Teaching phoneme awareness to pre-literate children with speech disorder: a randomized controlled trial.

BACKGROUND: Awareness of individual phonemes in words is a late-acquired level of phonological awareness that usually develops in the early school years. It is generally agreed to have a close relationship with early literacy development, but its role in speech change is less well understood. Speech and language therapy for children with speech disorder involves tasks that appear, either implicitly or explicitly, to require a phonemic level of awareness. However, children typically attend for intervention at a pre-school, pre-literate stage, i.e. before they would be expected to have developed

Association of Chiari I malformation, mental retardation, speech delay, and epilepsy: a specific disorder?

OBJECTIVE: The Chiari I malformation is defined as tonsillar herniation of at least 3 to 5 mm below the foramen magnum. Although Chiari I malformation is considered to derive from a mesodermal disorder resulting in underdevelopment of the posterior fossa relative to its content, evidence for a possible heterogeneous etiology also has been reported. The aim of the present study is to elucidate the relationship between Chiari I malformation and mental retardation, speech delay, and epilepsy to consider a possible specific pathogenetic background. METHODS: Thirty-five patients with Chiari I malf

The effects of minimal pairs treatment on the speech-sound production of two children with phonologic disorders.

Two children whose speech-sound production could be described by phonologic processes were administered a linguistically based treatment program that utilized minimal pair words. A subset of phonemes affected by a target phonologic process was taught consecutively. Spontaneous picture-naming probes were administered periodically to measure speech-sound production for all phonemes affected by the targeted phonologic process and several control phonologic processes. For both subjects, motoric components (i.e., models and phonetic placement cues) had to be added to the minimal pairs treatment. Wi

Screening for Speech and Language Delay and Disorders in Children: US Preventive Services Task Force Recommendation Statement.

IMPORTANCE: Speech and language delays and disorders can pose significant problems for children and their families. Evidence suggests that school-aged children with speech or language delays may be at increased risk of learning and literacy disabilities, including difficulties with reading and writing. OBJECTIVE: The US Preventive Services Task Force (USPSTF) commissioned a systematic review to evaluate benefits and harms of screening for speech and language delay and disorders in children 5 years or younger. POPULATION: Asymptomatic children 5 years or younger whose parents or clinicians do

Traditional and Visual-Acoustic Biofeedback Treatment via Telepractice for Residual Speech Sound Disorders Affecting /ɹ/: Pilot study.

PURPOSE: This study aimed to examine the feasibility of telepractice delivery of a treatment package including visual-acoustic biofeedback and motor-based treatment for residual speech sound disorder affecting /ɹ/ in school-age children. The overall study used a single-case randomization design; however, this preliminary report will simply quantify changes in accuracy before and after completion of the treatment package. The present analysis did not differentiate between the relative contributions of biofeedback and motor-based treatments. METHOD: Seven children aged 9-14 received speech ther

FOXP2 variants in 14 individuals with developmental speech and language disorders broaden the mutational and clinical spectrum.

BACKGROUND: Disruptions of the FOXP2 gene, encoding a forkhead transcription factor, are the first known monogenic cause of a speech and language disorder. So far, mainly chromosomal rearrangements such as translocations or larger deletions affecting FOXP2 have been reported. Intragenic deletions or convincingly pathogenic point mutations in FOXP2 have up to date only been reported in three families. We thus aimed at a further characterisation of the mutational and clinical spectrum. METHODS: Chromosomal microarray testing, trio exome sequencing, multigene panel sequencing and targeted sequen

Cross-linguistic generalization in the treatment of two sequential Spanish-English bilingual children with speech sound disorders.

PURPOSE: The effect of bilingual service delivery on treatment of speech sound disorders (SSDs) in bilingual children is largely unknown. Bilingual children with SSDs are typically provided intervention in only one language, although research suggests dual-language instruction for language disorders is best practice for bilinguals. This study examined cross-linguistic generalization of bilingual intervention in treatment of two 5-year-old sequential bilingual boys with SSDs (one with Childhood Apraxia of Speech), hypothesizing that selecting and treating targets in both languages would result

A comparison of surgical and prosthetic treatment for speech disorders attributable to surgically acquired soft palate defects.

