Long-term clinical and radiographic outcomes of anatomic total shoulder arthroplasty with a short-stem humeral component: a ten-year follow-up study.

PURPOSE: This study aimed to provide long-term clinical and radiographic follow-up data for anatomic total shoulder arthroplasty using a short-stem humeral component with a cemented keeled glenoid in primary glenohumeral osteoarthritis. METHODS: Between 2010 and 2013, 35 consecutive anatomic total shoulder arthroplasties were performed at a single institution by one surgeon. At a mean follow-up of 10.3 ± 1.1 years, seven patients had died and five were lost, leaving 23 shoulders (82% of surviving cases) available for independent clinical and radiographic analysis. Clinical evaluation included

Three-dimensional characterization of glenoid defects in failed shoulder arthroplasties.

BACKGROUND: Glenoid vault defects are often observed in revision shoulder arthroplasty, and improper management of glenoid bone loss during revision to reverse total shoulder arthroplasty (rTSA) increases the risk of complications. However, there is limited understanding of the types of glenoid vault defects that occur in patients. Therefore, the purpose of this study was to three-dimensionally characterize the underlying glenoid morphology of patients with failed shoulder arthroplasties subsequently undergoing revision arthroplasty to rTSA. METHODS: Computed tomography scans of 100 patients,

Stress shielding of the Univers Revers system in reverse shoulder arthroplasty is minimal and does not progress substantially from short- to midterm follow-up.

BACKGROUND: The purpose of this study was to compare short- to midterm signs of stress shielding with an inlay press-fit reverse shoulder arthroplasty (rTSA) humeral component. METHODS: A retrospective review was conducted on primary rTSAs with a minimum 5-year follow-up that were implanted with a press-fit 135° inlay humeral component (Univers Revers and Revers Apex; Arthrex, Inc.) designed to achieve metaphyseal fixation. Patient-reported outcomes (PROs) and range of motion (ROM) values were collected preoperatively and compared with 2 and 5 years postoperative values. Radiographic signs of

Diagnostic Arthroscopy for Detection of Periprosthetic Infection in Painful Shoulder Arthroplasty.

PURPOSE: To analyze the utility of arthroscopic biopsies for detection of periprosthetic infection in painful shoulder arthroplasty without objective signs of infection. METHODS: A retrospective analysis of all patients who underwent a diagnostic arthroscopy for painful shoulder arthroplasty from June 2012 through July 2018 was performed. Patients with a subsequent revision shoulder arthroplasty after diagnostic arthroscopy were included. Arthroscopic tissue culture results were compared with the culture results of intraoperative tissue samples obtained at the time of open revision surgery. A

Radiographic evaluation of short-stem press-fit total shoulder arthroplasty: short-term follow-up.

BACKGROUND: Humeral loosening is an uncommon etiology for revision shoulder arthroplasty. We aimed to evaluate the radiographic and clinical outcomes of a short-stem press-fit humeral component after primary total shoulder arthroplasty. METHODS: We reviewed our patient database, from January 2008 to December 2011, for primary total shoulder arthroplasties performed with a short-stem press-fit humeral component. Radiographs and clinical outcomes were evaluated in the immediate postoperative period and at the most recent follow-up, with at least 24 months of data for all patients. RESULTS: The

Implicit development of gaze strategies support motor improvements during action encoding training of prosthesis use.

BACKGROUND: Action observation training has been suggested to facilitate motor improvements in the lives of persons with neural injury. Previous studies have shown that for persons with upper limb amputation, matched limb training, where prosthesis users emulate each other, has shown promise above mismatched training where a prosthesis user emulates actions of a person with sound limbs (most commonly that of a therapist). OBJECTIVE: The mechanism underlying the matched limb training benefit is unclear. Gaze strategies may reveal unique patterns between matched and mismatched training which co

Outcomes Of The Combined Anteroposterior Approach For Forequarter Amputation In Shoulder Girdle Tumours.

