Do anterior interbody cages have a potential value in comparison to autogenous rib graft in the surgical management of Scheuermann's kyphosis?

BACKGROUND CONTEXT: Previous studies have analyzed the outcome following posterior correction and combined anterior-posterior correction for Scheuermann's kyphosis. Traditionally interbody fusion has been obtained using morselized rib graft. Recently the use of titanium anterior cages has been suggested for interbody use. There are no long-term studies comparing these two techniques. PURPOSE: To investigate the potential value of titanium anterior interbody cages compared with morselized rib graft for anterior interbody fusion in combination with posterior instrumentation, correction, and fus

Kyphosis in a Turner syndrome population.

OBJECTIVE: The purpose of this study was to determine the prevalence of kyphosis in a Turner syndrome (TS) population. METHODS: Standing lateral thoracic spine and standing anterior-posterior (A-P) scoliosis radiographs were obtained on all girls with TS between the ages of 5 and 18 years seen in a TS clinic between July 2000 and March 2001. Medical histories were reviewed, and a pediatric orthopedic surgeon evaluated the radiographs of each patient (N = 25). Excessive kyphosis was defined as an A-P curvature >40 degrees, vertebral wedging as any A-P deformity >5 degrees at an individual vert

Pathogenesis of osteochondrosis juvenilis Scheuermann.

In osteochondrosis juvenilis Scheuermann, foci of various sizes in the cartilaginous end plates of the vertebral bodies display a loosening or complete interruption of the collagen fibers. These findings, together with an alteration and occasional absence of the growth zone, may result in the typical deformation of the vertebral bodies. Electron micrographs of the areas with optically absent collagen fibers reveal collagen fibrils. They are arranged in an irregular pattern. We conclude that a disturbance of collagen or ground substance biosynthesis is of importance in the pathogenesis of juven

Study of vertebral morphology in Scheuermann's kyphosis before and after treatment.

Variation of vertebral morphology in Scheuermann's Kyphosis before and after orthopedic treatment is usually measured by the entity of the curve, using Cobb's method, and by vertebral wedging. But the lack of correlation between these parameters and the clinical evolution of the deformity, lead to the possibility of other alterations that can explain part of the kyphosis deformities before and after the treatment. In this group of alterations the inclination of anterior and posterior walls, that express the trapezoid deformity of vertebras, seem to be more reliable indicators of curve response

Radiographic abnormalities in the thoracolumbar spine of young elite skiers.

An increased frequency of radiologic abnormalities in the thoracolumbar spine has been reported among young athletes in various sports, but there are no data concerning ski sports. To evaluate the incidence of these abnormalities in young elite skiers, we compared 120 skiers younger than 17 years old (alpine skiers, ski jumpers, and Nordic cross-country skiers) with a random sample of 39 control subjects of the same age who had no history of high-performance sports participation. Standardized anteroposterior and lateral radiographs of the entire lumbar spine, the lower thoracic spine, and the

Effect of Milwaukee brace on static and dynamic balance of female hyperkyphotic adolescents.

BACKGROUND: Biomechanical factors, such as spinal deformities can result in balance control disorders. OBJECTIVES: The purpose of this study was to examine the effect of bracing on static and dynamic balance control of hyperkyphotic female adolescents. STUDY DESIGN: Clinical trial. METHODS: A force platform was employed to record center of pressure (COP) parameters. Ten adolescents undergoing Milwaukee brace for hyperkyphosis and 14 normal subjects participated in the study. The COP data were collected with and without brace immediately on first day and after 120 days of continuous brace we

Septic shock after posterior spinal arthrodesis on a patient with Scheuermann kyphosis and multiple body piercings.

STUDY DESIGN: A case report. OBJECTIVE: We report septic shock as postoperative complication following an instrumented posterior spinal arthrodesis on a patient with multiple body piercings. The management of this potentially catastrophic complication and outcome of treatment is been discussed. SUMMARY OF BACKGROUND DATA: Body piercing has become increasingly more common because of change in culture or as a fashion statement. This has been associated with local or generalized ill effects including tissue injury, skin and systemic infections, and septic shock. There is no clear guideline path

[Combined thoracoscopic anterior spinal release and posterior correction for Scheuermann's kyphosis].

OBJECTIVE: To evaluate the effectiveness of combined thoracoscopic anterior spinal release and posterior correction for Scheuermann's kyphosis. METHODS: Sixteen patients with a diagnosis of Scheuermann's kyphosis were selected to undergo thoracoscopic anterior spinal release, disc excision, and fusion in conjunction with instrumented posterior correction and spine fusion. Cobb angle of the kyphosis were measured before the operation and during the follow-up period to evaluate the correction. Pre- and post-operative Oswestry disability Index (ODI) were collected to evaluate the pain relief. R

Diagnostic imaging of sports-related spinal disorders.

Diagnostic imaging is a dynamic field that has taken a quantum leap over the past several years with the advent of MRI. Accordingly, it is integral that the sports medicine clinician be well acquainted with the sundry imaging modalities at his or her disposal, and be able to choose the appropriate study or studies that will provide the most useful and accurate information. Conventional radiography should be the first study performed in every athlete with sports-related injury to the spine. If radiography reveals evidence of spondylolysis with or without spondylolisthesis, MRI would be an extre

Major complications in revision adult deformity surgery: risk factors and clinical outcomes with 2- to 7-year follow-up.

STUDY DESIGN: Retrospective cohort comparative study. OBJECTIVE: To determine the prevalence of major complications, identify risk factors, and assess long-term clinical benefit after revision adult spinal deformity surgery. SUMMARY OF BACKGROUND DATA: No study has analyzed risk factors for major complications in long revision fusion surgery and whether or not occurrence of a major complication affects ultimate clinical outcome. METHODS: Analysis of consecutive adult patients who underwent multilevel revision surgery for spinal deformity with a minimum 2-year follow-up was performed. All co

Multiple disc herniation in spondyloepiphyseal dysplasia tarda: A rare case report and review of the literature.

BACKGROUND: Spondyloepiphyseal dysplasia tarda (SEDT) is a rare, hereditary, X-linked skeletal disorder. To our knowledge, there are few reports about orthopedic surgery in these patients. This is the first report on patients with SEDT undergoing spinal and fracture reduction surgery. CASE PRESENTATION: A 31-year-old male patient who had been misdiagnosed with juvenile idiopathic arthritis (JIA) for 20 years and who had been treated with femoral shaft internal fixation for lower extremity fracture caused by minor trauma presented at hospital with stiffness and weakness in the lower extremitie
