Problems associated with the atrophic mandible.

A range of treatment options has been presented regarding the use of dental implants to rehabilitate patients who otherwise have compromised function using conventional tissue-borne prostheses. This patient population offers significant challenges to both the surgeon and the prosthodontist. Generally speaking, IODs retained on two or more endosteal osseointegrated implants; transosteal, subperiosteal, ramus frame implant-supported and implant-retained prostheses, and a totally implant-supported design offer options to treat the severely atrophic mandible. When bone support or volume is so lack

Prosthetic replacement of the rabbit medial meniscus.

Implants of polyurethane-coated polyester (Dacron), polyurethane-coated PTFE (Teflon), or uncoated PTFE were used to substitute the medial rabbit meniscus, and were observed over a 3-month period. Joint mechanics were similar for sham-operation, meniscectomy, or substitution with a coated implant. Joints with an uncoated PTFE prosthesis tended to be less compliant and to show greater load dissipation during relaxation. Cartilage indentation showed softening with all alternatives compared to sham-operation. Ingrowth into the prosthesis periphery was best with the PTFE implants. Both types of co

Hydroxyapatite ceramic implants for cranioplasty in children: a retrospective evaluation of clinical outcome and osteointegration.

INTRODUCTION: Cranioplasty in children is a controversial and challenging issue, since there is still no consensus on the ideal material. Main problems in paediatric age are represented by the child's growing skull, the lower bone thickness and the high incidence of cerebrospinal fluid (CSF) disorders or brain swelling. Autologous bone is still considered the "gold standard". When it is not available, a wide range of alloplastic materials have been proposed. Hydroxyapatite, a ceramic-based derivative, bears a chemical composition very similar to the human natural bone, making this material a v

Asymptomatic chronic rhinosinusitis and osteitis in patients treated with multiple zygomatic implants: a long-term radiographic follow-up.

PURPOSE: The aim of this study was to investigate implant survival and the reactions of the bone and mucosa of the maxillary sinuses in patients treated with fixed dental prostheses supported by multiple zygomatic implants. MATERIALS AND METHODS: Between 2001 and 2007, patients with extensive resorption of the basal bone of the maxilla received multiple zygomatic implants without grafts. In 2012, the maxillary sinuses were examined with cone beam computed tomography. Neo-osteogenesis at the posterior wall of the sinus was considered to be a sign of osteitis and classified as mild (3 to 4 mm),

Prosthetic rehabilitation of a cleidocranial dysplasia patient with vertical maxillofacial deficiency: a clinical report.

Cleidocranial dysplasia (CCD) is a rare congenital disorder characterized by skeletal and dental anomalies. This clinical report describes the prosthodontic approach to treating a CCD patient who presented with decreased facial height and relative mandibular protrusion due to maxillary hypoplasia after orthodontic treatment. Functional and esthetic rehabilitation was achieved using telescopic detachable prostheses in the maxilla and osseointegrated implants and metal-ceramic fixed dental prostheses in the mandible. These treatment approaches precluded the need for orthognathic surgical correct

Total midface reconstruction after radical tumor resection: a case report and overview of the problem.

We report an unusual repair of a massive midface defect resulting from resection of a recurrent squamous cell carcinoma of the nasal vestibule. The defect included both maxillas, the hard palate, the upper lip, and all nasal and perinasal tissues. After treatment, reconstruction was accomplished using prostheses and autologous tissue transferred from local and distant sites. The osseous component of the transferred tissue permitted placement of osseointegrated implants for fixation of maxillary and nasal prostheses. The rationale for this reconstruction and the problems associated with midface

Local residual ridge augmentation with solid hydroxyapatite blocks: Part II--Correction of local resorption defects in 50 patients.

Replacement of anterior teeth with fixed or removable prostheses is often a compromise, because the resorbed residual ridge in the area of missing teeth cannot be ideally restored functionally and esthetically at the same time. To address this problem, 67 blocks of hydroxyapatite were placed subperiosteally to improve residual ridge resorption defects subsequent to loss of anterior teeth in 50 patients. The implants were evaluated clinically and radiographically for 6 months to 7 years after implantation. Results indicate that the suggested approach improves the esthetic results and the progno

Short Implants Versus Standard Implants: Midterm Outcomes of a Clinical Study.

INTRODUCTION: The aim of this study was to evaluate the midterm survival rate, marginal bone resorption (MBR), and stability of short implants and to compare the results with standard length implants. MATERIALS AND METHODS: A total of 38 patients were included. In total, 147 implants (Nucleoss Implants, Izmir, Turkey) were placed (86 short implants and 61 standard implants). Cement-retained metal-ceramic prostheses were fabricated. MBR was evaluated on periapical radiographs taken at implant placement, at the time of crown insertion and annually thereafter. The stability of the implants was e

What you don't know can hurt you (and your patients).

The goal of modern dentistry is to return patients to oral health in a predictable fashion. The partial and complete edentulous patient may be unable to recover normal function, aesthetics, comfort, or speech with a traditional removable prosthesis. The aesthetics of the edentulous patient are affected as a result of bone atrophy. Continued resorption leads to irreversible facial changes. An implant stimulates the bone and maintains its dimension in a manner similar to healthy natural teeth. As a result, the facial features are not compromised by lack of support. Implant prostheses often offer

Immediate Loading of Zygomatic Implants Using a Dual Scan Technique.

