Diethyl Azelate for the Treatment of Brown Recluse Spider Bite, a Neglected Orphan Indication.

BACKGROUND/AIM: Brown recluse spider bite releases hemolytic and cytotoxic phospholipase D to the wound that may cause necrosis or even death. We examined diethyl azelate (DEA), a plasma membrane fluidizer with a broad range of immunomodulatory activities, as a potential treatment for the brown recluse spider bite. MATERIALS AND METHODS: Topical DEA was used in emergency to treat brown recluse spider bites in a human subject. We subsequently evaluated the effects of DEA on hemolysis induced by the brown recluse spider venom, recluse recombinant phospholipase D (rPLD), and venoms from honey be

Is severity of Daboia (Vipera) palaestinae snakebites influenced by season of exposure?

INTRODUCTION: The most common venomous snake in Israel, both in geographic spread and in number of snakebite incidents, is Daboia (Vipera) palaestinae. The clinical presentation of D. palaestinae envenomation varies and includes both local and systemic symptoms. Studies conducted on D. palaestinae revealed different amounts of venom in the snakes' glands in different seasons, however little is known regarding the potential impact of this finding on the clinical presentation after D. palaestinae bites during different seasons. OBJECTIVE: To evaluate whether there is a difference in the severit

Epidemiology of snakebite in Eshowe, KwaZulu-Natal, South Africa.

Three hundred and thirty-three patients admitted to Eshowe Hospital with snakebite were analysed. Forty-one were non-envenomed, 282 exhibited painful progressive swelling (PPS), seven showed progressive weakness (PW), and there were three cases of minor envenomations that were unclassifiable. Offending dead venomous snakes included Naja mossambica (Mozambique spitting cobra), Bitis arietans (puff adder), Atractaspis bibronii (stiletto snake), Causus rhombeatus (common night adder) and Dendroaspis polylepis (black mamba). Most bites occurred on the leg in the wet summer months during the first

Clinical studies of the effectiveness and safety of antivenoms.

In the 1890s, hyperimmune sera proved effective in animals against challenge by the snake venom against which they had been raised. They were first used, apparently successfully, in a human patient in about 1895. Since then, antivenoms have become accepted as the only reliable specific treatment for snake-bite envenoming. Despite decades of accumulated clinical experience and a number of published randomized comparative and observational studies, the clinical effectiveness and safety of some antivenoms remain open to question, due to a lack of robust randomized controlled trial data. Antivenom

Human envenomation from a wandering garter snake.

Garter snake bites are generally innocuous to human beings. We report a case of human envenomation from the Wandering Garter snake (Thamnophis elegans vagrans). The patient, who was bitten on his right third fingertip, rapidly developed local edema, ecchymosis, and hemorrhagic vesicles. Systemic signs and symptoms did not develop. The clinical picture was similar to that in three previous patients with Thamnophis envenomation in that clinical signs followed a prolonged bite. Thamnophis species have Duvernoy's glands, which may be analogous to venom glands in Crotalidae (pit viper) species. The…

Envenoming by the viperid snake Proatheris superciliaris: a case report.

Snake bites caused by viperid snakes of Atheris genus are extremely rare, envenoming of a bite of related viper Proatheris superciliaris was described only once in the literature. The present case study depicts the envenoming of a 57 years old Czech man, a private herpetologist, who was bitten to his finger. He developed painful local reaction, nausea, hematuria, hypertension, chest and lumbar pain. Coagulopathy and thrombocytopenia subsequently developed as well as acute renal failure, hepatic and lung lesion. Intensive care therapy was purely symptomatic and supportive as no antisera exists.…

Three instructive cases of tiger snake (Notechis scutatus) envenomation--and how a radioimmunoassay proved the diagnosis.

Three unusual cases are described, in which, in separate incidents, young snake bite victims were found comatose. Tiger snake vemon was detected by radioimmunoassay in the urine and serum, and in one case on the clothing of these children. Gross myoglobinuria was present in all cases. The three cases demonstrate clearly the myolytic potential of tiger snake venom if specific treatment (administration of antivenom) is delayed. Extreme elevation of serum enzyme levels was found in the two victims who survived. Evidence was obtained that the major neurotoxin of tiger snake venom is absorbed at le

Envenomation: a real risk of keeping exotic house pets.

