Omental vascularized lymph node transplant for the treatment of breast lymphedema: A case report.

Breast lymphedema is a type of breast cancer related lymphedema that leads to significant discomfort and negative impact on body image. Conservative therapy and lymphovenous bypass have been previously described as possible treatment methods for breast lymphedema, however, a unified approach to treatment is lacking. The current report describes a case of breast lymphedema successfully treated with vascularized lymph node transfer (VLNT) after failed attempt at management with conservative therapy. The patient is a 48-year-old female with right-sided breast cancer who underwent breast conservat

Soft Tissue Reconstruction Using the Free Forearm Fillet Flap in the Salvage of Forequarter Amputations for Recurrent Sarcoma: A Report of 2 Cases.

Reconstruction after radical forequarter amputation for recurrent proximal extremity sarcoma is challenging because the defect is significant, and the axillary or subclavian vessels are resected with the tumor, indicating that the pedicle of nearby flaps is often cut off. Free flaps are commonly used to cover the defect, but the donor site morbidity is problematic. Another problem with resecting the axillary or subclavian vessels is difficulty obtaining recipient vessels with matching calibers for another free flap. The authors presented 2 cases that successfully solved all these problems by c…

Vascular considerations in composite midfacial allotransplantation.

BACKGROUND: Advances in microsurgery and immunosuppression have allowed for facial reconstruction at a qualitatively new level with facial c … te tissue transplantation of the midface. Careful graft harvest, appropriate selection of donor and recipient vessels, complete allograft revascularization, and restoration of sensory and motor function are critical to making face transplant surgery safe and functional.

Robotic Kidney Transplantation Under Regional Hypothermia.

The technique of robotic kidney transplantation (RKT) under regional hypothermia is being described. It may be performed in all patients requiring kidney transplantation with some relative contraindications. Unlike open transplantation, RKT is a transperitoneal procedure. Patient lies supine with low lithotomy and 15° to 20° reverse Trendelenburg position. Port placement is like standard radical prostatectomy with a multiport device placed at the umbilicus. The graft after harvest needs special care on the bench to ligate all vascular communications and placed in a gauze jacket for easy handli

Internal Maxillary Artery to Anterior Circulation Bypass with Local Interposition Grafts Using a Minimally Invasive Approach: Surgical Anatomy and Technical Feasibility.

BACKGROUND: The internal maxillary artery (IMA) is a reliable donor for extracranial-intracranial high-flow bypasses. However, previously described landmarks and techniques to harvest the IMA are complex and confusing and require extensive bone drilling, carrying significant neurovascular risk. The objective of our study was to describe a minimally invasive technique for exposing the IMA and to assess the feasibility of using the IMA as a donor for anterior-circulation recipient vessels using 2 different local interposition vessels. METHODS: Via a minimally invasive technique, the IMA was har…

Sequential selective laser photocoagulation of communicating vessels in twin-twin transfusion syndrome.

OBJECTIVE: We have previously described the selective laser photocoagulation of communicating vessels (SLPCV) technique for the treatment of twin-twin transfusion syndrome (TTTS). Because TTTS is thought to result from a net transfer of blood from the donor twin to the recipient twin, we hypothesized that lasering the arteriovenous anastomoses from the donor to the recipient (AVDRs) first (sequential SLPCV or SQLPCV) would result in an improved hemodynamic status and decreased likelihood of intrauterine fetal demise of the donor twin (IUFD-D). MATERIALS AND METHODS: The diagnosis of TTTS was

Long-term outcomes of venous graft preservation in a rat autologous transplant model.

Venous grafts remain the most used conduits in coronary artery bypass grafting (CABG) but are highly susceptible to early failure due to ischemia-reperfusion injury, intimal hyperplasia, and accelerated atherosclerosis. Specialized preservation solutions such as DuraGraft and TiProtec aim to protect endothelial integrity during intraoperative storage; however, their long-term effects on venous graft adaptation remain unclear. In this study, saline, DuraGraft, and TiProtec were compared in a rat model of arterialized autologous jugular vein interposition grafting. Male Wistar rats were assigned

Combination of 5-aminolevulinic acid and iron prevents skin fibrosis in murine sclerodermatous graft-versus-host disease.

Scleroderma or systemic sclerosis (SSc) is a clinically heterogeneous rheumatological autoimmune disease affecting the skin, internal organs and blood vessels. There is at present no effective treatment for this condition. Our study investigated the effects of 5-aminolevulinic acid (5-ALA), which is a precursor of haem synthesis, on graft-vs-host disease (GvHD)-induced SSc murine model. Lymphocytes were intravenously injected from donor mice (B10.D2) into recipient BALB/c mice (recombination-activating gene 2 (Rag-2)-null mice) deficient in mature T and B cells to induce sclerodermatous GvHD (

Self-contained diffuse optical imaging system for real-time detection and localization of vascular occlusions.

