We describe a full term male infant born by cesarean section, who presented a deep round depression of the left parietal bone at birth. MRI scan showed no signal alteration on cortical and sub-cortical level in correspondence of the depressed skull fracture. At four months of age, a skull X-ray was normal. At 18 months of age growth and neurologic follow-up is normal.

Newly discovered Homo remains, stone artifacts, and animal fossils from Dmanisi, Republic of Georgia, provide a basis for better understanding patterns of hominin evolution and behavior in Eurasia ca. 1.77 million years ago. Here we describe a fourth skull that is nearly complete, lacking all but one of its teeth at the time of death. Both the maxillae and the mandible exhibit extensive bone loss due to resorption. This individual is similar to others from the site but supplies information about variation in brain size and craniofacial anatomy within the Dmanisi paleodeme. Although this assemb…

OBJECTIVE: Innovations in diagnosis, surgical techniques, and perioperative care have dramatically improved outcomes in lateral skull base procedures in recent years. There is a belief, however, that children with skull base tumors have yet to benefit from these technological and procedural advances. The purpose of this study is to provide a clinical review of neurotologic skull base surgery in the pediatric population.
STUDY DESIGN: Retrospective case review.
SETTING: Private practice tertiary referral center.
PATIENTS: Eighty-nine pediatric patients undergoing 115 neurotologic procedures …

In our case, MR was performed first and showed all the classical signs of skull base chordoma. CT provided complementary information about bone destruction which are also usual in this type of lesion. Major differential diagnoses of the chordoma in the clivus are the other central skull base masses. Biopsy and histology make th …

In spite of the fact that there are many articles considering that intrauterine depressed skull fractures are caused essentially by instrumental extraction, literature is scarce about spontaneous foetal head injuries. Here, we report the case of two depressed skull fractures and one of linear fracture not associated with any known trauma during the pregnancy or delivery. The etiological process leading to the idiopathic character of such lesions, the aetiology, the treatment and the prognostic will be discussed. The forensic problem raised by such cases is very important.

Meningiomas are the most frequent benign tumors treated by gamma knife radiosurgery and the majority of them are located on the skull base. Between 1992 and 1999, 197 skull base-located meningiomas in 192 patients were treated by gamma knife in Prague. Contact with the chiasma or optic tract was not regarded as a contraindication for gamma knife radiosurgery and such contact was observed in 32% of the skull base meningiomas treated. 176 patients were monitored during a median of 36 months, of whom 73% showed a decrease in tumor volume; no change was observed in 25% and continued growth was obs…

Encephaloceles are relatively rare phenomena produced by the protrusion of brain and dura through an anterior skull base defect. Although they can occur as congenital defects, encephaloceles can also present after trauma. The diagnosis is usually made with nasal endoscopy and imaging studies. This report reviews our recent experience repairing 5 encephaloceles in 4 patients. The diagnostic approach and the technical aspects of surgical management are discussed. Although encephaloceles are a rarity, this diagnosis should be considered as part of the differential diagnosis in evaluating a patien…

BACKGROUND: Prolactinomas invading the skull base are rare, and could easily be confused with skull base tumors of nonpituitary origin.
CASE DESCRIPTION: We report a series of 4 cases of giant prolactinomas invading the skull base and presenting with atypical symptoms. Case 1 presented with a short history of headache and nasal obstruction. Case 2 presented with progressive hypoacusia, dizziness, and ophthalmoplegia. In Case 3, the patient developed rapid progressive visual failure and psychiatric symptoms. Case 4 presented with a 1-year history of headache and retrorbital pain. The diagnosis…

Radiosurgery has been proven to be a safe and effective management strategy for skull base meningiomas either primarily or for tumor recurrence or progression after prior microsurgical resection. With its steep radiation falloff, radiosurgery provides long-term tumor growth control without the complications associated with conventional fractionated radiation therapy. Stereotactic MR imaging has allowed better definition of the tumor margin for precise multiisocenter conformal dose planning, and our current radiation dose prescription has decreased the incidence of new cranial nerve deficits af…

