This case report details the management of a 60-year-old male who sustained a complex proximal humerus fracture with an axillary artery injury following a fall. Initial assessment revealed a fractured humeral head and complete occlusion of the axillary artery, which was repaired using a saphenous vein graft. Subsequently, the patient underwent reverse shoulder arthroplasty (RTSA) after the limb stabilized. The staged approach, prioritizing urgent vascular repair followed by delayed orthopedic intervention, proved effective. The patient achieved satisfactory functional recovery with an improved

Proximal humerus fractures (PHFs) are a common type of fracture in adults. Although PHFs are common, bilateral presentation is extremely rare. Most PHFs are treated conservatively. In this report, we describe a 69-year-old right-hand-dominant male patient who was involved in a high-impact motor vehicle accident (MVA). The patient's upper limbs were in a fully extended position while he was holding the driving wheel, where he sustained a side impaction to the car by a hard object that caused bilateral four-part PHF with dislocation, which was confirmed on radiological investigations. The orthop

INTRODUCTION: It is rare to see chronic bilateral anterior fracture-dislocations as a result of seizure, and we present a case of this type and review of the literature. Despite the signs and symptoms of shoulder dislocation being well documented, and X-ray imaging being good at identifying such pathology, there are a few cases in the literature of missed or chronic shoulder dislocation (a shoulder that has been dislocated for more than 3 weeks) but these are extremely rare. Our case represents the first example of chronic bilateral locked anterior fracture-dislocations requiring open reductio

In this paper, our main objective was to emphasize the competency of extended deltopectoral exposure, enforced with the supraspinatus and subscapularis detachment, to gain access to the entire head. The second important point in this paper was to underline the importance of the knowledge that is necessary for interpreting classic radiologic signs of posterior fracture-dislocation of the shoulder. A 47-year-old woman fell down directly onto her shoulder while she was skiing. She was diagnosed with posterior shoulder dislocation, associated with fracture of the head (head splitting) and humeral

INTRODUCTION: Most published floating clavicle report a dislocation or fracture of one or both ends of the clavicle.
CASE PRESENTATION: We reported a new framework of this injury in a young triathlon athlete; medial-end displaced fracture with co-existent double disruption of the superior shoulder suspensory complex (SSSC) with the anterior shoulder arch wholly disconnected from the nearby structure.
CONCLUSION: The management of these complex fractures remains an open debate. The infrequent publications and the rarity of this type of injuries don't support the surgeon about the choice of th

BACKGROUND: The Disabilities of the Arm, Shoulder and Hand (DASH) and Constant-Murley scores are commonly used instruments. The DASH is patient-reported, and the Constant-Murley combines a clinician-reported and a patient-reported part. For patients with a humeral shaft fracture, their validity, reliability, responsiveness, and minimal important change (MIC) have not been published. This study evaluated the measurement properties of these instruments in patients who sustained a humeral shaft fracture.
METHODS: The DASH and Constant-Murley instruments were completed 5 times until 1 year after

Many tibial and radial-ulnar fractures in cattle and small ruminants may be treated successfully with Thomas splint and cast combinations (TSCC). The Walker splint has been specifically designed for treating tibial fractures and stifle injuries in cattle. It usually fits better in rear limbs of cattle than the TSCC and thus can provide more stability. Spica bandages combined with lateral splints placed over the dorsum of the affected limb and down the contralateral shoulder may be used for partial immobilization of humeral fractures in calves and small ruminants when surgical repair is not an o

INTRODUCTION: Traumatic anterior dislocation of the shoulder is an emergency and warrants urgent attention. However, it becomes difficult to manage in cases of associated fractures of humerus and other bones surrounding the shoulder joint. There have been reports of traumatic anterior dislocation of the shoulder associated with humeral fractures in the literature but the trilogy of anterior dislocation of the shoulder; humeral shaft fracture; and scapular fracture have never been described.
CASE PRESENTATION: We present the case of a 27-year-old south Asian man presenting with the above-menti

BACKGROUND: The aim of this study was to evaluate the prognostic factors and limitations of anatomic unconstrained shoulder arthroplasty, performed without tuberosity osteotomy, for the treatment of secondary glenohumeral arthritis following posttraumatic cephalic collapse or necrosis of the humeral head, defined as type-1 fracture sequelae.
METHODS: Fifty-five patients with type-1 fracture sequelae treated with anatomic shoulder arthroplasty were included in this retrospective single-center cohort study. All anatomic humeral prostheses were implanted without performing a greater tuberosity o

