Renal Resistive Index: not only kidney.

From growing data from the literature emerges that so-called ecoDoppler "Renal" Resistive Indexes (RRI) are not only specific markers of kidney damage and indicators of renal functional prognosis but they always express more clearly a strong link with the systemic circulation. In fact, some cardiovascular parameters such as aortic pulse pressure, aortic stiffness, abdominal aortic calcifications and clinical settings for instance heart failure progression have shown a great association with RRI. Relationships between renal and inflammatory indexes in hypertensive patients have been found and d

Sir George Johnson FRCP (1818-96), high blood pressure and the continuing altercation about its origins.

NEW FINDINGS: What is the topic of this review? The review takes a historical approach to examining where in the body it might be possible to identify the most common cause, or causes, of long-term hypertension. It gathers evidence from histology, human and animal physiology, and computational modelling. The burden of decades of controversy is noted. What advances does it highlight? The review highlights the distinctive pathology of the afferent renal circulation and what its consequences are for the widespread view that essential hypertension is caused by elevated peripheral vascular resistan…

Baclofen-Induced Encephalopathy in End Stage Renal Disease.

Baclofen is a highly used centrally acting GABA agonist that continues to be an effective therapy for spasticity and chronic hiccups. The renally dependent excretion determines the circulating concentrations and guides effective dosing to decrease adverse reactions. Caution should be considered in administering baclofen to patients with decreased renal function. We present a patient with end stage renal disease on hemodialysis with recent baclofen ingestion who presented with toxic encephalopathy that was resolved with additional dialysis sessions.

The finding of vascular and urinary anomalies in the harvested kidney for transplantation.

INTRODUCTION: In kidney transplantation, anatomical vascular and excretory anomalies may represent causes of failure. Today's surgical techniques have made the most of the organs with anatomic anomalies and iatrogenic injury successfully used for transplantation. MATERIALS AND METHODS: From January 2000 to June 2006, we harvested 230 kidneys, of including 88 kidneys (20%) with vascular, urinary, or vascular-urinary anomalies; 64 kidneys were implanted and 15 were sent to other transplantation centers. Only 9 kidneys were not appropriate for transplantation. RESULTS: All patients who received

[Renovascular hypertension in neurofibromatosis. Simultaneous occurrence of multiple abdominal and cerebral vascular abnormalities (author's transl)].

Vascular involvement in von Recklinghausen's neurofibromatosis has been known since the work of Reubi (1944, 1946). We found 72 cases of neurofibromatosis with renovascular hypertension reported in the literature. We have observed three female patients, of whom two had unilateral renal artery abnormalities consisting of stensoses and small aneurysms. The third patient, who had a right-sided nephrectomy for probable occlusion of the renal artery, had a coarctation of the abdominal aorta and complete occlusion of the left renal artery and of the coeliac-mesenteric trunk. There were, in addition,

Osteoglycin and Sclerostin Imbalance in Hypophosphatasia: Bone-Derived Markers of Mineralization and Systemic Involvement.

Hypophosphatasia (HPP) is a rare inherited disorder caused by deficient tissue-nonspecific alkaline phosphatase (TNSALP) activity and classically characterized by impaired mineralization processes, although growing evidence suggests broader systemic involvement beyond bone. This cross-sectional study aimed to characterize circulating levels of osteoglycin and sclerostin in patients with HPP and to explore their relationships with TNSALP activity and systemic clinical-biochemical profiles. This cross-sectional study included 25 genetically confirmed HPP patients and 25 age- and sex-matched cont

Acute renal failure secondary to myoglobinuria associated with Legionnaires' disease.

A case of nonfatal Legionnaires' disease was complicated by rhabdomyolysis, myoglobinuria, and acute nonoliguric renal failure. It was not determined whether the rhabdomyolysis was secondary to direct toxic effect of the organism or due to a circulating factor causing muscle necrosis. This case provides additional evidence that rhabdomyolysis with subsequent renal failure may be a serious complication of Legionnaires' disease.

Renal transplantation in patients with microscopic polyarteritis and antimyeloperoxidase antibodies: report of three cases.

This paper reports on 3 patients on renal dialysis for crescentic glomerulonephritis associated with microscopic polyarteritis (MPA) and antineutrophil cytoplasmic autoantibodies specific for myeloperoxidase (MPO-ANCAs). They successfully underwent renal transplantation from a cadaver donor 6-63 months after the onset of the disease, despite the persistence of antibodies at high titer. A triple immunosuppressive regimen including steroids, cyclosporin and azathioprine was used. One patient underwent transplantectomy for surgical complications 3 months later, while the serum creatinine was 2.0

Addition of terlipressin to norepinephrine in septic shock and effect of renal perfusion: a pilot study.

PURPOSE: Terlipressin improves renal function in patients with septic shock. However, the mechanism remains unclear. Here, we aimed to evaluate the effects of terlipressin on renal perfusion in patients with septic shock. MATERIALS AND METHODS: This pilot study enrolled patients with septic shock in the intensive care unit of the tertiary hospital from September 2019 to May 2020. We randomly assigned patients to terlipressin and usual care groups using a 1:1 ratio. Terlipressin was intravenously pumped at a rate of 1.3 μg/kg/hour for 24 h. We monitored renal perfusion using renal contrast-enh

A Novel Role of Semaphorin 3C in Modulating Systemic and Renal Hemodynamics.

BACKGROUND: Alterations of renal hemodynamics play an essential role in renal homeostasis and kidney diseases. Recent data indicated that semaphorin 3C (SEMA3C), a secreted glycoprotein involved in vessel development, can modulate renal vascular permeability in acute kidney injury, but whether and how it might impact systemic and renal hemodynamics is unknown. OBJECTIVES: The objective of the study was to explore the effect of SEMA3C on systemic and renal hemodynamics. METHODS: SEMA3C recombinant protein was administered intravenously in two-month-old wild-type mice, and the variations of me

Renal tubulointerstitial hypoxia: cause and consequence of kidney dysfunction.