BACKGROUND: Forequarter amputation (FQA) is the surgical treatment of choice for tumours in the upper extremity and shoulder girdle that infiltrate the neurovascular bundle, shoulder joint and muscles of the shoulder girdle in non-salvageable cases. In both curative and palliative settings, FQA can serve as an effective oncological treatment for local control of tumour. METHODS: All patients who underwent FQA in our unit from January 2016 till August 2019 for oncological indications were included in our study and their clinical outcomes were calculated. RESULTS: Thirteen patients were includ

Does the etiology of a failed hemiarthroplasty affect outcomes when revised to a reverse shoulder arthroplasty?

BACKGROUND: The purposes of this study were to evaluate patient outcomes after revision of hemiarthroplasty to reverse shoulder arthroplasty (RSA) based on initial pathology, to determine the re-revision rate, and to identify characteristics that may predict subsequent re-revision. METHODS: A total of 207 shoulder hemiarthroplasty, bipolar prosthesis, and humeral resurfacing cases revised to RSA between January 2004 and January 2017 were reviewed. Outcome measures included shoulder motion and American Shoulder and Elbow Surgeons and Simple Shoulder Test (SST) scores. Sixteen RSAs underwent re

Beyond Implant Design: Surgical Principles and Early Outcomes of Patient-Specific Scapular Reconstruction After Total Scapulectomy.

Background/Objectives: Total scapulectomy for malignant scapular tumors remains a major reconstructive challenge because resection removes the osseous framework, muscular insertions, and biomechanical support required for shoulder function. Patient-specific three-dimensional (3D)-printed implants may improve anatomical reconstruction, but the indications and surgical principles underlying successful total scapular replacement remain poorly defined. Methods: This retrospective single-center case series included the first three consecutive patients who underwent total scapulectomy followed by im

Physiological Loading of the Coonrad/Morrey, Nexel, and Discovery Elbow Systems: Evaluation by Finite Element Analysis.

PURPOSE: Wear of polyethylene bearings represents a limiting factor in the long-term success of total elbow prostheses. Bearing stress is 1 factor contributing to accelerated wear. Physiological loading of total elbow prostheses and implant design influence upon bearing stresses have not been well described. This study evaluates bearing stresses in 3 commercially available implant designs under loads associated with daily living. METHODS: Motion tracking from a healthy volunteer helped establish a musculoskeletal model to simulate flexor and extensor muscle activation at 0°, 45°, and 90° of s

Predictive Modeling of Clinically Apparent Shoulder Periprosthetic Joint Infection (PJI).

INTRODUCTION: The first International Consensus Meeting (ICM) definition of shoulder periprosthetic joint infection (PJI) introduced a tiered framework of PJI categories based on weighting of minor criteria for those cases not having major criteria of Definite infection. The selection of minor criteria and weightings was based on expert opinion and limited available evidence. The purpose of this study was to use prospective, multicenter data to develop a predictive model for clinically apparent shoulder PJI to inform a more evidence-based understanding of minor criteria and PJI categories. Spatial mapping of humeral head bone density.

BACKGROUND: Short-stem humeral replacements achieve fixation by anchoring to the metaphyseal trabecular bone. Fixing the implant in high-density bone can provide strong fixation and reduce the risk of loosening. However, there is a lack of data mapping the bone density distribution in the proximal humerus. The aim of the study was to investigate the bone density in proximal humerus. METHODS: Eight computed tomography scans of healthy cadaveric humeri were used to map bone density distribution in the humeral head. The proximal humeral head was divided into 12 slices parallel to the humeral ana

Association of lateral humeral offset with functional outcome and geometric restoration in stemless total shoulder arthroplasty.

BACKGROUND: Restoration of shoulder geometry is desirable in total shoulder arthroplasty (TSA) and thought to influence the postoperative clinical outcome. We aimed to study the association of postoperative lateral humeral offset (LHO) changes and clinical outcome, as well as to investigate the ability of stemless anatomic TSA to restore shoulder geometry. METHODS: In patients with primary shoulder osteoarthritis who underwent stemless anatomic TSA, the preoperative and postoperative clinical outcome was measured. Shoulder geometry was measured on preoperative computed tomography for the oste

An Evolution of Shoulder Periprosthetic Infections Management: MicroDTTect, Bioactive Glass and Tantalum Cones Employment.

