INTRODUCTION: Agitation management in delirious patients is crucial in a crowded emergency department (ED) for both patient and personnel safety. Benzodiazepines, antipsychotics, and newly derived ketamine are among the most commonly used drugs in controlling these cases. This study aimed to compare the effectiveness of haloperidol-midazolam with haloperidol-ketamine combination in this regard.
METHODS: In this double-blind randomized clinical trial, delirious patients with agitation in ED were randomly assigned to a group: group A: haloperidol 2.5 mg IV and midazolam 0.05 mg/kg IV or group B

Nutritional rehabilitation and weight restoration remain key components of the treatment calculus for individuals suffering with anorexia nervosa (AN). While voluntary oral food intake has long been the most frequently used and preferred method to accomplish this, it is not uncommon for these individuals occasionally to require alternative modes of refeeding, such as tube feeding or even intravenous feeding (total parental nutrition; TPN). In addition, there are definite, albeit rare, indications to revert to involuntary feedings and even restraints if the medical condition becomes unstable du

UNLABELLED: Parents of 40 patients with Lesch-Nyhan disease completed a questionnaire detailing developmental history, life course, management, medication, factors influencing variability and topography of self-injury. Several conclusions were reached. Characteristics: Biting was the predominant form, perhaps only because of the difficulty of preventing it. There was considerable variability in self-injury which was strongly related to stress rather than to operant influences. Even though patients could not inhibit self-injury they could predict it and request restraints. Aggression against ot

Exposure to unpredictable chronic stress has a significant impact on physical and mental health. We have previously reported that a chronic unpredictable movement restraint (CUMR) paradigm increased immobility in the forced swim test and hippocampal pro-inflammatory cytokines. In the present study, we assessed CUMR impact on social motivation and neurobiological underpinnings. Adult male rats were grouped as either control or CUMR and subjected to 2, 4, or 6 h of restraint/day for 3 weeks. Thereafter, social behaviours (social investigation and social interaction) and synaptophysin, serotonin,

OBJECTIVE: To quantify quality of care following an admission to a nursing home with low or high antipsychotic drug use.
BACKGROUND: Misuse of antipsychotics in U.S. nursing homes is a huge concern for policymakers.
METHODS: We utilized an instrumental variable approach to estimate the effect of facility-level antipsychotic use on patient outcomes. The instrument was the differential distance to the nearest low-use antipsychotic nursing home relative to the nearest high-use antipsychotic nursing home. Post-acute care short-stay and long-stay residents in U.S. nursing homes were identified us

Chronic physical restraint stress increases oxidative stress in the brain, and dysregulation of oxidative stress can be one of the causes of major depressive disorder. To understand the underlying mechanisms, we undertook a systematic proteomic analysis of hippocampus in a chronic restraint stress mouse model of depression. Combining two-dimensional gel electrophoresis (2D-PAGE) for protein separation with nanoUPLC-ESI-q-TOF tandem mass spectrometry, we identified sixty-three protein spots that changed in the hippocampus of mice subjected to chronic restraint stress. We identified and classifi

BACKGROUND: People admitted to psychiatric hospitals with a diagnosis of schizophrenia may display behavioural problems. These may require management approaches such as use of coercive practices, which impact the well-being of staff members, visiting families and friends, peers, as well as patients themselves. Studies have proposed that not only patients' conditions, but also treatment environment and ward culture may affect patients' behaviour. Seclusion and restraint could possibly be prevented with staff education about user-centred, more humane approaches. Staff education could also increa

The treatment and care for severe psychiatric symptoms associated with dementia with Lewy bodies is challenging. This is especially true for elderly patients because the use of antipsychotics is associated with an attendant mortality risk. In this article, dementia patients with Lewy bodies who presented with severe psychiatric symptoms such as Capgras syndrome (delusional misidentification syndrome), are described, and pharmacological and non-pharmacological strategies to address these symptoms are discussed. Measures to be avoided include antipsychotic administration and physical restraint,

Transporting acutely psychotic patients is hazardous because of the risks they present to themselves, escorting staff, and aircraft. Various strategies have been proposed, usually involving combinations of sedating drugs and physical restraint. Thus far, none guarantees safe retrieval while completely mitigating risks. This case proposes the use of propofol as an alternative to more traditionally used agents. An infusion facilitated the uneventful and safe retrieval of a patient who had demonstrated resistance and tolerance to other drugs. Discussion is also presented on the potential utility