The immediate loading protocol has become increasingly popular due to the progressive growth in demand for a reduction in treatment times. The possibility of applying this protocol would depend on certain important factors. The application of the digital workflow mentioned in the protocol guarantees rapidity, precision, and esthetics. This report aims to describe a fully digital workflow using a dual scan impression technique to fabricate immediate fixed complete dentures (FCDs) for zygomatic and standard implants. A 58-year-old female patient requested treatment for her severely atrophic maxi

Single-step resection and reconstruction using patient-specific implants in the treatment of benign cranio-orbital tumors.

PURPOSE: The aim of this study was to evaluate surgical outcomes using patient-specific prostheses produced by computer-aided design and manufacturing for primary reconstruction in patients with benign cranio-orbital tumors. Polyetheretherketone was used to manufacture the implants. MATERIALS AND METHODS: The present study included 3 patients who underwent fronto-orbito-pterional craniotomy using individual custom-made surgical guides. Patient-specific polyetheretherketone prostheses were used for reconstruction during the same surgery. All patients underwent esthetic examination (facial and

Silicone rubber distal ulnar replacement arthroplasty.

We retrospectively reviewed 42 patients who underwent resection of the distal ulna with implantation of a silicone rubber ulnar head prosthesis (45 wrists). Two prostheses were used: the original Swanson prosthesis, and a prosthesis of our own design. Follow-up X-rays showed migration or breakage of 63% of the prostheses. No statistically significant correlation existed between the quality of functional outcome and the integrity of the prostheses. There was no significant difference between pre-operative and post-operative range of motion for the entire group or between patients with broken or

Grading of radiographic osteolytic changes after silastic metacarpophalangeal arthroplasty and a prospective trial of osteolysis following use of Swanson and Sutter prostheses.

The purpose of this study was to compare the incidence of radiographic osteolysis following insertion of 89 Swanson and 126 Sutter metacarpophalangeal implants in rheumatoid arthritis patients. The mean follow-up time in the two groups of patients was 57 (40-80) and 55 (36-79) months, respectively. This paper proposes a new method of classifying radiographic osteolysis. The remarkable number of osteolytic changes seen in the bones adjacent to MCP prostheses in this study would suggest that silastic prostheses should only be used when other surgical alternatives cannot be used and that long-ter

[Teeth as prostheses materials for reconstruction of disrupted ossicular chain].

Since 1988 we have routinely used prostheses made from teeth for ossicular reconstruction and have now done more than 400 implants. Short-time results with at least 1 year implantation were studied in a retrospective analysis of 319 cases with a review of 88 patients. Eighty percent of the implants were covered by an intact tympanic membrane, while in 10% the prosthesis was completely uncovered. This apparent perforation was not due to a material-related extrusion, but was caused by persistent ventilation dysfunction of the middle ear. This was associated with a progressive retraction of the t

Cementless total joint arthroplasty prostheses with titanium-alloy articular surfaces. A human retrieval analysis.

Surgical implants of titanium alloy (Ti-6Al-4V) have been found to be susceptible to abrasive wear, resulting in the well-known phenomenon of metallosis. In the presence of the acrylic debris generated by loose joint arthroplasty prostheses, this abrasive wear is greatly increased. However, the bone loss seen in these cases has generally been presumed to be due to the osteolytic effects of particulate acrylic, not the alloy debris, which has generally been assumed to be biocompatible. To examine the in vivo effects of titanium-alloy wear without the added presence of acrylic debris, a comprehe

[Implanting the edentulous jaws with "All-on-4" immediate reconstruction: a preliminary clinical observation].

OBJECTIVE: To evaluate the treatment outcome of the "All-on-4" immediate loading protocol via survival rate of the implants, survival rate of the prosthesis, marginal bone, postoperative complications and patient satisfaction. METHODS: In our study, 40 patients with 49 edentulous jaws (31 mandibles and 18 maxillae) were enrolled. Each jaw was restored by the shortened dental arch prosthesis supported by only 4 implants according to the All-on-4 protocol (All-on-4, Nobel Biocare AB, Goteborg, Sweden). For all the patients enrolled in the study, the loading was applied within 12 hours of surgery.

[Results of the GECO series].

301 hip prostheses were implanted in patients under the age of 50 years. All patients were followed for a minimal period of 3 years or until revision. The young age is due to the etiologies: femoral head necrosis, congenital hip dislocation, post-traumatic arthrosis. The implanted prostheses were of different types: both components were usually cementless; most bearing surfaces were alumina-polyethylene, with a head diameter of 32 or 28 mm. Patients were followed for 3 to 18 years after implantation (mean: 7 years). At the last follow-up, 59 prostheses (20%) had been exchanged; 232 cases had a

Fiber-reinforced composite fixed dental prostheses: two clinical reports.

Various options are available in clinical practice for the replacement of a single missing tooth, ranging from conventional fixed and removable dental prostheses to a single implant-supported crown. There are situations in which a semipermanent fixed dental prosthesis may be desirable, particularly for patients who have completed orthodontic treatment but are too young to embark on implant therapy. Following advances in fiber-reinforcement technology, fiber-reinforced composite resin (FRC) now represents a lower-cost alternative to traditional metal-ceramic for the construction of resin-bonded

Histopathology of ossicular implants.

Ossicular and cortical bone grafts maintain their morphologic structure for long periods of time and show varying amounts of replacement of nonviable bone by new bone through a slow process of creeping substitution. Cartilage grafts develop chondromalacia, lose stiffness, and tend to be resorbed over time. Plastipore prostheses elicit foreign body giant cell responses with microscopic biodegradation of the implants. There is a great need for the study of well-documented human temporal bone cases with in situ ossicular implants.
