Two elderly patients were noted to have unilateral central retinal arterial narrowing with cilioretinal arterial collaterals supplying the bulk of the flow to the inner retina. One patient had excellent acuity and normal pupillary reactions; the other had marked impairment of vision and an afferent pupillary defect. Both patients were asymptomatic. We suggest that these patients had gradual or partial central retinal arterial occlusion which allowed the development of a collateral arterial supply from the ciliary system.

The effectiveness of thrombolytic agents used in local infusion has been demonstrated in the treatment of acute myocardial infarctions by several randomized trials. Thrombolytic therapy seems also to improve the prognosis of certain acute life-threatening brain stem strokes. Two cases of central retinal artery occlusion were treated with intracarotid urokinase injections. Visual acuity was improved in both cases in less than 24 hours, repermeation of the occluded vessels was observed 48 hours after the beginning of treatment. The limits of this kind of therapy are emphasized: there must be no …

The most important rheologic parameters were examined in six patients with acute occlusion of the central retinal artery. Blood samples were taken as part of the initial clinical examination on the day of the occlusion before the onset of therapy, and the following parameters were studied: (1) yield shear stress; (2) hematocrit; (3) plasma viscosity; (4) extent of erythrocyte aggregation; (5) erythrocyte deformability; (6) plasma fibrinogen concentration. A definite shear stress could be measured in all patients, something not possible in rheologically healthy volunteers. Hematocrit values wer…

We report a case of central retinal artery occlusion in an 18-year-old black woman with sickle-trait haemoglobinopathy and acute glaucoma after hyphaema. The central retinal artery occlusion occurred immediately after treatment of the glaucoma with osmotic agents, raising the possibility that they played a precipitating role. We suggest that osmotic agents be used with extreme caution in sickle patients with glaucoma. The occlusion was treated by anterior chamber paracentesis with eventual return of good vision. The reversibility of retinal and optic nerve function after total ischaemia is dis…

Acute retinal necrosis normally occurs at the periphery retina and gradually merges and progresses to the posterior pole. Optic neuropathy and central retinal artery occlusion as initial manifestation is very rare. We report the case of a patient with optic neuropathy and central retinal vessels as the first manifestations of acute retinal necrosis. Antiviral drugs, corticosteroids, and drugs that improve blood circulation were given. The necrotic retina and swollen optic disc disappeared gradually. However, the final vision of this eye declined to no light perception. From the first case repo…

OBJECTIVE: To study aberrant filling of the retinal vessels on fluorescein angiography (FA) images in patients with branch (BRAO) or hemi-central retinal artery occlusions (hemi-CRAO) and correlate with associated changes on optical coherence tomography (OCT).
METHODS: Cases of acute BRAO or hemi-CRAO having FA and OCT images at presentation between June 2017 to May 2022 were included. Comparisons were made between the cases with and without aberrant filling of the retinal vessel.
RESULTS: Thirty-nine eyes of 39 patients with acute retinal artery occlusions underwent FA and OCT imaging. Seve…

Optical coherence tomography (OCT) provides not only morphological information but also information about layer-specific optical intensities, which may represent the underlying tissue properties. The purpose of this study is to quantitatively investigate the optical intensity of each retinal layers in central retinal artery occlusion (CRAO). Twenty-nine CRAO cases at acute phase and 33 normal controls were included. Macula-centered 3D OCT images were segmented with a fully-automated Iowa Reference Algorithm into 10 layers. Layer-specific mean intensities were determined and compared between th…

PURPOSE: The purpose of this study was to determine the efficacy and safety of selective intra-arterial thrombolysis in patients with central retinal artery occlusion (CRAO).
METHODS: Medical records for 44 eyes of 44 patients diagnosed with acute non-arteritic CRAO and thrombolysis between October 2010 and February 2019 were analyzed retrospectively. Based on visual acuity, fundoscopic findings, and fluorescein angiography, the patients were classified into three stages: incomplete, subtotal, and total. The perfusion state using the best-corrected visual acuity (BCVA), arm to retina time, an…

PURPOSE: To report a case of central retinal artery (CRA) occlusion secondary to prepapillary loop in a 13-year-old girl.
METHODS: A 13-year-old girl presented with a history of sudden visual loss in her left eye.
RESULTS: Fundus examination confirmed thrombosis in a prepapillary arterial loop causing CRA occlusion and extensive retinal ischemia. Macular region was watered by an anomalous macular branch, which explained her 20/20 vision central vision.
CONCLUSION: Congenital prepapillary vascular loops are rare and usually asymptomatic. We report a case of central artery occlusion confirmed…

BACKGROUND Central retinal artery occlusion (CRAO) is an ophthalmic emergency and a retinal ischemic stroke. Cilioretinal arteries may remain perfused during CRAO because they arise from the posterior ciliary circulation. Most reported cilioretinal sparing involves papillomacular or foveal preservation; nasal sparing is atypical. We report CRAO with nasal retinal sparing and spontaneous head-turn behavior during screening. CASE REPORT A 55-year-old man presented with a 2-day history of sudden, painless vision loss in the right eye. He had newly diagnosed hypertension and was an active smoker. …

PURPOSE: Report of a case of central retinal artery occlusion after sphenopalatine artery (SPA) embolization. A review of the literature describing complications after SPA embolization and ligation.
METHODS: Retrospective case report and literature review.
RESULTS: A 68-year-old woman with a history notable for hypertension, hyperlipidemia, and 8 years of intermittent right-sided epistaxis previously requiring blood transfusions underwent right-sided embolization of the SPA with the neurosurgery service. Sphenopalatine artery embolization was achieved with Embosphere microspheres and Concert…

Coronary artery disease (CAD) remains one of the leading causes of mortality in the UK. The growing and ageing population has led to increasing CAD prevalence and escalating healthcare costs. Current diagnostic tools like coronary angiography are too invasive for large population-level screening. Retinal artery microaneurysms (RAMs) are caused by microvascular damage and are detectable via non-invasive eye screening. This raises the possibility of a novel screening tool to help identify individuals at risk of CAD. This systematic review investigates whether RAMs are a reliable predictor of CAD…

BACKGROUND Central retinal artery occlusion (CRAO) is the occlusion of the central retinal artery resulting in retinal infarction and acute vision loss. Digital subtraction angiography (DSA)-guided superselective ophthalmic artery or selective carotid thrombolysis remains the preferred treatment method for CRAO. This study aimed to evaluate the safety and clinical efficacy of the novel ophthalmic artery branch retrograde thrombolytic intervention for CRAO. MATERIAL AND METHODS Fifty patients with monocular CRAO were enrolled, including 28 males and 22 females (mean age: 55.7±2.3 years). The pa…
