Obstructive sleep apneas naturally occur in mice during REM sleep and are highly prevalent in a mouse model of Down syndrome.

STUDY OBJECTIVES: The use of mouse models in sleep apnea study is limited by the belief that central (CSA) but not obstructive sleep apneas (OSA) occur in rodents. We aimed to develop a protocol to investigate the presence of OSAs in wild-type mice and, then, to apply it to a validated model of Down syndrome (Ts65Dn), a human pathology characterized by a high incidence of OSAs. METHODS: In a pilot study, nine C57BL/6J wild-type mice were implanted with electrodes for electroencephalography (EEG), neck electromyography (nEMG), and diaphragmatic activity (DIA), and then placed in a whole-body-p

Sleep in depression: the influence of age, gender and diagnostic subtype on baseline sleep and the cholinergic REM induction test with RS 86.

One hundred and eight healthy controls and 178 patients with a major depressive disorder according to DSM-III were investigated in the sleep laboratory after a 7-day drug wash-out period. Subsamples of 36 healthy controls and 56 patients additionally took part in the cholinergic rapid eye movement (REM) sleep induction test with RS 86. Data analysis revealed that age exerted powerful influences on sleep in control subjects and depressed patients. Sleep efficiency and amount of slow wave sleep (SWS) decreased with age, whereas the number of awakenings, early morning awakening, and amounts of wa

Exercise Intervention for REM Sleep Behavior Disorder in Parkinson's Disease: Mechanisms and Implications for Neurorehabilitation.

BACKGROUNDS: REM sleep behavior disorder (RBD) is a prodromal non-motor symptom of Parkinson's disease (PD) and other α-synucleinopathies, affecting approximately 30%-60% of PD patients. It is closely associated with α-synuclein (α-syn) aggregation, with its primary pathophysiology involving the impairment of brainstem circuits, resulting in movement disorders and circadian rhythm dysfunction. Although exercise is recognized for improving motor symptoms and delaying neurodegenerative disease progression, a comprehensive framework of its underlying mechanisms for RBD symptoms has not yet been t

Analysis of AQP4 SNPs and Their Association With REM Sleep Behavior Disorder and Hallucinations in Parkinson's Disease.

The glymphatic system is a glial-dependent network responsible for the clearance of waste products from the brain through cerebrospinal fluid (CSF) circulation. This process, which involves astrocytes and the water channel AQP4, facilitates the removal of harmful proteins like β-amyloid and tau, making it crucial in neurodegenerative diseases such as Alzheimer's (AD) and Parkinson's disease (PD). AQP4 dysfunction has been linked to the accumulation of these proteins and related pathologies. This study aimed to investigate the association between AQP4 gene polymorphisms (rs2075575, rs162009, an

Platelet miRNAs as early biomarkers for progression of idiopathic REM sleep behavior disorder to a synucleinopathy.

Individuals diagnosed with isolated REM sleep behavior disorder (IRBD) have a high risk of developing Lewy body disorders (LBD), mainly Parkinson's disease (PD) or dementia with Lewy bodies (DLB). As we have previously identified seven platelet-derived miRNAs as potential biomarkers for DLB, in this pilot study we aimed to investigate whether specific expression changes of these miRNAs are also present in IRBD. RNA was obtained from platelets of individuals with IRBD (n = 29) and controls (n = 34), and miRNA levels were determined with a miRCURY LNA miRNA Custom PCR Panel. miRNA interactomes o

Prevalence and correlated factors of REM sleep behavior disorder in patients with progressive supranuclear palsy.

BACKGROUND: Progressive supranuclear palsy (PSP) is a rapidly advancing tauopathy, and amongst rapid eye movement (REM) sleep behavior disorder (RBD) has been observed. Prior studies were limited by small samples and inconsistent results; meanwhile, research is lacking since the updated criteria for PSP. This cross-sectional study aimed to assess RBD in a sizable PSP cohort. METHODS: Participants were consecutively recruited from the movement disorders specialty at West China Hospital between January 2019 and January 2021, adhering to established inclusion and exclusion criteria. We collected

The dual effect of cholesterol on cognitive functions in REM sleep behavior disorder: A pilot study.