Proximal humerus fractures are common in the older population. These fractures can be associated with dislocation of the shoulder. Four-part fracture-dislocations and head-splitting 3-part fractures are generally indicated for shoulder hemiarthroplasty in the older population. When such surgery is warranted, inspection of the glenoid during the procedure is indicated, as loss of evaluation may lead to postoperative shoulder instability. There is value in alerting orthopedic surgeons to the possibility of anterior glenoid rim fracture in association with proximal shoulder fracture-dislocation.

Periprosthetic fractures are a serious complication of joint replacement surgery. With the growing prevalence of reverse total shoulder arthroplasty (RTSA), the incidence of relatively uncommon periprosthetic humeral fractures has increased. Here, we present the unique case of a 74-year-old woman who developed atrophic non-union after plate osteosynthesis for a periprosthetic fracture associated with RTSA. Fixation failure was evident 3 months after the surgical intervention; the patient underwent a 3-month course of arm sling immobilization. However, bone resorption continued, and varus angul

INTRODUCTION: High-energy trauma can result in complex orthopedic injury patterns. The "floating shoulder" and "floating elbow" are distinct, severe injuries involving serial fractures of the upper extremity, disrupting adjacent joint function. The simultaneous ipsilateral occurrence of both, further complicated by a pelvic fracture, represents an exceptionally rare and challenging clinical scenario.
CASE PRESENTATION: We present the case of a 45-year-old male with a history of schizophrenia who sustained multiple fractures after a fall from a height during a psychotic episode. He was diagnos

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) for proximal humerus fractures is becoming more common. The aim of this study was to investigate the radiographic outcomes of uncemented rTSA for proximal humerus fractures and compare these outcomes based on 3 different implants.
METHODS: This was a retrospective review of patients treated with uncemented primary rTSA for acute proximal humerus fracture at 2 large healthcare institutions using one of 3 different humeral stem designs. Group A (n = 46) received a Zimmer TM stem, group B (n = 27) received a DJO Altivate stem, and group C (n

BackgroundProximal humerus fractures (PHF) are the third most common osteoporotic fracture. Non-operative management and reverse total shoulder arthroplasty (rTSA) have become the mainstay of management for elderly patients, however there is yet to be a consensus on the best treatment modality. This systematic review aims to compare the clinical outcomes of non-operative management and rTSA in the management of PHF.MethodsA systematic review was performed in accordance with PRISMA guidelines. A search was conducted using MEDLINE, CINAHL, EMBASE and CENTRAL databases.ResultsThis study analysed

INTRODUCTION: Osteoporosis is prevalent in patients undergoing total shoulder arthroplasty (TSA) and affects perioperative management and outcomes, including fragility fractures. Despite guidelines recommending dual-energy radiograph absorptiometry (DXA) screening in women at risk for osteoporosis, screening before TSA remains low. With TSA procedures rising, it is important to identify factors that may contribute to low DXA utilization rates. This study aims to (1) evaluate the influence of demographic and social determinants of health on preoperative DXA screening and (2) identify predictors

PURPOSE: Distal radius fracture (DRF), sarcopenia, and malnutrition have been reported to be interrelated. However, there are few reports on the effects of sarcopenia and malnutrition on DRF patients' postoperative outcomes. This study examined the healthy-side grip strength and preoperative blood tests to determine the presence of possible sarcopenia (PS) and malnutrition in geriatric women with DRF and their impact on postoperative functional outcomes.
METHODS: Fifty-five woman older than 60 years treated with volar-locking plate fixation for low-energy DRF from standing-level falls were re

A combination of coracoid fracture and acromioclavicular dislocation is rare. Detecting further pathologies that could cause multiple superior shoulder suspensory complex disruptions is mandatory. Literature regarding these injuries and their management is lacking. We report our diagnostic and surgical strategies, and the postoperative outcomes of superior shoulder suspensory complex disruptions. We present five cases, treated from 2011 to 2016, who had >2 disruptions of the superior shoulder suspensory complex, involving at least a coracoid fracture and an acromioclavicular joint dislocation.
