PURPOSE: Treatment strategies of rectal cancer differ between tumors located above (RS/Ra) and below (Rb) the peritoneal reflection. Based on the extent of distal spread (DS), the Japanese Society for Cancer of Colon and Rectum proposed an optimal distal margin in RS/Ra and Rb tumors. In this study, we investigated the clinical significance of DS between RS/Ra and Rb tumors.
METHODS: We analyzed 287 stage I-III rectal cancer patients who underwent curative intent resection without preoperative therapy. DS and other pathological factors were evaluated using whole-mount sections. To investigate

355 patients with hepatic metastases were followed between 1970 and 1984 at the Hôpital Paul Brousse. 132 cases were of colonic origin: 32 were treated by means of hepatic resection (including 5 metastasectomies), 7 by intra-hepatic anastomosis and 5 by devascularisation. The other 88 patients did not receive any surgical treatment. In 148 of the other 233 patients, the metastases were of non colonic gastro-intestinal or rectal origin and in the remaining 75 cases the metastases were derived from another site. 12 patients were treated by hepatic resection, 6 by devascularisation, 2 by intra-he

The treatment of locally advanced rectal cancer has been transformed over the last century. While initially treated with surgery alone, adjuvant therapy with chemotherapy and radiation became common for locally advanced disease to prevent local and distant recurrence. Several trials subsequently showed benefit to moving this therapy into the neoadjuvant setting which led to improved oncologic outcomes, greater compliance, and similar surgical morbidity. In this article, we will explore this historical transition to a total neoadjuvant approach that can lead to cure in select patients and bring