Isolated REM sleep behavior disorder (iRBD) is widely recognized as a prodromal stage of α-synucleinopathies. However, the factors influencing its clinical progression to a neurological syndrome remain poorly understood. Cholesterol metabolism has increasingly been implicated in the pathophysiology of neurodegenerative diseases, including Parkinson's disease and Dementia with Lewy Bodies. Yet, its role in iRBD has not been previously explored. This pilot study is the first to investigate the relationship between cholesterol levels and cognitive performance in individuals with iRBD. We assessed

Evolution of Motor and Nonmotor Characteristics in an Idiopathic/Isolated REM Sleep Behavior Disorder Cohort.

BACKGROUND AND OBJECTIVES: Years before diagnosis of Parkinson disease (PD), dementia with Lewy bodies (DLB), or multiple system atrophy (MSA), mild prodromal manifestations can be detected. Longitudinal follow-up of people with prodromal synucleinopathy, particularly idiopathic/isolated REM sleep behavior disorder (iRBD), enables in-depth clinical phenotyping of early disease, which could facilitate stratification for clinical trials, provide the definition of appropriate end points, or predict phenoconversion more precisely. The aim of this study was to update and expand on previous studies

Enrichment of gut-derived metabolites in a Parkinson's disease subtype with REM sleep behavior disorder.

Recent reports indicate that Parkinson's disease (PD)-like changes in gut microbial signatures develop in idiopathic REM sleep behavior disorder (iRBD), a prodrome of α-synucleinopathies. However, <50% of PD cases exhibit RBD at onset, underscoring PD's heterogeneity. Using untargeted metabolomics, plasma samples from PD patients with and without RBD, iRBD subjects, and healthy controls were analyzed to characterize the metabolic differences in PD patients with and without RBD. Metabolomic analysis revealed the enrichment of gut microbial-origin metabolites, such as secondary bile acids and p-

Caffeine and Exercise: A Dual Approach to Combat Cognitive Decline Induced by REM Sleep Deprivation.

Sleep deprivation (SD) is known to impair memory and cognitive functions often linked to heightened oxidative stress and neuroinflammation. While caffeine and exercise individually offer neuroprotective effects, the combined influence of these interventions on preserving cognitive function and reducing hippocampal damage during REM-SD has yet to be fully explored. This study investigates the synergistic potential of caffeine and exercise in mitigating these neurological damages induced by REM-SD. Male Wistar rats were divided into five groups: control, SD, caffeine + SD, exercise + SD, and caf

Validation of automated detection of REM sleep without atonia using in-laboratory and in-home recordings.

STUDY OBJECTIVES: To evaluate the concordance between visual scoring and automated detection of rapid eye movement sleep without atonia (RSWA) and the validity and reliability of in-home automated-RSWA detection in patients with rapid eye movement sleep behavior disorder (RBD) and a control group. METHODS: Sleep Profiler signals were acquired during simultaneous in-laboratory polysomnography in 24 isolated patients with RBD. Chin and arm RSWA measures visually scored by an expert sleep technologist were compared to algorithms designed to automate RSWA detection. In a second cohort, the accura

EEG Entropy in REM Sleep as a Physiologic Biomarker in Early Clinical Stages of Alzheimer's Disease.

BACKGROUND: Alzheimer's disease (AD) is associated with EEG changes across the sleep-wake cycle. As the brain is a non-linear system, non-linear EEG features across behavioral states may provide an informative physiologic biomarker of AD. Multiscale fluctuation dispersion entropy (MFDE) provides a sensitive non-linear measure of EEG information content across a range of biologically relevant time-scales. OBJECTIVE: To evaluate MFDE in awake and sleep EEGs as a potential biomarker for AD. METHODS: We analyzed overnight scalp EEGs from 35 cognitively normal healthy controls, 23 participants wi

Constipation is linked to accelerated cognitive and motor decline in isolated REM sleep behavior disorder.

