Re-emerging scrub typhus is gaining recognition as an important cause of paediatric meningoencephalitis in tropics. We studied the clinical profile of scrub typhus meningoencephalitis (STME) in children <12 years. We developed a scoring system to differentiate severe fever with thrombocytopenia syndrome (SFTS) in 21 patients with SFTS and 91 with scrub typhus; it provided 100% sensitivity and 97% specificity (score > 1). We report a case of a patient with scrub typhus who presented with diabetic ketoacidosis (DKA). The patient initially manifested with DKA, and was diagnosed with acute scrub typhus after evaluation the precipitating factors of DKA. No other complications of scrub typhus were noted. This case report suggests that acute scrub typhus should be considered as a precipitating factor of DKA, especially during the endemic season.

Scrub typhus in Bhutan was first reported in 2009. We investigated an outbreak of scrub typhus in a remote primary school during August-October 2014. Delay in recognition and treatment resulted in 2 deaths from meningoencephalitis. Scrub typhus warrants urgent public health interventions in Bhutan.

Scrub typhus infection is an important cause of acute undifferentiated fever in South East Asia. The clinical picture is characterized by sudden onset fever with chills and non-specific symptoms that include headache, myalgia, sweating and vomiting. We report on a scrub typhus patient who contracted meningoencephalitis during doxycycline administration. This is the first case of scrub typhus in which doxycycline concentrations were measured in serum and cerebrospinal fluid. Clinicians should be alerted to the possibility that meningoencephalitis can occur because of inadequate maintenance of serum doxycycline level caused by antacids administered along with doxycycline.

Scrub typhus is a potentially fatal infectious disease. However, to this date, no epidemiologic study on mortality of complicated scrub typhus has been reported. We reviewed the clinical records of 302 patients with diagnosis of scrub typhus who were admitted to our institute between January 2000 and December 2006. In total, 297 patients with scrub typhus were analyzed and the mortality rate of this study group was 6.1%. We report three patients who presented with fever and late onset diarrhoea mimicking enteric fever. All three patients were diagnosed with an Orientia tsutsugamushi infection and responded dramatically to doxycycline treatment. Clinicians practicing in rickettsial disease endemic areas should be made aware of similar clinical presentations in order to prevent morbidity and mortality associated with rickettsioses.
