BACKGROUND: Patellofemoral pain (PFP) affects one-third of adolescents and can persist into adulthood, negatively impacting health and quality of life. Foot orthoses are a recommended treatment for adults with PFP, but have not been evaluated in adolescents. The primary objective was to determine the feasibility of conducting a full-scale randomised controlled trial (RCT) evaluating effects of contoured, prefabricated foot orthoses on knee pain severity and patient-perceived global change, compared to flat insoles. The secondary objective was to describe outcomes on a range of patient-reported

BACKGROUND: Falls in older people are common and can result in loss of confidence, fear of falling, restriction in activity and loss of independence. Causes of falls are multi-factorial. There is a paucity of research assessing the footwear characteristics among older people who are at high risk of falls, internationally and in the Irish setting. The aim of this study was to examine the proportion of older adults attending a geriatric day hospital in Ireland who were wearing incorrectly sized shoes.
METHODS: A consecutive sample of 111 older adults aged 60 years and over attending a geriatric…

Opportunistic onychomycosis is defined, when a non-dermatophyte mould is cultured from an abnormal nail unit in the absence of a dermatophyte. The presumption is that the mould has caused the abnormal clinical appearance of the nail unit, yet there are no data available to substantiate this claim. Reports have only identified the mould being recovered from the nail unit niche. A review of the published dermatologic literature describing toenail opportunistic onychomycosis by non-dermatophyte fungi has shown toenails with onycholysis, nail bed (NB) keratosis and nail plate surface abnormalities

OBJECTIVE: This study was conducted in order to assess the effect of wearing a lateral wedged insole with a subtalar strap for 2 years in patients with osteoarthritis varus deformity of the knee (knee OA).
DESIGN: The setting was an outpatient clinic. The efficacies of the strapped insole and a traditional shoe insert wedged insole (the inserted insole), as a positive control, were compared at the baseline and after 2 years of treatment. Randomization was performed according to birth date. The 61 female outpatients with knee OA who completed a prior 6-month study were asked to wear their resp

OBJECTIVE: To assess the effect of a lateral-wedge insole with elastic strapping of the subtalar joint on the femorotibial angle in patients with varus deformity of the knee.
METHODS: The efficacy of a wedged insole with subtalar straps and that of a traditional wedged insole shoe insert were compared. Sixty-six female outpatients with knee osteoarthritis (OA) were randomized (according to birth date) to be treated with either the strapped or the traditional inserted insole. Standing radiographs with unilateral insole use were used to analyze the femorotibial angles for each patient. In both

OBJECTIVE: A prior parameter-optimization study identified 9-15-mm support heights at 22%-26% infill as a candidate range for 3D-printed arch-support inserts, but whether responses within this range differ between runners with normal and high arches remains unclear. This arch-type-stratified secondary analysis compared shoe-ground interface loading under Single and 9-, 12-, and 15-mm support conditions at 24% infill and explored the temporal localization of condition-related differences during stance.
METHODS: The primary balanced analysis included thirty male marathon runners (15 normal arch

BACKGROUND: A distal radius fracture (DRF) is a common initial fragility fracture among women in their early postmenopausal period, which is associated with an increased risk of subsequent fractures. Gait assessments are valuable for evaluating fracture risk; inertial measurement units (IMUs) have been widely used to assess gait under free-living conditions. However, little is known about long-term changes in patients with DRF, especially concerning daily-life gait. We hypothesized that, in the long term, the daily-life gait parameters in patients with DRF could enable us to reveal future risk

BACKGROUND: Clinical gait analysis is widely used to aid the assessment and diagnosis of symptomatic pathologies. Foot function pressure systems such as F-scan and analysis of the spatial-temporal parameters of gait using GAITRite® can provide clinicians with a more comprehensive assessment. There are systems however, such as Strideway™ that can measure these parameters simultaneously but can be expensive. F-Scan in-shoe pressure data is normally collected whilst the person is walking on a hard floor surface. The effects of the softer Gaitrite® mat upon the F-Scan in-shoe sensor pressure data

INTRODUCTION: Studies have indicated that gait intervention programmes with minimalist shoes are effective for reducing pain, improving functionality and reducing knee joint overload in older women with knee osteoarthritis (OA). Other clinical trials with knee and foot muscle strength training and/or dynamic balance training have also shown clinical and functional effectiveness. Despite promising strategies, there is no evidence of the combination of shoes with gait intervention programmes. Thus, the objective of this randomised clinical trial is to investigate the effects of therapeutic progr…