INTRODUCTION: Constipation is associated with higher clinical severity and predicts cognitive decline in Parkinson's disease (PD). Whether the non-motor marker is associated with unfavorable motor and cognitive trajectories from the prodromal stage remains unclear. METHODS: In a longitudinal prospective cohort of subjects with isolated REM sleep behavior disorders (iRBD), subjects underwent repeated MDS-UPDRS and Mini-Mental Status Examination (MMSE) assessments. Generalized-estimating-equations (GEE) regression model was used to compare the time-dependent trajectories of MDS-UPDRS-III and MM

The prevalence of REM-related obstructive sleep apnoea is reduced by the AASM 2012 hypopnoea criteria.

PURPOSE: The variations in reported prevalence of rapid eye movement-related obstructive sleep apnoea (REM-OSA) have been attributed to different definitions, although the effect of hypopnoea criteria has not been previously investigated. METHODS: Within this retrospective study, 134 of 382 consecutive patients undertaking polysomnography (PSG) for the suspicion of OSA met the inclusion criteria. PSGs were scored using both the 2007 AASM recommended hypopnoea criteria (AASM2007Rec) and the 2012 AASM recommended hypopnoea criteria (AASM2012Rec). For each hypopnoea criteria, REM-OSA patients we

Effects of methylphenidate on the impairment of spontaneous alternation behavior in mice intermittently deprived of REM sleep.

Attention deficit/hyperactivity disorder (ADHD) is a neurodevelopmental disorder characterized by inattention, hyperactivity, and impulsivity. We have previously shown that abnormal behaviors elicited by intermittent rapid eye movement (REM) sleep deprivation stress may fulfill the profile of a model of ADHD. It is well known that the impairment of spontaneous alternation behavior (SAB) in the Y-maze indicates inattentive features of ADHD model animals. On the other hand, it has been reported that nitric oxide (NO) in the hippocampus is required for SAB. In this study, using mice, we investiga

Central Apneas and REM Sleep Behavior Disorder as an Initial Presentation of Multiple System Atrophy.

We present the case of a patient with multiple system atrophy who presented with central apnea as the only sleep disordered breathing, associated with REM behavior disorder and restless legs syndrome. This presentation of the disease is unusual and probably reflects more widespread involvement at the onset. With this case, we show the importance of considering this kind of disease in the differential diagnosis of central sleep apnea syndromes.

Injurious REM sleep behaviour disorder in narcolepsy with cataplexy contributing to criminal proceedings and divorce.

We present a case of violent sleep-related behaviour in a patient with narcolepsy with cataplexy (NC) and REM sleep behaviour disorder (RBD), causing injuries to his wife that resulted in charge of assault, followed by a divorce case. This observation pinpoints that RBD in NC can arise early in a patient's life and that it can be violent, that pharmacological treatment for RBD has to be considered in this case, that lack (or delay) of RBD diagnoses may further exacerbate the psychosocial burden of NC, and that a general awareness of the nature of sleep related violence may avoid unfortunate le

REM sleep behavior disorder preceding other aspects of synucleinopathies by up to half a century.

BACKGROUND: Idiopathic REM sleep behavior disorder (RBD) may be the initial manifestation of synucleinopathies (Parkinson disease [PD], multiple system atrophy [MSA], or dementia with Lewy bodies [DLB]). METHODS: We used the Mayo medical records linkage system to identify cases presenting from 2002 to 2006 meeting the criteria of idiopathic RBD at onset, plus at least 15 years between RBD and development of other neurodegenerative symptoms. All patients underwent evaluations by specialists in sleep medicine to confirm RBD, and behavioral neurology or movement disorders to confirm the subseque