Free flap surgery is a procedure where healthy tissue is transferred from a donor site to a recipient site of the body to fill a defect without maintaining the original blood supply to the flap. The anastomosis of the vascular network of the flap to the blood vessels adjacent to the recipient site has associated risks of arterial and/or venous occlusions that must be promptly detected to avoid temporary or permanent tissue damage. In this work, we present a skin-contact diffusion optical imaging (DOI) system able to continuously provide a three-dimensional representation of the flap oxygenatio

Temporary Vascular Shunting to Prevent Excessive Ischemia Time for a Fillet-of-Leg Free Flap During External Hemipelvectomy: A Case Report.

The free fillet flap is a well-established reconstructive option following external hemipelvectomy. However, mitigating prolonged warm ischemia during the extensive tumor resection poses a significant challenge to flap viability. Presented herein is a case illustrating the novel use of temporary intraoperative vascular shunting to overcome this limitation. During hemipelvectomy for a 17.7 × 16.9 × 19.3 cm osteosarcoma of the proximal left femur in a 74-year-old man, unplanned early ischemia of the lower extremity was addressed by placing carotid shunts between the external iliac and popliteal

Anterior approach to posterior auricular artery in microvascular anastomosis - a new concept in replanting amputated ears - experience from four successful cases and literature review.

INTRODUCTION: Auricular amputation is rare but devastating to the patient's aesthetics and psychology. Only about 60 cases of successful microsurgical replantation were reported in the literature. There is currently no complete procedure for auricular replantation, including a recommended operating position. MATERIALS AND METHODS: Four cases of microsurgical auricular replantation from February 2012 to July 2022 were studied. Data included age, cause, duration of ischemia, patient position, sequence of vascular anastomosis, method of arterial/venous anastomosis/grafting, amount of blood trans

Clinical evidence of the absence of brain perfusion during thoraco-abdominal normothermic regional perfusion in donation after circulatory determination of death.

The use of thoraco-abdominal normothermic regional perfusion (TA-NRP) has been associated with improved graft utilization and superior recipient outcomes compared with rapid recovery in donation after circulatory determination of death (DCDD). A major barrier to its broader adoption is the concern for inadvertent brain reperfusion, which would invalidate the determination of death. We aimed to confirm that, during TA-NRP in DCDD, clamping and sectioning of the supra-aortic vessels, with venting the cephalad ends to atmospheric pressure, effectively prevents brain perfusion. A prospective study

Stabilization of Microsurgical Anastomosis Using Fat-piece Graft With Autologous Fibrin Glue in Autologous Breast Reconstruction.

Flap loss is a severe complication of autologous breast reconstruction. Most flap losses are caused by thrombosis of the anastomosed artery … nclude flap design, assessment using multidetector computed tomography, selection of perforator and recipient vessels, vascular anastomosis method, postoperative management, and so on. In addition to these factors, this technique, stabilization of vessels using a fat-piece graft and fibrin glue, may lead to fewer complications during autologous breast reconstructio …

Free Flap Transfer with Arteriovenous Loop Establishment for Upper Limb Salvage in a Crush Injury.

Constructing arteriovenous (AV) loops saves a crushed limb when healthy recipient vessels are unavailable at the site of injury by supporting a transferred free flap. Literature about using AV loops in the upper limb defect is scarce. We present a case with a devastating upper limb crush injury that was successfully managed with fibula flap transfer preceded by AV loop construction. A 38-year-old woman was admitted to our hospital with severe crushing trauma to her right forearm and hand. There was significant skin loss with injuries to blood vessels and tendons. She underwent multiple staged …

New trends in breast reconstruction.

BACKGROUND: Breast reconstruction (BR) after radical mastectomy is intended to recreate symmetrical natural-appearing breasts while preservi … forator flaps, with the DIEP flap being the leader. Internal mammary vessels are most often used as recipient vessels for anasthomoses. Immediate BR when indicated has better aesthetic results. In less well-developed countries, insurance companies do not cover implant expenses and the autologous procedures remain the best option.

Distally based free flap using retrograde arterial flow.

In lower extremity injuries, the major arteries are frequently damaged and the remaining artery may be located in the traumatized zone. Recently, vascular anastomoses to recipient vessels distal to the zone of injuries have been advocated in the lower limb reconstruction and flaps by using retrograde arterial flow are well established in island flaps. Twelve patients with soft-tissue defects below the knee underwent lower extremity reconstruction with distally based free flaps by using retrograde arterial flow. For assessment of retrograde flow, the intraoperative retrograde arterial pressure …

["Saphenous vascular loop" technique in the treatment of lower limb defects. Report of five cases].

Five patients with high energy trauma of the lower limb with tissue defect located in the knee or the proximal third of the leg underwent reconstruction with a free latissimus dorsi flap. This flap was connected to a vascular saphenous loop which initially creates an arterioveous shunt between proximal femoral vessels. This was performed in a single operation with two teams of surgeons. This technique was chosen because a healthy recipient pedicle was not available in the vicinity of the defect. Application of vein grafts is not the usual procedure but there are situations in which it becomes
