Terrible Triad Elbow Fracture Dislocation with Ipsilateral Triceps Avulsion and Radial Shaft Fracture: A Case Report.

CASE: We report an unusual combination of injuries, terrible triad elbow fracture dislocation with ipsilateral avulsion of the triceps, and radial shaft fracture in a 23-year-old man. He was managed surgically and had good postoperative outcomes. CONCLUSION: Evaluation of the triceps tendon avulsion injury is difficult in this context, such as problems evaluating the rotator cuff injuries after a shoulder fracture dislocation. Because of the diagnostic difficulty, a high index of suspicion is necessary.

Rotator cuff tear degeneration and the role of fibro-adipogenic progenitors.

The high prevalence of rotator cuff tears poses challenges to individual patients and the healthcare system at large. This orthopedic injury is complicated further by high rates of retear after surgical repair. Outcomes following repair are highly dependent upon the quality of the injured rotator cuff muscles, and it is, therefore, crucial that the pathophysiology of rotator cuff degeneration continues to be explored. Fibro-adipogenic progenitors, a major population of resident muscle stem cells, have emerged as the main source of intramuscular fibrosis and fatty infiltration, both of which ar

Shoulder arthroscopy in the throwing athlete.

The throwing athlete is a unique patient. High demands on the shoulder, combined with poor tolerance for slight changes in performance, make this a particularly difficult group of patients to treat successfully. In addition, there is overlap between multiple pathologic conditions in the shoulder, including rotator cuff tendinopathy, shoulder instability, SLAP lesions, acromioclavicular arthrosis, and scapulothoracic problems. These disorders rarely occur in isolation in the throwing athlete. Arthroscopy of the shoulder may be invaluable as a diagnostic tool to confirm clinical and radiographic

Proximal humerus fractures: A review of current practice.

The management of proximal humeral fractures (PHF) remains controversial. Its incidence is increasing. Patients should be meticulously assessed clinically for co-morbidities and neuro-vascular injuries. Radiological investigation helps provide information on the fracture configuration and dislocations. Enhanced by 3-dimensional CT scanning, these further help in decision making and operative planning. PHF classifications have been demonstrated to have poor intra-observer and inter-observer reliability. Research has identified some radiographic predictive factors for humeral head ischaemia and

Sonography of the shoulder in patients with tears of the rotator cuff: accuracy and value for selecting surgical options.

OBJECTIVE: The management of patients with signs and symptoms referable to the rotator cuff depends on the presence of cuff injury and the size of the tear. Treatment options include conservative nonsurgical management for patients with an intact or partially torn cuff, arthroscopic decompression of the coracoacromial space for those not responding to nonsurgical management, and a range of surgical techniques to repair full-thickness tears. This study was designed to determine whether sonographic evaluation with classification of the extent of cuff injury is accurate for purposes of treatment

Subscapularis tendon tears hidden by the medial biceps sling can be missed on arthroscopic examination.

INTRODUCTION: To investigate (1) the prevalence of "hidden lesions" and "non-hidden lesions" of subscapularis tendon tears requiring repair during arthroscopic examination that would be missed by a 30° arthroscope, but could be identified by a 70° arthroscope, from the standard posterior portal and (2) the correlation of preoperative internal rotation weakness and findings of magnetic resonance imaging (MRI) indicating hidden lesions. MATERIALS AND METHODS: We retrospectively examined 430 patients who underwent arthroscopic subscapularis repair between was initially nonvisible with a 30° arth

How reliable are imaging protocols in the diagnosis of subscapularis tears?

The purpose of the study is to evaluate the accuracy of detecting subscapularis tendon tears on different imaging modalities in comparison with surgical findings. In addition, the accuracy of long head of biceps tendon pathology in assisting the diagnosis of a subscapularis tear was evaluated. Retrospectively, 336 patients who underwent surgery in the UZ Brussel for rotator cuff pathology and had pre-operative imaging at the hospital were included. Pathology of the subscapularis tendon and the long head of biceps tendon on imaging modalities was compared to arthroscopic and/or open surgery fin

Intraobserver and interobserver agreement of Goutallier classification applied to magnetic resonance images.

BACKGROUND: Fatty infiltration of the muscle bellies is an important prognostic factor in rotator cuff tears. It was described initially in computed tomography examinations, and there is an ongoing debate about whether magnetic resonance (MR) is a reliable method for staging fatty infiltration. This study sought to determine intraobserver and interobserver agreement for Goutallier's classification of fatty infiltration of the rotator cuff as evaluated through MR imaging. MATERIALS AND METHODS: Twenty MR examinations of the shoulder showing full-thickness tear of the supraspinatus tendon, with

Common Shoulder Injuries in American Football Athletes.

American football is a collision sport played by athletes at high speeds. Despite the padding and conditioning in these athletes, the shoulder is a vulnerable joint, and injuries to the shoulder girdle are common at all levels of competitive football. Some of the most common injuries in these athletes include anterior and posterior glenohumeral instability, acromioclavicular pathology (including separation, osteolysis, and osteoarthritis), rotator cuff pathology (including contusions, partial thickness, and full thickness tears), and pectoralis major and minor tears. In this article, we will r

Outcome of single-tendon rotator cuff repair in patients aged older than 65 years.