The fashion of exotic animals maintained as pets is increasing in France. Cases of envenomation after exotic animals bites or stings were studied. All 54 non-native animal envenomations reported by hospitals at the Poison Centre of Marseilles (3 south-eastern regions of France) between 1997 and 2002 were surveyed. They involved 22 snakes, 18 fishes, 11 spiders, 1 scorpion and 2 marine invertebrates. The snakes belonged to crotalids (genus Crotalus, Sistrurus, Agkistrodon and Trimeresurus), viperids (genus Bitis, Echis and Macrovipera, responsible for extensive swelling and coagulation disturba

[Acute management of patients bitten by poisonous snakes].

The management of poisonous snake bites includes first aid and clinical medical treatment. First aid consists of reassurement of the patient, immobilisation of the bitten limb and rapid transport to the nearest hospital to monitor the vital functions. In no case suction, incision or tight bandages should be applied. The degree of envenomation can be classified in three categories depending on the spread of the symptoms: no symptoms, only local non-progressive symptoms, and systemic or local rapidly progressive symptoms (severe envenomation). Antivenin therapy is indicated in severe envenomatio…

Medically important venomous animals: biology, prevention, first aid, and clinical management.

Venomous animals are a significant health problem for rural populations in many parts of the world. Given the current level of the international mobility of individuals and the inquisitiveness of travelers, clinicians and travel clinics need to be able to give advice on the prevention, first aid, and clinical management of envenoming. Health professionals often feel overwhelmed by the taxonomy of venomous animals; however, venomous animals can be grouped, using a simple set of criteria, into cnidarians, venomous fish, sea snakes, scorpions, spiders, hymenoterans, and venomous terrestrial snake

An investigation, in vitro, of the actions of three Western Australian snakes on the blood coagulation of the dog, cat, horse and wallaby.

Venoms of the tiger snake and brown snake were procoagulant, in vitro, when tested with cat, dog, horse and wallaby plasma. In the absence of calcium and phospholipid the coagulant activity of tiger snake venom was minimal. In contrast, brown snake venom alone had marked procoagulant activity. This activity, however, was enhanced by the presence of calcium and phospholipid. Death adder venom exerted an anticoagulant effect. Apparent species' differences in susceptibility to the coagulant venoms were noted. However, the probable explanation of these differences was attributed to variation in th

Systemic Envenomation via Ocular Route from a Neurotoxic Snake Venom Ophthalmia--A Case Report.

Snake envenomation is a neglected tropical disease with two million snake bites reported each year (WHO). Much less common is snake venom ophthalmia secondary to eye exposure from spitting snakes. This paper reports an unusual case of systemic envenomation via the ocular route of a 14-year old male from the Philippines. A mixture of snake blood and venom came in contact with the patient's left eye causing severe pain and blurring of vision, shortly followed by generalized body weakness and ptosis. Copious irrigation with normal saline was done at the emergency room, and Purified Cobra Antiveni…

Enhydrina schistosa (Elapidae: Hydrophiinae) the most dangerous sea snake in Sri Lanka: three case studies of severe envenoming.

Sea snakes are highly venomous and inhabit tropical waters of the Indian and Pacific Oceans. Enhydrina schistosa is a common species of sea snake that lives in the coastal waters, lagoons, river mouths and estuaries from the Persian Gulf through Sri Lanka and to Southeast Asia. It is considered one of the most aggressive sea snakes in Sri Lanka where fishermen and people wading are at high risk. However, sea snake bites are rarely reported. In this report, we describe three cases where E. schistosa was the offending species. These three patients presented to two hospitals on the west coast of …

Anti-snake Venom-Induced Kounis Syndrome: A Unique Case in the Emergency Department.

Kounis syndrome is the concurrence of acute coronary syndromes associated with allergic or hypersensitivity and anaphylactic or anaphylactoid triggers. Although it is not a rare diagnosis, the different presentations and afflictions of all ages, sex, and racial groups make it a diagnostic challenge. Various triggers include food types, environmental exposure, and drugs. Cases triggered by serum sickness, tetanus antitoxin, and snake bites have been documented in the literature. However, to the best of our knowledge, no case triggered by anti-snake venom (ASV) has been reported yet, as seen in …