1. Intrarenal oxygen availability is the balance between supply, mainly dependent on renal blood flow, and demand, determined by the basal metabolic demand and the energy-requiring tubular electrolyte transport. Renal blood flow is maintained within close limits in order to sustain stable glomerular filtration, so increased intrarenal oxygen consumption is likely to cause tissue hypoxia. 2. The increased oxygen consumption is closely linked to increased oxidative stress, which increases mitochondrial oxygen usage and reduces tubular electrolyte transport efficiency, with both contributing to i

Pathogenesis of vascular calcification in chronic kidney disease.

Pathogenesis of vascular calcification in chronic kidney disease. Background. Hyperphosphatemia and hypercalcemia are independent risk factors for higher incidence of cardiovascular events in patients with chronic kidney disease. In addition to increased calcium-phosphate product, hyperphosphatemia accelerates the progression of secondary hyperparathyroidism with the concomitant bone loss, possibly linked to vascular calcium-phosphate precipitation. Results. The control of serum phosphate levels reduces vascular calcification not only by decreasing the degree of secondary hyperparathyroidism a

The role of medullary ischemia in acute renal failure.

The introduction of new techniques for the determination of renal parenchymal oxygenation and intrarenal microcirculation has elucidated some important aspects in the pathophysiology of acute renal failure (ARF). Data accumulated over the last decade with these techniques, together with improved morphologic evaluation of the kidney, indicate that medullary damage may play a pivotal role in various forms of acute and chronic renal hypoxic and toxic insults. The outer medulla functions normally under hypoxic conditions, as a result of limited regional oxygen supply and high oxygen consumption fo

Effects of alpha-adrenoceptor blockers on renal function and blood pressure adjustment in human hypertension.

In this paper the different aspects of the role played by alpha-adrenoceptors in the control of renin secretion from the juxtaglomerular apparatus and renal sodium and water reabsorption, and the effects of alpha-adrenoceptor antagonists on systemic haemodynamics, will be investigated. Animal experiments suggest that the renal alpha-adrenoceptors exert a restraining action on renin secretion while increasing tubular reabsorption of sodium and water. A recent study in man has confirmed the alpha-adrenoceptor-mediated inhibition of renin secretion. Previously available ganglion blocking and anti

Venous digital subtraction angiography in the evaluation of renovascular hypertension.

The study of renovascular hypertension (R.V.H.) presently requires multiple non invasive examinations in order to select between patients with R.V.H. or other kind of hypertension, before resorting to angiography. The use of venous digital subtraction angiography (V.D.S.A.) may change this diagnostic flow-chart. For this purpose, 100 patients with clinical and laboratory data suspect of R.V.H. underwent V.D.S.A. Compared to angiography, V.D.S.A. showed a 100% sensitivity and 93% specificity. Since the sensitivity and specificity of the non invasive techniques vs. V.D.S.A. in the same series wa

Filtration fraction: an index of renal disease activity in patients with systemic lupus erythematosus.

Renal plasma flow (RPF) and glomerular filtration rate (GFR) were estimated by 125I hippuran and 51Cr EDTA clearances using a single shot technique on two occasions at one-year intervals in 22 patients fulfilling the ARA criteria for systemic lupus erythematosus (SLE). All these patients had histologically proven renal disease. Filtration fraction was a better parameter than proteinuria, urinary sediment or GFR for recognising diffuse proliferative glomerulonephritis with a sensitivity of 61 per cent and a specificity of 88 per cent. After one year all the patients with an initially low filtra

[Subclinical glomerulonephritis with dense deposits in the course of partial lipodystrophy (author's transl)].

In a young woman with partial lipodystrophy accompanied by hypocomplementemia related to circulating C3NF activity, renal biopsy revealed dense deposits in the basement membranes of the glomerular capillaries without any evidence of urinary syndrome. This early form was not revealed by light microscopy and was confirmed only by ultrastructural examination. The morphological characteristics are similar to those occurring in renal grafts. The authors point out that subclinical forms of glomerulonephritis must be considered in cases of lipodystrophy and hypocomplementemia.

Soluble PD-1 drives renal fibrosis in CKD by disrupting immune homeostasis: Therapeutic mitigation via a targeted sPD-1 sequestration strategy.

AIMS: Renal interstitial fibrosis (RIF) drives chronic kidney disease (CKD) progression, with elevated soluble PD-1 (sPD-1) exacerbating chronic inflammation. This study aims to elucidate the pathogenic role of sPD-1 in RIF and evaluate "Exo-PD1"-a novel engineered extracellular vesicle strategy designed to sequester circulating sPD-1-as a targeted therapeutic intervention to mitigate renal fibrosis. MATERIALS AND METHODS: We analyzed serum and renal biopsy samples from clinical CKD cohorts and three distinct mouse models-unilateral ureteral obstruction (UUO), unilateral ischemia-reperfusion

CKD-induced wingless/integration1 inhibitors and phosphorus cause the CKD-mineral and bone disorder.

In chronic kidney disease, vascular calcification, renal osteodystrophy, and phosphate contribute substantially to cardiovascular risk and are components of CKD-mineral and bone disorder (CKD-MBD). The cause of this syndrome is unknown. Additionally, no therapy addresses cardiovascular risk in CKD. In its inception, CKD-MBD is characterized by osteodystrophy, vascular calcification, and stimulation of osteocyte secretion. We tested the hypothesis that increased production of circulating factors by diseased kidneys causes the CKD-MBD in diabetic mice subjected to renal injury to induce stage 2