The purpose of this study was to determine the effects of foot orthoses on the pattern of center of pressure in women with forefoot deformities during walking. Nine subjects with a forefoot varus and nine subjects with a forefoot valgus walked across a force platform three times for each of the following five treatment conditions: 1) barefoot, 2) shoes only, 3) rigid orthoses with shoes, 4) semi-rigid orthoses with shoes, and 5) soft orthoses with shoes. Force data were analyzed to determine the area under the center-of-pressure curve. A significant omnibus F ratio was obtained for treatments …

BACKGROUND: Lateral ankle sprains are common in indoor sports. High shoe-surface friction is considered a risk factor for non-contact lateral ankle sprains. Spraino is a novel low-friction patch that can be attached to the outside of sports shoes to minimise friction at the lateral edge, which could mitigate the risk of such injury. We aimed to determine preliminary effectiveness (incidence rate and severity) and safety (harms) of Spraino to prevent lateral ankle sprains among indoor sport athletes.
METHODS: In this exploratory, parallel-group, two-arm pilot randomised controlled trial, 510 s…

BACKGROUND: Although foot orthotics are widely prescribed for the treatment of flatfoot, the biomechanical effects of such devices are not yet fully clear. Accordingly, this study conducted an experimental investigation to evaluate the effects of orthoses on the gait patterns of patients with flatfoot during level walking.
METHODS: Eleven adults with flatfoot deformities were recruited. For each participant, kinematic and kinetic data were measured under three test conditions, i.e. walking barefoot, walking with shoes, and walking with shoes and insoles. During each test, the participants' ga…

BACKGROUND: Distance running has been revolutionised by the introduction of modern running shoes, named advanced footwear technology (AFT). AFT shoes commonly implement carbon fibre plates to increase longitudinal bending stiffness as well as highly responsive midsole foams, which increase energy return. In 2022, World Athletics stipulated that midsoles in competition road running shoes cannot exceed 40 mm in thickness, but this limit seems arbitrary. Therefore, this study aimed to understand the effect of midsole thicknesses in AFT shoes at (40 mm) and above (50 mm) the current regulation on …

This study explores the relationship between dance shoe type and foot pressure characteristics. During adolescence, while the foot is still developing, limiting focal pressure on the feet may help reduce the risk of injury. In order to "condition" the feet for advanced dance, where pointe shoes are worn, it may be advisable to first utilize demi-pointe shoes. Eight female dancers were each tested in four footwear conditions (barefoot, soft, demi-pointe, and pointe shoes), and patterns of foot pressure were compared. A questionnaire was also distributed among sixty-five adolescent females curre…

Evaluation of potential fatigue for the elderly could minimize their risk of injury and thus encourage them to do more physical exercises. Fatigue-related gait instability was often assessed by the changes of joint kinematics, whilst planar pressure variability and asymmetry parameters may complement and provide better estimation. We hypothesized that fatigue condition (induced by the treadmill brisk-walking task) would lead to instability and could be reflected by the variability and asymmetry of plantar pressure. Fifteen elderly adults participated in the 60-min brisk walking trial on a trea

This study examined the effects of shoes' segmented forefoot stiffness on athletic performance and ankle and metatarsophalangeal joint kinematics and kinetics in basketball movements. Seventeen university basketball players performed running vertical jumps and 5-m sprints at maximum effort with 3 basketball shoes of various forefoot plate conditions (medial plate, medial + lateral plates, and no-plate control). One-way repeated measures ANOVAs were used to examine the differences in athletic performance, joint kinematics, and joint kinetics among the 3 footwear conditions (α = .05). Results in…

This study evaluated the effects of a top-tier carbon plated shoe with highly responsive foam (CP-shoe) on changes in running economy (RE) a … RE and lower HR. However, the time course of changes during the 80-min runs did not differ between shoes (i.e., no shoe × time interactions). Across all 80-min runs, LT-speed and RE improved postexercise, with the CP-shoe yielding higher LT speeds than the NCP-shoe (+0.5-0.6 km·h-1). In conclusion, the CP-shoe enhanced RE and reduced perceived exertion …
