We describe 2 cases of idiopathic retroperitoneal fibrosis presenting with back pain. Additional features included weight loss, constitutional symptoms and elevated erythrocyte sedimentation rate (ESR). One patient had sacroiliitis and was HLA-B27 positive. A further finding was obstruction of the superior mesenteric artery which has not been previously described in retroperitoneal fibrosis. The importance of considering idiopathic retroperitoneal fibrosis in the differential diagnosis of back pain has not been emphasized in the rheumatological literature.

With improvement and refinement of therapy, the majority of Hodgkin's disease patients are alive and free of disease at 5 years. As these patients continue to be observed, a variety of late complications have been reported. We describe herein three patients who developed retroperitoneal fibrosis following definitive therapy for Hodgkin's disease. The incidence approaches that seen with methysergide.

Eight patients had 13 ureters treated by peritoneal flap ureteropexy. There were no significant post-operative complications. Eleven ureters were functioning normally after an average follow-up of 28 months. The operation is suggested as a method of choice where omental wrapping is not possible.

The study was undertaken to focus attention on idiopathic retroperitoneal fibrosis, which may confront any surgeon operating in the abdomen or retroperitoneal area. Eleven patients, six men and five women varying in age from 35 to 76 years, were treated from 1969 to 1983. Two patients gave a history of methysergide and one gave a history of ergotamine ingestion. Three patients with biopsy-proven idiopathic retroperitoneal fibrosis were studied by computed tomography (CT) and nuclear magnetic resonance (NMR). The nuclear magnetic resonance images revealed the extent of the fibrous mass or plaque and showed that it involved the periaortic area in each case. The diagnostic information regarding the extent of the disease was similar to that derived from the x-ray CT. Idiopathic retroperitoneal fibrosis with duodenal obstruction is a rarely reported clinical entity. The presenting symptoms of nausea, vomiting, weight loss, and progressive fatigability can be easily overlooked if attention is focused on the more prominent symptoms of low back and flank pain. Duodenal obstruction can present several months after the initial diagnosis of retroperitoneal fibrosis. Nutritional support before surgical correction is imperative.

We report two cases of unusual ureteric obstruction in patients with an excessive consumption of analgesics. In a retrospective survey of seven cases of non-malignant retroperitoneal fibrosis seen in the last 15 years it was found that four had taken excessive amounts of analgesics. A careful drug history should be taken in all patients with restroperitoneal fibrosis and ureteric obstruction.