Agitation immediately after electroconvulsive therapy (ECT) is a common clinical problem. This can vary in severity from mild to severe and may necessitate the administration of sedating medications and at times physical restraint. There are many factors involved in the etiology of post-ECT agitation. One factor, the choice of anesthetic agent, is described in this case. Here, etomidate was associated with significant post-ECT agitation after several uneventful treatments with methohexital. The patient had no further episodes of post-ECT agitation after being switched back to methohexital.

A simple surgical technique for implanting chronic electroencephalographic electrodes in the skull of the dog was devised to obtain electroencephalograms from nonsedated and nonanesthetized dogs frequently. Orthopedic screws were used as implant electrodes and for securing the recording assembly over the frontal sinus. The electrode assemblies have been tolerated well for as long as 10 months in 17 of 20 dogs and have given reliable and satisfactory electroencephalograms for computer analysis and have been virtually free of all physiologic and motion artifacts in all dogs, with minimal use of

OBJECTIVE: Major Depressive Disorder (MDD) is a psychiatric disorder that has a tight connection to stressful experiences, decreased levels of endogenous antioxidants and enhanced levels of oxidative stress. We drafted this research to define the results of combining agmatine and melatonin on stress-induced depression in mice.
METHODS: Experimental groups included the non-stressed group treated with vehicle (ethanol at a concentration of 0.0005%), stressed vehicle (ethanol at a concentration of 0.0005%)-treated group, group treated with fluoxetine (10 mg/kg/day), group treated with melatonin

BACKGROUND: Pruritus is a prevalent condition that significantly impairs quality of life. Although emotional stress is known to exacerbate pruritus in individuals with skin diseases, its effect on spontaneous scratching behavior in otherwise healthy individuals remains poorly understood. Recent evidence suggests that stress itself may trigger scratching behavior independent of any pruritogenic stimulus. We aimed to determine whether acute stress induces spontaneous scratching in the absence of an externally applied pruritogen and to identify which stressor types are responsible.

BACKGROUND: Stress is closely related to life, and it can also cause many mental disorders. However, there are significant sex differences in neuropsychiatric disorders associated with stress, particularly in depression, where the lifetime risk of depression in women is approximately twice that of men. However, the specific mechanism of this process has not been explained in detail.
METHODS: Chronic restraint stress (CRS) + chronic and unpredictable mild stress (CUMS) was used to simulate social stress, and behavioral experiments, HE staining of rectal and hippocampal pathological sections, d

Mu opioid receptors (MORs) represent a vital mechanism related to the modulation of stress-induced analgesia (SIA). Previous studies have reported on the gamma-aminobutyric acid (GABA)ergic "disinhibition" mechanisms of MORs on the descending pain modulatory pathway of SIA induced in the midbrain. However, the role of the MORs expressed in the medial prefrontal cortex (mPFC), one of the main cortical areas participating in pain modulation, in SIA remains completely unknown. In this study, we investigated the contributions of MORs expressed on glutamatergic (MORGlut) and GABAergic (MORGABA) neu

Regular use of vibrating hand tools results in cold-induced vasoconstriction, finger blanching, and a reduction in tactile sensitivity and manual dexterity. Depending upon the length and frequency, vibration induces regeneration, or dysfunction and apoptosis, inflammation and an increase in reactive oxygen species (ROS) levels. These changes may be associated with mitochondria, this study examined the effects of vibration on total and functional mitochondria number. Male rats were exposed to restraint or tail vibration at 62.5, 125, or 250 Hz. The frequency-dependent effects of vibration on mi

In fish, as well as in other vertebrates, contrasting suites of physiological and behavioral traits, or coping styles, are often shown in response to stressors. However, the magnitude of the response (i.e. stress responsiveness) has been suggested to be independent of stress coping style. One central neurotransmitter that has been associated with both stress responsiveness and differences in stress coping styles is serotonin (5-hydroxytryptamine, 5-HT). In this study, we investigated to what extent stress responsiveness reflects differences in stress coping, and the potential involvement of th
