Women with nonorganic psychosis: mental disturbance during pregnancy.

Rates of mental disturbance during pregnancy were studied in 88 index women with a history of nonorganic psychosis and 104 demographically similar control women. The sources of information were the woman's own report at interview during pregnancy, the interviewer's assessment and, for index cases, psychiatric record notations relevant to the pregnancy period. Highly significantly increased rates of active mental disturbance were identified for index women, as compared with controls, based both on the woman's own report and on the interviewer's assessment. Most disturbed were women with a histo

Lorazepam treatment of psychogenic catatonia: an update.

Six papers have reported the efficacy of lorazepam in alleviating catatonic symptoms that have psychogenic causes. The authors report five new cases of catatonia; three patients responded favorably and two failed to respond to lorazepam. Investigation of the nonresponders revealed the presence of pernicious anemia in one and postpartum psychosis in the other. These findings demonstrate that lorazepam has an important role in the treatment of catatonic symptoms. Its effects appear to be specific for psychogenic catatonic states and for conditions in which biogenic amines, specifically dopamine

Uncommon postpartum delayed hemolytic transfusion reaction caused by anti-C antibodies: A rare presentation without prior risk factors.

Delayed hemolytic transfusion reaction (DHTR) is a rare post-transfusion complication, typically involving extravascular hemolysis. We present an unusual case of postpartum DHTR caused by anti-C antibodies in a 35-year-old Japanese woman, gravida 2, para 1, with no history of transfusion, alloimmunization, or underlying hemoglobinopathy. The patient developed dark-colored urine on postpartum day 25, 12 days after receiving a blood transfusion for hemorrhage due to placenta accreta spectrum. Laboratory findings revealed indirect hyperbilirubinemia, elevated lactate dehydrogenase, low haptoglobi

Postpartum Stress Urinary Incontinence: Current Advances in Non-Pharmacological Therapies.

Stress urinary incontinence (SUI) is a prevalent postpartum condition that significantly impacts women's quality of life and mental health. This systematic review synthesizes current advances in non-pharmacological therapies for postpartum SUI, focusing on pelvic-floor muscle training (PFMT), electrical and magnetic stimulation, acupuncture, and other therapies like Pilates and breathing exercises. Our literature search in PubMed, Cochrane Library, and Web Science identified studies examining these interventions, which are crucial for treating SUI due to their safety and efficacy. PFMT, also k

Lasix for the prevention of de novo postpartum hypertension: a randomized placebo-controlled trial (LAPP Trial).

BACKGROUND: Birthing people with de novo postpartum hypertensive disorders continue to be among the populations at highest risk for severe maternal morbidity. Randomized controlled trials demonstrate a benefit of oral loop diuretics in decreasing postpartum hypertensive morbidity in patients with an antenatal diagnosis of preeclampsia. It is not known whether this same therapy benefits patients at risk for new-onset postpartum hypertension. OBJECTIVE: This study aimed to evaluate whether oral furosemide can reduce the risk for de novo postpartum hypertension among high-risk birthing people by

Peripartum Cardiomyopathy: Current Understanding of Pathophysiology, Diagnostic Workup, Management, and Outcomes.

Peripartum cardiomyopathy (PPCM) is a relatively rare, potentially life-threatening, idiopathic form of cardiomyopathy that affects previously healthy young women during late pregnancy or in the early postpartum period and is characterized by left ventricular systolic dysfunction in the absence of any other identifiable cardiac causes. Morbidity and mortality with PPCM are remarkably high and it continues to be one of the leading causes of maternal death. Although remarkable advances have been made in our understanding of PPCM in the last few decades, unanswered questions remain regarding its

ACOG Committee Opinion No. 767 Summary: Emergent Therapy for Acute-Onset, Severe Hypertension During Pregnancy and the Postpartum Period.

Acute-onset, severe systolic hypertension; severe diastolic hypertension; or both can occur during the prenatal, intrapartum, or postpartum periods. Pregnant women or women in the postpartum period with acute-onset, severe systolic hypertension; severe diastolic hypertension; or both require urgent antihypertensive therapy. Introducing standardized, evidence-based clinical guidelines for the management of patients with preeclampsia and eclampsia has been demonstrated to reduce the incidence of adverse maternal outcomes. Individuals and institutions should have mechanisms in place to initiate t
