Facial diplegia, pharyngeal paralysis, and ophthalmoplegia after a timber rattlesnake envenomation.

The timber rattlesnake, also known as Crotalus horridus, is well known to cause significant injury from toxins stored within its venom. During envenomation, toxic systemic effects immediately begin to cause damage to many organ systems including cardiovascular, hematologic, musculoskeletal, respiratory, and neurologic. One defining characteristic of the timber rattlesnake is a specific neurotoxin called crotoxin, or the "canebrake toxin," which is a potent β-neurotoxin affecting presynaptic nerves that can cause paralysis by inhibiting appropriate neuromuscular transmission. We present an unus

[Delayed repair for blunt injury of the thoracic aorta complicated with the spinal cord injury; report of a case].

Blunt aortic injury is a major cause of death from blunt trauma and it generally accompanies with the other multiple serious injuries. A 67-year-old man sustained a blunt aortic injury and a fracture of 5th cervical vertebrae complicated with a spinal cord injury. A neurological examination showed a complete paralysis of the limbs. However, his respiratory status and circulatory status were stable. On admission, the paralysis improved resulting from high dose steroid injection and a cervical traction. An orthopedic operation was required before a cardiovascular operation even though computed t

Vocal cord paralysis as a consequence of peritonsillar infiltration with bupivacaine.

Reduction of postoperative pain is an important goal in the perioperative management of tonsillectomy patients. This is particularly the case for children, who often exhibit resistance to intramuscular or rectal administration of drugs. Peritonsillar bupivacaine infiltration, a relatively safe method of pain control, is in some centers frequently used by otolaryngologists for pain relief. We present the case of a 5-year-old girl who developed bilateral vocal cord paralysis following preoperative peritonsillar bupivacaine infiltration. After an uneventful tonsillectomy and extubation, stridor a

[Meningoradiculitis after tick bites. Apropos of 9 cases].

The authors report a series of 9 cases of meningoradiculitis after tick bites and compare them with 56 other cases in the literature. Clinically, the bite is followed by a free interval, then more or less extensive local erythema, pain then paralysis is undoubtedly the most typical presentation. Uni- or bilateral paralysis of the 7th cranial nerve was found in more than 50% of cases. Erythema may exceptionally be absent. There may be forms with pain alone. Finally, there may sometimes be pyramidal signs or signs of brain irritation. As concerns the course, one should note the absence of respir

Outcomes of Extracorporeal Removal in Acute Barium Poisoning: A Scoping Review.

Acute barium poisoning, though rare, can cause severe hypokalemia, respiratory paralysis, and life-threatening arrhythmias. Conventional treatments, including potassium replacement, may not rapidly resolve severe toxicity. Extracorporeal removal therapies (ECT), such as hemodialysis (HD) and continuous renal replacement therapy (CRRT), have been used to accelerate barium elimination, but their overall clinical outcomes remain unclear. This scoping review systematically summarizes current literature on the clinical outcomes of ECT in acute barium poisoning. We searched Embase, Ovid MEDLINE, and…

Late-Onset Hypokalemic Periodic Paralysis in an Adult Female With Type 2 Renal Tubular Acidosis: A Case Report.

Proximal renal tubular acidosis (type 2 RTA) is a metabolic disorder characterized by an inability of the proximal renal tubules to reabsorb bicarbonate, resulting in excessive urinary loss of bicarbonate. In return, this causes a standard anion gap metabolic acidosis with aberrant renal acidification, culminating in excessive urinary potassium loss and hyperchloremic metabolic acidosis. Several sources can induce potassium deficiency, ranging from slight abnormalities in potassium homeostasis to catastrophic and occasionally lethal circumstances. Hypokalemic periodic paralysis (HPP) manifests

[Establishment of a Nomogram model for individualized prediction of the risk of acute spinal cord injury complicated with respiratory dysfunction].

OBJECTIVE: To analyze the risk factors of acute spinal cord injury complicated with respiratory dysfunction, and to construct the clinical prediction model of acute spinal cord injury complicated with respiratory dysfunction. METHODS: Continuous 170 cases of acute spinal cord injury treated from April 2019 to October 2022 were retrospectively collected, and clinical data were uniformly collected. Patients were divided into respiratory dysfunction group 30 cases and non-respiratory dysfunction group 140 cases according to whether they had respiratory dysfunction during treatment. The predictiv

[Bilateral vocal cord paralysis with acute airway obstruction and urgent tracheotomy in a patient with an acute vertebrobasilar stroke].

INTRODUCTION: Vertebrobasilar stroke can be a diagnostic challenge. Bilateral vocal cord paralysis is very rare as a manifestation of acute stroke, yet it is potentially life-threatening because of the possibility of acute airway obstruction. No cases of bilateral vocal cord paralysis have been reported as a presenting symptom of acute stroke of mixed central and peripheral neurological aetiology. CASE REPORT: An 88-year-old woman with dysphonia resulting from paralysis of the right vocal cord following a thyroidectomy presented with sudden onset of vertigo, dysmetria and mild dysarthria (Nat

Takotsubo-like Myocardial Dysfunction in a Patient with Botulism.

