Vascularised local and free flaps in anterior skull base reconstruction.

Lesions of the anterior skull base often require sufficient closure in order to prevent cerebrospinal fluid (CSF) leak, ascending infection and/or brain tissue prolapse. The transfer of devitalized autologous, allogenic or xenogeneic material is not always sufficient particularly not in larger defects or in the recurrent situation. Here the transfer of vascularised tissue seems to be more appropriate. The anterior skull base with various complex defects of 41 patients was reconstructed in an interdisciplinary setting by vascularised, autologous tissue transfer. Minor defects (<2.5 cm in max. d…

Ethmoid skull-base height: a clinically relevant method of evaluation.

BACKGROUND: The Keros classification scheme has been used to describe the ethmoid skull-base configuration; however, this scheme only partially addresses key anatomic issues in ethmoid skull-base injury avoidance during surgery. METHODS: Coronal computed tomography (CT) scans were reviewed for 200 patients undergoing sinus evaluation at a tertiary care institution. Keros classification, maxillary sinus:ethmoid height ratio, and the skull-base height:orbital height ratio were recorded for each patient. The Pearson correlation coefficient was used to determine if a relationship existed between

Posterolateral skull base reconstruction using the supraclavicular artery island flap.

The supraclavicular artery island (SAI) flap is a viable fasciocutaneous option for the reconstruction of head and neck defects. Although authors have reported success using SAI flaps for various reconstructive indications, concerns of a tenuous blood supply and distal ischemia have previously limited its use in the posterolateral skull base. This case series reports the outcomes of 5 consecutive patients receiving SAI flaps for posterolateral skull base reconstruction. All flaps were harvested in less than 1 hour with primary closure of all donor sites. A single patient developed superficial …

Fractionated stereotactic conformal radiotherapy for large benign skull base meningiomas.

PURPOSE: To assess the safety and efficacy of fractionated stereotactic radiotherapy (FSRT) for large skull base meningiomas. METHODS AND MATERIALS: Fifty-two patients with large skull base meningiomas aged 34-74 years (median age 56 years) were treated with FSRT between June 2004 and August 2009. All patients received FSRT for residual or progressive meningiomas more than 4 centimeters in greatest dimension. The median GTV was 35.4 cm3 (range 24.1-94.9 cm3), and the median PTV was 47.6 cm3 (range 33.5-142.7 cm3). Treatment volumes were achieved with 5-8 noncoplanar beams shaped using a micro…

Endoscopy skull-base resection for ethmoid adenocarcinoma and olfactory neuroblastoma.

STATEMENT OF PROBLEM: Olfactory neuroblastoma (ON) and ethmoid adenocarcinoma (AC) are rare sinonasal malignancies that often involve the skull base. Standard surgical treatment is craniofacial resection (CFR), which allows for efficient removal but entails significant morbidity and mortality. Because expanded endoscopy nasal approaches are newly developed, we aimed to describe the procedure in patients with ON and AC and compare it with CFR in terms of efficiency and morbidity. METHODS: This work reports on a retrospective series of 16 patients with AC and ON treated endoscopically with ante…

Minimally invasive endoscopic endonasal management of skull base cholesterol granulomas.

BACKGROUND: Skull base cholesterol granulomas (SBCGs) are rare lesions that have traditionally been managed via middle fossa or transtemporal approaches. Despite the relative paucity in the literature, the endoscopic endonasal approach may serve as a potential alternative surgical route. In this study, we report our experience with the management of SBCG using this minimally invasive approach. METHODS: The charts and imaging studies of three patients were reviewed retrospectively. The mean age was 36 years (range 30-42). The male/female ratio was 2:1. The epicenter of the lesion was petrous a…

Predicting postoperative hydrocephalus in 227 patients with skull base meningioma.

OBJECT: Some patients develop communicating hydrocephalus after meningioma surgery, and this can develop into a serious clinical condition. However, this has rarely been addressed in the literature. Therefore, the authors sought to determine predictive patient variables for the occurrence of postoperative hydrocephalus following skull base meningioma surgery. METHODS: For this purpose, the authors retrospectively analyzed all patients who underwent resection of intracranial meningiomas between 1998 and 2009 at the Department of Neurosurgery, University Hospital Zurich, Switzerland. Of 594 pat…

[Diagnosis and treatment of injuries to the frontal skull base].

The role of ENT surgery in the management of anterior skull base defects has become increasingly important in recent years. Transnasal endoscopic surgical techniques and intraoperative navigation enable a minimally invasive approach in a large proportion of patients, thus helping to avoid morbidity typically associated with neurosurgical subfrontal approaches. Whereas traffic accidents and sport injuries are the main causes of anterior skull base trauma in the civilian setting, penetrating injuries caused by gunshots and improvised explosive devices (IEDs) play an increasing role in the milita…

Osteomalacia caused by skull base tumors: report of 2 cases.

BACKGROUND AND IMPORTANCE: Tumor-induced osteomalacia (TIO) is an uncommon paraneoplastic syndrome rarely encountered in neurosurgical practice. We report on 2 cases of TIO caused by skull base tumors. Although the diagnosis of TIO is difficult to make and often is delayed because of the insidious nature of the symptoms, mostly systemic pain and weakness, it is curable once it is diagnosed and properly treated. CLINICAL PRESENTATION: Both patients presented with severe pain developing in the lower extremities and moving out to the entire body, as well as difficulty moving. They were diagnosed…

Skull base chondrosarcoma: a case treated by the CyberKnife system.

Skull base chondrosarcomas represent 2% of all chondrosarcomas. They usually have a low grade of malignity and a complete resection may be curative. However, often they are considered irresectable because of the difficult approach and another kind of treatment is needed. We present a 54-year-old male case who, after weeks suffering from diplopia, was diagnosed with skull base chondrosarcoma and treated with the CyberKnife system. A review of radiosurgery techniques for skull base neoplasms is carried out.

[Transmaxillary approach to the skull base: surgical anatomy and guidelines].

BACKGROUND: A surgical and anatomic approach to the skull base using the transmaxillary route is presented. This route is well-known and used for a long time for sinus conditions. METHOD: This study was performed on injected cadavers. This study describes step by step this approach in microsurgical conditions following a vital lead: the infraorbital nerve. RESULTS: Anatomical landmarks are located in order to avoid complications. These complications are on one hand, hemorrhages by vascular lesions and on the other, definitive nerve palsy. CONCLUSION: Several skull base approaches exist, tra…

Frontal skull base surgery combined with endonasal endoscopic sinus surgery.

BACKGROUND: Postoperative infection remains a serious complication after radical resection of anterior skull base lesions because intracranial, nasal, and paranasal cavities are opened during surgery. To prevent complications from postoperative infection, we combined endonasal endoscopic sinus surgery (ESS) with the frontal transbasal approach in patients with skull base lesions. METHODS: Patients (n = 16) with anterior skull base lesions extending to the paranasal or nasal cavity underwent surgical resection via the frontal transbasal approach. After removal of the lesion via the transcrania…