BACKGROUND: Rotator cuff repairs in patients aged older than 65 years are reported to have a high failure rate. Furthermore, asymptomatic cuff tearing is frequent in this age group, so the value of tendon repair has been questioned. Our aim was to review the results of cuff repair in these patients and to identify factors predicting outcome. METHODS: In this study, 58 patients aged older than 65 years with reparable supraspinatus tears underwent primary open (22 patients) or arthroscopic (36 patients) repair. The leading symptom was pain despite nonoperative treatment for at least 6 months. T

Ultrasound-guided percutaneous tenotomy of the long head of the biceps tendon: a non-reliable technique.

PURPOSE: To evaluate the feasibility of ultrasound-guided percutaneous tenotomy of the long head of the biceps tendon via a keyhole incision. METHODS: This was an anatomical study performed on twelve embalmed cadaveric shoulder joints. The rotator cuff and the position of the long head of the biceps tendon were explored by ultrasound prior to beginning the procedure. The biceps tenotomy was performed under ultrasound guidance by a highly experienced sonographer who was trained in shoulder tendon exploration. Arthroscopic exploration of the shoulder was performed immediately after the percutan

The role of repair tension on tendon to bone healing in an animal model of chronic rotator cuff tears.

Rotator cuff tendon tears are one of the most common shoulder injuries. Although surgical repair is typically beneficial, re-tearing of the tendons frequently occurs. It is generally accepted that healing is worse for chronic tears than acute tears, but the reasons for this are unknown. One potential cause may be the large tensions that are sometimes required to repair chronically torn tendons back to bone (i.e., repair tension). Therefore, the objective of this study was to utilize an animal model of chronic rotator cuff repairs to investigate the role of increased repair tension on tendon to

Tears of the subscapularis tendon in athletes--diagnosis and repair techniques.

Isolated injury to the subscapularis tendon is an uncommon - but potentially debilitating - injury among athletes. The most frequent injury pattern seen in younger athletes is an acute, traumatic tendon avulsion. In the older athletic population, chronic degenerative tearing of the tendon may precede sports-related rupture, particularly following shoulder dislocation. This article reviews how to diagnose this injury in athletes, and describes both conservative and operative treatment options and their outcomes.

Examining the association between musculoskeletal injuries and carpal tunnel syndrome in manual laborers.

INTRODUCTION: The association between musculoskeletal injuries and carpal tunnel syndrome (CTS) has not been investigated in a large, population-based study. METHODS: Latino manual laborers were recruited as part of a study of work-related health conditions. Each had a clinical examination, completed a hand diagram, and had nerve conduction studies. RESULTS: A total of 512 individuals completed all testing. An association was found between rotator cuff syndrome and CTS, with an adjusted odds ratio of 2.25 (P = 0.01) for the right arm, 2.08 (P = 0.03) for the left arm, and 1.84 (P = 0.03) for

Coracohumeral distance and coracoid overlap as predictors of subscapularis and long head of the biceps injuries.

BACKGROUND: Subscapularis (SS) lesions are often underdiagnosed because of an incomplete understanding of contributing factors but also because of a greater difficulty in SS tear diagnosis with magnetic resonance imaging or physical examination. In this setting, predicting factors can be useful tools in these injuries' management. The goal of this study was to determine the influence of the coracohumeral distance (CHD) and coracoid overlap (CO) in anterior rotator cuff lesions, as well as to determine the CHD and CO values that can accurately predict SS and long head of the biceps (LHB) injuri

Skiing-related injuries to the shoulder.

An analysis of 135 consecutive shoulder injuries seen in one clinic during a single ski season revealed anterior dislocation to be the most common ailment, constituting 52% of the total. Rotator cuff tears accounted for 20% of the total, followed by acromioclavicular (AC) separations (18%) and a miscellaneous group of contusions and isolated fractures comprising the other 10%. All of the shoulder dislocations were anterior, most were primary (81%) and men (83%) were involved more frequently. Twenty-four percent of the dislocations were thought to be equipment related. At follow-up examination

A Mechanistic Classification for Superior Labral Injuries Guides Operative Management.

The purpose of this article is to provide a clinically oriented classification system for superior labral injuries based on etiology, pathoanatomy, and associated biceps-labrum anchor complex injuries. The proposed classification system is based primarily on the mechanism of superior labral injuries as an ABCD classification (A, acute trauma; B, Bankart extension from instability; C, chronic repetitive overhead activity; and D, degenerative). The recognition of the cause is paramount to appropriately treating these patients, especially when considering operative treatment. Traumatic injuries i

Editorial Commentary: Stiff Patients After Rotator Cuff Repair: How Many Had Underrecognized Preoperative Adhesive Capsulitis?

Stiffness after rotator cuff repair is one of the most common sources of disability and one of the most common complications. Smoking, diabetes, Workers' compensation status, and traumatic tears are among the strongest risk factors. This is important information in setting appropriate expectations for both the patient and the surgeon preoperatively. Some of these factors are also associated with preoperative stiffness, so surgeons should maintain a high index of suspicion for concomitant adhesive capsulitis in patients presenting with limited motion, particularly in diabetic and non-English sp