Periprosthetic joint infections of the shoulder (PJIS) are the major cause for revision within the first two post-operative years, and are challenging both to diagnose and treat. Success depends on early identification of microorganisms, appropriate surgical procedures and efficient antibiotic administration. The peculiar microbiology of the shoulder may render the criteria for hip/knee PJI management inappropriate. In addition, later cases with clinically subtle signs often present diagnostic challenges. In recent years, specific issues of PJIS have been managed through the use of new instrum

Proximal humeral fractures treated with hemiarthroplasty: does tenodesis of the long head of the biceps improve results?

INTRODUCTION: Pathology of the long head of the biceps (LHB) may be the cause of anterior shoulder pain after hemiarthroplasty for treatment of fractures of the proximal humerus. The currently available literature lacks adequate randomised trials examining whether tenodesis of the LHB improves results. The purpose of this study was to evaluate the effects of tenodesis of the LHB on the clinical outcome following hemiarthroplasty for fractures of the proximal humerus. METHODS: This prospective randomised study included 37 patients treated with hemiarthroplasty for four-part fractures, fracture

B2 and B3 glenoid osteoarthirtis: outcomes of corrective and concentric (C2) reaming of the glenoid combined with pyrocarbon hemiarthroplasty.

BACKGROUND: Posterior humeral subluxation (PHS) in B2 and B3 glenoid is a cause of asymmetric long-term stress on the glenoid and the potential reason for glenoid loosening in anatomic total shoulder arthroplasty and painful glenoid erosion in hemiarthroplasty with metallic heads. We hypothesized that corrective and concentric (C2) reaming of the glenoid associated with pyrocarbon hemiarthroplasty (HA-PYC) could improve the centering of the humeral head and decrease the risk of persistent painful glenoid erosion in young and active patients with B2 and B3 glenoid. METHODS: Between 2014 and 20

Radiographic and clinical comparison of pegged and keeled glenoid components using modern cementing techniques: midterm results of a prospective randomized study.

BACKGROUND: Glenoid component loosening remains a significant issue after anatomic shoulder arthroplasty. Pegged glenoid components have shown better lucency rates than keeled components in the short term; however, midterm to long-term results have not fully been determined. We previously reported early outcomes of the current randomized controlled group of patients, with higher glenoid lucency rates in those with a keeled glenoid. The purpose of this study was to evaluate the radiographic and clinical outcomes of these components at minimum 5-year follow-up. METHODS: Fifty-nine total shoulde

Reverse total shoulder versus angular stable plate treatment for proximal humeral fractures in over 65 years old patients.

BACKGROUND: Treatment for displaced proximal humeral fractures is still under debate. Poor rotator cuff status and non-union of the tubercles in elderly patients has caused reversed total shoulder prosthesis growing popularity and showed promising results, even in comparison to angular stable plates fixation. The purpose of this study is to report clinical and radiological results of proximal humerus fractures treated with rTSA or ORIF in elderly. MATERIALS AND METHODS: The study has investigated retrospectively a consecutive series of 73 patients over 65 years old (range 65-91) with proximal …

Reverse Delta-III total shoulder replacement combined with latissimus dorsi transfer. A preliminary report.

BACKGROUND: Reverse total shoulder arthroplasty allows the restoration of active overhead elevation in patients with a massive rotator cuff tear and pseudoparesis of elevation. However, it does not restore active external rotation, the lack of which can also constitute a substantial functional handicap and compromise the outcome of this arthroplasty. Latissimus dorsi tendon transfer reliably restores control of active external rotation in rotator-cuff-deficient shoulders. In this preliminary study, we assessed the results of the combination of a latissimus dorsi transfer to the greater tuberos
