndrome with Graves' Disease.

Cowden syndrome (CS) is a rare dominantly inherited multisystem disorder, characterized by an extraordinary malignant potential. In 80% of cases, the human tumor suppressor gene phosphatase and tensin homolog (PTEN) is mutated. We present a case of a 17-year-old boy with genetically confirmed CS and Graves' disease (GD). At the age of 15, he presented with intention tremor, palpitations, and marked anxiety. On examination, he had macrocephaly, coarse facies, slight prognathism, facial trichilemmomas, abdominal keratoses, leg hemangioma, and a diffusely enlarged thyroid gland. He started antith…

Growth retardation, dysmorphic facies and minor malformations following massive exposure to phenobarbitone in utero.

A syndrome of facial dysmorphism, pre- and postnatal growth deficiency, developmental delay and minor malformations is described in two siblings. The facial anomalies consist of short nose with low nasal bridge, hypertelorism, epicanthic folds, ptosis of eyelid (patient 2), lowset ears, wide mouth with protruding lips and relative prognathism. Patient 2 in addition had a cleft soft palate and a hypoplastic phalanx of his fifth fingers. Both siblings were exposed to extraordinary high levels of phenobarbitone (5.0-8.6 mg/100 ml) in utero. The same clinical picture has been reported by others fo…

Surgical cutting guide and single plate fixation for intraoral vertical ramus osteotomy.

With the advancement of digital technology over the last few decades, the use of virtual surgical planning and fabrication of surgical guides have tremendously improved the outcomes of various maxillofacial surgical procedures. The intraoral vertical ramus osteotomy (IVRO) is an orthognathic surgical procedure largely employed for mandibular setback in correcting dentofacial deformities. This study describes the design and application of a surgical cutting guide for IVRO. The guide can also be used to facilitate the placement of miniplate fixation. The initial experience at the authors' centre

Spontaneous Union of an Accidentally Fractured Proximal Segment During Vertical Ramus Osteotomy.

After sawing in the intraoral vertical ramus osteotomy, in the case of an incomplete amputation, the proximal segment is split using the osteotome. Unfortunately, in this case, the proximal segment was accidentally fractured. The operation was completed without performing any other fixation. Postoperatively intermaxillary fixation was performed and kept for a week, followed by physical therapy, which was initiated immediately after. The patient was closely observed for 6 months. No abnormal findings were detected clinically, and the union of bone was verified.

[Significance of ramus measurement through computed tomography in the sagittal split osteotomy of the ramus].

OBJECTIVE: To investigate significance of the distribution and thickness of cancellous and cortical bone in the mandibular ramus using computed tomography (CT) in the design of osteotomy at the medial aspect of the ramus. METHODS: 45 cases with class III prognathism underwent CT before operation. The distribution and thickness of cancellous and cortical bone in the mandibular ramus was measured every 2.5 mm thick from mandibular lingular plane to 20 mm above it. RESULTS: The upper ramus was separated by cancellous bone in only 37.5% of the cases. The other cases had no cancellous bone in the…

A special case of anencephaly in an early-born baby with an exagerated prognastic face: further example for human devolution.

A 7-month-old baby was born in a village near Iskenderun (Turkey) where "Unertan Syndrome" with quadrupedality and primitive cognitive abilities was discovered. The clinical diagnosis was anencephaly. However, his head did not show the classical symptoms of anencephaly because it was covered with bony structures. The baby has an ape-like, prognasthic head with low-set ears and flapped ear flaps. The other parts of the body were similar to humans with broad shoulders and a short neck. This may be a further example of human devolution, which was first reported by Tan (2005, 2006a,b,c). A genetic

[Postural changes of the tongue and hyoid bone following orthognathodontic surgical interventions].

The study was carried out on 28 dysgnathic patients submitted to surgical correction of malocclusion with the use of latero-lateral X-rays pre- and postoperatively. Cephalometry included a tongue and hyoid bone profiles and changes were assessed and quantified. It was interesting to note that aspecific evaluation of progenia patients confirmed the raising of the tongue and the lowering of the hyoid bone. Evaluation of the type of operation carried out led to further considerations. In patients undergoing combined operation there was a prevalence of a raising of the two components; mandible ope

[Profile-altering interventions as preventive measures (author's transl)].

Operations to improve the profile in abnormal positions of the mandible serve as preventive measures on the one hand for improving and harmonizing the external shape of the face and for improving the function of chewing on the other, with consequent prevention of premature loss of teeth, periodontosis and complaints of the temporomandibular joint, and preventing the appearance of digestive diseases. The different bite and mandibular abnormalities are discussed in groups classified according to their operability.

Beta-galactosidase deficiency: prolonged survival in three patients following early central nervous system deterioration.

Three adult patients from two families have shown slowly progressive neurologic deterioration since the age of 3 years, associated with profound beta-galactosidase deficiency. Although affected individuals from the two different families differ in degree of intellectual deficit, facial coarseness and spondyloepiphyseal dysplasia, all lack visceromegaly and macular red spots. The diversity of phenotypic expression in these patients and others previously reported suggests the existence of composite genotypes (compound and double heterozygosity).
