Wrist function recovery course in patients with scaphoid nonunion treated with combined volar bone grafting and a dorsal antegrade headless screw.

BACKGROUND: Surgical treatment is necessary for scaphoid nonunion. Open surgery with a combined volar and dorsal approach is thought to have poor functional outcomes and a prolonged recovery course. However, the detailed recovery course for this approach is rarely reported. The aim of this study was to investigate the recovery course and radiographic outcome for patients with scaphoid nonunion who underwent a combined volar bone grafting and dorsal antegrade headless screw approach. MATERIAL AND METHODS: Eighteen patients with scaphoid nonunion who underwent combined volar bone grafting and d

Using a vascularized bone graft from the distal metaphysis of radius in treating scaphoid nonunion: first report from a developing country.

Scaphoid fractures are common and prone to nonunion if left untreated. In this study, the outcome of using a vascularized bone graft was investigated in scaphoid nonunion. Twenty patients with scaphoid nonunion were recruited from Shohada Hospital. A reverse-flow vascularized bone graft was obtained from the distal metaphysis of radius and inserted into the gap of scaphoid fracture. Long and short thumb spica casts were used for 1.5 and 2 months thereafter, respectively. Union, motion, pain and the patients' satisfaction rate were documented. Twenty patients, 19 males and 1 female, with a mean

The impact of motion induced artifacts in the evaluation of HR-pQCT scans of the scaphoid bone: an assessment of inter- and intraobserver variability and quantitative parameters.

BACKGROUND: In-vivo high-resolution peripheral quantitative computed tomography (HR-pQCT) has high potential in scaphoid bone pathologies' scientific and clinical fields. The manufacturer's visual grading scale (VGS) classifies motion artifacts and divides scans into five quality grades ranging from grade 1 (good quality) to grade 5 (poor quality). This prospective study aimed to investigate the feasibility of the VGS and the influence of image quality on bone density and microarchitecture parameters for the scaphoid bone. METHODS: Within one year, twenty-two patients with scaphoid fractures …

[A new vascularized cartilague-bone-graft for scaphoid nonunion with avascular necrosis of the proximal pole. Description of a new type of surgical procedure].

The formation of a scaphoid pseudarthrosis with avascular necrosis in the area of the carpus is a The formation of a scaphoid pseudarthrosis with avascular necrosis in the area of the carpus is a dreaded complication after conservative or operative treatment of a scaphoid bone fracture, which previously often led to partial or total stiffening operations on the wrist. Vascularized bone grafts can be used to increase the bone fusion rates in the presence of scaphoid pseudarthrosis with avascular necrosis. On a note of caution, it must be mentioned, though, that such a procedure in the presence …

Treatment of delayed union and non-union of the carpal scaphoid with a compression staple and cancellous bone grafting--new method and preliminary results.

Scaphoid non-union (eight cases) and delayed union (three cases) were treated by compression staple and inlay of cancellous bone from iliac crest between the bony fragments. Union was achieved in ten of the cases (91 per cent). The consolidation time was 8-12 weeks in seven cases and 24 weeks or more in three cases. The compression stable fixation connected with bone grafting is a new and simple method to treat chronic fractures of the carpal scaphoid which is a difficult problem. The early results appear promising.

Measurement study of the "optimal" screw axis location and its angles with the first and second metacarpals based on X imaging of the scaphoid bone.

BACKGROUND: Accurate screw placement is pivotal to the success of internal fixation of scaphoid fractures. However, no anatomically standardized reference currently exists for guiding entry-point selection and screw-axis orientation during percutaneous Herbert-screw fixation through the volar approach. Conventional techniques rely heavily on surgeon experience or intraoperative fluoroscopy, thereby increasing radiation exposure and compromising accuracy. This study proposes a novel reference method based on the angular relationship between the scaphoid screw axis and the first and second metac

The Impact of Pronator Quadratus Origin Release on the Clinical Outcomes of Scaphoid Nonunion Patients Treated with Pronator Quadratus Pedicled Bone Grafts.

Background/Objectives: A pronator quadratus pedicled bone graft (PQPBG) is a distal radius volar vascularized bone graft used not only for avascular necrosis of the lunate but also for scaphoid nonunion. Despite its potency and its possession of a muscular shield, this vascularized graft has a disadvantage in that the potential shortness of the muscular leash may limit the distal transfer of the bone graft. Releasing of the pronator quadratus (PQ) ulnar origin was used to enhance the distal mobility of the graft. We aimed to investigate the effect of a PQ release on the surgical outcomes of sc

The Outcome of Using a Jamshidi Biopsy Trocar Needle in a Novel Technique for Bone Grafting in Percutaneous Internal Fixation of Scaphoid Non-Union.

INTRODUCTION: We report the outcome of using a novel technique of minimally invasive internal fixation and distal radius bone grafting using the Jamishidi Trephine needle and biopsy/graft capture device. METHODS: The technique utilises a 8 mm incision at the distal pole of the scaphoid. The non-union is excavated using the standard Acutrak drill. An 8 gauge Jamshidi trephine needle is used to harvest bone graft from the distal radius which is impacted into the scaphoid and fixed with an Acutrak screw. Fifteen patients were available for retrospective review, 14 male, age mean 29.5 (15-56). Av

Fracture Fixation in an Unstable Scaphoid Fracture Model: A Biomechanical Comparison of Double Screw Osteosynthesis With Two Common Bone Graft and Fixation Techniques.

PURPOSE: Rotational instability is an important contributor to nonunion in scaphoid waist fractures treated with a single compression screw. Double screw osteosynthesis demonstrates enhanced resistance to bending loads in unstable fracture models. This method may also provide better rotational strength, improving the construct's resistance to higher loads. This study aimed to evaluate and compare the torsional and cantilever bending strength of double screw fixation against single screw repairs combined with a strut graft and a wedge graft. METHODS: Volar open wedge osteotomies were performed

Mechanical strength and optimal site of placement of a threaded bone screw assessed on the basis of the screw breakage for non-union of the scaphoid: a biomechanical study.

We have performed the surgical procedure on the patients with the fracture of the scaphoid using mini-acutrak screws (ACUMED, Inc.). A 25-year-old man was injured in a fall. Surgical treatment for non-union of the scaphoid using a mini-acutrak screw was performed. However, this screw was broken, despite the absence of any obvious trauma mechanism. A structural examination was performed with a metal microscope and a hardness test was performed. A hardness evaluation was conducted on this broken screw and similar screws. Cannulated screws having diameters of 2.7 and 3.5 mm were also evaluated in