INTRODUCTION: Elevation and repair of an open depressed skull fracture is often thought of as an emergency procedure. Common indications for emergent elevation of a depressed skull fracture have been dural tear, seizure, gross contamination or mass effect from bone or a sizable underlying intracerebral hematoma. As treatment of head injury moves towards management of cerebral perfusion pressure (CPP) rather than intracranial pressure (ICP), we sought a way to maximize CPP in the initial treatment of head-injured patients with depressed skull fractures that would eventually require surgery by d…

Stereotactic radiosurgery is an increasingly safe and usually effective method of preventing growth of small to moderate-sized primary tumors of the anterior skull base. Tumor growth control is obtained in more than 90% of patients with skull base tumors having benign histology. Neurologic function is maintained in most patients. The risk of temporary or permanent injury to critical neural and vascular structures is significantly lower than the risk associated with microsurgery. The optic nerves, chiasm, and tracts are structures that appear most sensitive to the radiation doses used during ra…

The field of Skull base Surgery evolved rapidly over the past several years. The techniques that were developed now make it possible to treat lesions that were previously deemed inaccessible. This article is a review of some of the aspects of this evolving specialty. Three representative cases will be presented and discussed with special emphasis on patient selection, surgical complications and postoperative rehabilitation.

The sonographic finding of a strawberry-shaped skull is a marker of serious fetal malformation, most commonly associated with trisomy 18. We report a novel association of this sign with thanatophoric dysplasia.

In a series of 527 cerebellopontine angle tumors, there were 416 cases of acoustic nerve tumors and 14 cases of primary tumor of the facial nerve in the petrous bone or intracranial cavity. Six additional patients were presumed to have facial tumors, although they were not operated on. Of the 14 verified facial nerve tumors, all but two were neurinomas and 11 had important intracranial extensions into the middle and/or the posterior fossa. In most of these 14 cases, surgical removal was performed via the translabyrinthine route, which is advantageous in that it displays the characteristic rela

A case of squamous cell carcinoma of the fronto-ethmoid complex in an 18-year-old man is presented. At the time of resection both the anterior and posterior tables of the frontal sinus had to be removed. The resulting surgical defect left a marked cosmetic deformity. CT scan five months after the resection suggested calcification with possible ossification of the dura. Exploration of the area confirmed these findings. Because of the firmness of the dura it was possible to contour a methyl methacrylate template to fill the defect. This was wired to the surrounding bone and provided an excellent

Neonatal skull depressions may occur from traumatic delivery or abnormal intrauterine position. Surgery has been recommended when the condition is thought to threaten neurologic development; however, normal neurologic outcomes have been observed in conservatively managed infants. A review of four patients with neonatal skull depression is presented together with the obstetric history and findings from nursery examinations, skull roentgenograms, electroencephalograms, and follow-up pediatric visits. Two patients underwent surgical elevation of the depression, while two children with depressions…

A male infant with cloverleaf skull and multiple other birth defects born to unrelated, healthy, young parents is presented. Radiologic findings in addition to the cloverleaf skull configuration included short, wide clavicles, winged scapulae, unusual shapes of ribs with abnormal spacing between them and with prominent costovertebral junctions, and widely separated ischia. Ulnae appeared angular with probable fusion to the midportion of the radial bones bilaterally. There was polydactyly of the hands and feet with grossly abnormal metacarpal and metatarsal bones. Skeletal maturation was normal…

The nose has been reconstructed with bone grafts, cartilage, and silicone. Each of these methods has disadvantages, but the bone graft is most trouble free. It is possible to take a split skull graft with a high speed drill and an osteotome. The position of the graft site on the skull determines its shape. This can be tailored to the desired nasal contour. Several grafts may be used to build up the nasal dorsum further. This method has been used in patients suffering from acute trauma and deformities resulting from previous trauma, rhinoplasty, congenital anomalies, and after cancer resection.…