Botulinum toxin A (BTXA) can disrupt the neuromuscular and autonomic functions. We herein report a case of autonomic system dysfunction that manifested as Takotsubo-like myocardial dysfunction in a patient with botulism. Takotsubo syndrome results in acute cardiac insufficiency, another fatal complication of botulism in addition to respiratory muscle paralysis, particularly in patients with cardiovascular disease.

Muscular paralysis and ventilatory failure caused by hyperkalaemia.

We report the history of a diabetic man presenting with hyperkalaemia and rapidly progressing muscular paralysis with severe respiratory embarrassment. His symptoms resolved rapidly with reduction of the serum potassium concentration. Similar cases have been reported previously but this is a problem that few will have encountered and is a diagnosis which should not be overlooked before artificial ventilation is commenced.

Rare laryngeal anomaly or spontaneously drained bilateral saccular cyst?

Laryngeal malformations are usually present at birth. Common anomalies include laryngomalacia, vocal fold paralysis, and subglottic stenosis; less common are congenital saccular cysts, laryngeal webs and atresias, and laryngeal lymphangiomas. Symptoms may range from stridor and hoarseness to respiratory distress. In some cases, patients are asymptomatic. Here, we present a case with abnormal laryngeal ventricles.

[Two types of diaphragmatic plication; therapeutic plication and prophylactic plication].

Plication of the hemidiaphragm is effective to relieve respiratory symptoms such as exertional dyspnea and orthopnea of the patients with unilateral diaphragmatic paralysis. We have experiences of 2 types of diaphragmatic plication;one for the symptomatic patients with unilateral diaphragmatic paralysis, and the other for the patients who underwent phrenicotomy during extended surgery for a recurrent thymoma. The former plication was performed for a therapeutic purpose to alleviate the patient's symptoms. The latter was performed for a prophylactic purpose to keep the patient free from possibl

Neuromuscular disease and respiratory failure.

Neurologists should be able to anticipate and recognise the onset of respiratory failure in patients with neuromuscular disorders. Symptoms will differ depending on the speed of onset of the respiratory muscle weakness. Careful monitoring of respiratory function is particularly important in acute disorders such as Guillain-Barré syndrome. Patients with an unrecognised neuromuscular disorder may occasionally present with respiratory failure. Important investigations include vital capacity, mouth pressures, arterial blood gases, chest x ray and sometimes overnight respiratory monitoring. Patient

Management of Neuromuscular Blocking Agents in Critically Ill Patients with Lung Diseases.

The use of neuromuscular blocking agents (NMBAs) is common in the intensive care unit (ICU). NMBAs have been used in critically ill patients with lung diseases to optimize mechanical ventilation, prevent spontaneous respiratory efforts, reduce the work of breathing and oxygen consumption, and avoid patient-ventilator asynchrony. In patients with acute respiratory distress syndrome (ARDS), NMBAs reduce the risk of barotrauma and improve oxygenation. Nevertheless, current guidelines and evidence are contrasting regarding the routine use of NMBAs. In status asthmaticus and acute exacerbation of c

Laser microsurgical bilateral posterior cordectomy for the treatment of bilateral vocal fold paralysis.

OBJECTIVES: We performed a prospective study to assess respiratory function and voice quality before and after laser microsurgical bilateral posterior cordectomy performed for chronic airway obstruction in patients with bilateral vocal fold paralysis. METHODS: In 17 patients a laser microsurgical posterior cordectomy was performed as an immediate bilateral approach. Roughness, breathiness, hoarseness, and dyspnea were evaluated both subjectively (on a scale from 0 to 3) and objectively (body plethysmography, computerized voice analysis: Göttingen Hoarseness Diagram). RESULTS: After laser sur

Acute Respiratory Failure Associated with Severe Obstructive Sleep Apnea as an Early Manifestation of Multiple System Atrophy: A Case Report.

Sleep-related breathing disorders are common in patients with multiple system atrophy (MSA), typically presenting as laryngeal stridor due to vocal cord abductor paralysis; however, severe respiratory impairment in the early stage is rare. We report the case of an 81-year-old woman with MSA who developed acute respiratory failure associated with severe obstructive sleep apnea syndrome (OSAS) preceding stridor. The patient required intubation. Although extubation was successful after six days, nocturnal hypercapnia persisted, necessitating noninvasive ventilation. Polysomnography confirmed the

A puzzling cause for bilateral vocal cord palsies in a patient with relapsed metastatic nasopharyngeal carcinoma.

Unilateral vocal cord paralysis (VCP) is a frequent complication of a variety of cancers. It is easy to diagnose and not fatal. Bilateral VCP is rare and it is fatal if not appropriately managed. We encountered a patient with nasopharyngeal carcinoma who developed bilateral VCP with hoarseness and respiratory distress 2 years after initial diagnosis. A tracheostomy was performed to restore airway patency.

Cervical anterior spinal artery syndrome associated with cardiopulmonary arrest.

Nontraumatic spinal cord ischemia is uncommon, especially when the cervical cord is involved. We present an elderly man who sustained acute occlusion of the anterior spinal artery at a high cervical level. This was followed by a respiratory arrest due to the paralysis of the diaphragm and chest wall muscles. A review of the vascular supply to the cord and of nontraumatic ischemic myelopathy is provided.