Surgical and prosthetic treatments for speech disorders attributable to surgically acquired soft palate defects were investigated. Ten patients who had undergone soft palate resection for extensive cancer served as the subjects. In four patients, the resected portion of the soft palate was confined to the anterior or middle segments, with an intact posterior band, whereas in the remaining six patients the resected portion extended into the posterior edge. An obturator with or without a speech appliance was adapted to all patients in the former group and to one patient in the latter group. The …

Speech and language pathologists' diagnostic approaches to developmental language disorders in German-speaking children with intellectual disability.

INTRODUCTION: Children with an intellectual disability (ID) often manifest a developmental language disorder (DLD). However, conventional standardized DLD diagnostic tools often prove unsuitable for this population. Furthermore, no guidelines are provided on how speech and language pathologists (SLPs) should conduct the assessment. This study investigated how practitioners in German-speaking countries assess DLD in children with ID, including the role of multilingualism and practitioners' self-perceived competence. METHODS: A cross-sectional online survey was conducted with 102 practitioners

Multidomain Developmental Indicators in 4- to 9-Year-Old Children with Oral Motor and Speech Sound Disorders.

INTRODUCTION: Persistent speech sound disorders (SSDs) are common in childhood and affect communication, literacy, and social development. Identifying risk indicators (RIs) and predictors for atypical oral motor and speech development is crucial for early intervention. This study examined medical, developmental, oral-behavioural, and hereditary RIs in children with SSD and compared diagnostic subgroups to typically developing peers. METHODS: A clinical cohort of 198 children (ages 4-9) referred for specialist assessment was compared to 77 age-matched controls. Children were classified into fo

Intranetwork and Internetwork Functional Connectivity Changes Related to Speech Disorders in Adults With Cleft Lip and Palate.

Cleft lip and palate (CLP) may induce alterations in functional connectivity (FC) throughout the whole brain, potentially leading to speech dysfunctions; however, the precise neurobiological mechanisms involved remain unknown. This study aimed to systematically examine the consequences of neurological impairments associated with CLP on whole-brain FC and speech functionality. A total of 33 CLP individuals and 41 control participants were included in this study. Eight meaningful brain networks were identified through independent component analysis (ICA). The intergroup differences and correlati

Differential Diagnostic Patterns of Acquired Motor Speech Disorders in Children: A Preliminary Investigation of Four Case Studies.

PURPOSE: This clinical focus article aimed to explore how motor speech disorders (MSDs) manifest in children with acquired brain injury (ABI) and determine if an adult diagnostic system, such as the Mayo Clinic System, applies to pediatric cases. METHOD: Four children with ABI with differing loci of impairment were evaluated to compare perceptual speech features and how they align or diverge from expected adult profiles with similar loci of impairment. Two expert speech-language pathologists identified perceptual speech features from the recorded motor speech evaluations and determined motor …

[Orofacial myofunctional disorders: Evaluation of screening practices and speech therapy referrals by dentists and orthodontists in France].

INTRODUCTION: Orofacial myofunctional disorders (OMDs) refer to alterations in orofacial functions that may hinder craniofacial development. Early screening by dentists and orthodontists, together with timely referral for a speech-language evaluation and possible orofacial myofunctional therapy, is essential to optimize patient care. This study aims to assess the knowledge, practices, and training needs of French dentists and orthodontists regarding OMDs and the role of speech therapy in their management. MATERIAL AND METHODS: A descriptive survey was conducted using an online questionnaire d

What School-Based Speech-Language Pathologists Need to Know About Pediatric Swallowing and Feeding Development and Disorders.

PURPOSE: School-based speech-language pathologists are at the frontline for treating children with pediatric feeding disorders (PFDs). Often, speech-language pathologists are the primary allied health professionals who are charged with maximizing safety, supporting nutrition and neurodevelopmental growth. Due to the increasing need for assessment and treatment of PFDs in schools, it is fundamental that PFDs and dysphagia are detected, differentially diagnosed, and addressed to support developing children. This tutorial addresses the anatomy, physiology, and development of functional feeding an

Best practice recommendations for speech-language pathology in children with neuromuscular disorders: A Delphi-based consensus study.

PURPOSE: Speech-language pathology (SLP) is considered an essential intervention due to the high prevalence of dysphagia and dysarthria in paediatric neuromuscular disorders (pNMD). Evidence-based guidelines for SLP in pNMD are missing and children could be deprived the best of care. This study aimed to achieve consensus and present best practice recommendations on SLP intervention in pNMD. METHOD: A modified Delphi technique was used with a panel of experienced Dutch speech-language pathologists. In two online survey rounds and a face-to-face consensus meeting, the SLP experts proposed inter
