Pediatric spinal cord injury in the Netherlands: Data from the Dutch Spinal Cord Injury Database 2015-2022.

Context/Objective: The Dutch Spinal Cord Injury Database (DSCID) collects nationwide data on patients with newly acquired spinal cord injury (SCI) undergoing inpatient rehabilitation in the Netherlands. Pediatric SCI is rare and underrepresented in research. This study aimed to describe the pediatric SCI population, examine differences between age groups (0-8 vs. 9-17 years) and etiology (traumatic (T-SCI) vs. non-traumatic (NT-SCI)), and evaluate the applicability of the DSCID for this population. Design Retrospective observational study using data from DSCID. Setting Dutch inpatient rehabili…

Effect of epidural spinal cord stimulation in individuals with sensorimotor complete spinal cord injury: a pilot study.

Spinal cord injury (SCI) often results in irreversible sensorimotor and autonomic dysfunction, severely limiting independence and quality of life. Individuals with clinically complete SCI (AIS A) have traditionally been considered beyond functional recovery. This pilot study aimed to evaluate the therapeutic potential and broader impact of epidural spinal cord stimulation (eSCS) on this population. Three participants with chronic, sensorimotor complete thoracic SCI received implantation of a 32-contact epidural stimulator targeting the lumbosacral spinal cord. Personalized stimulation protocol…

Generation of engineered human spinal cord organoid in channel-patterned collagen and transplantation for spinal cord regeneration.

The construction of artificially manipulable tissue-engineered human spinal cord organoids (HSCO) in vitro is crucial for recapitulating spinal cord development and facilitating the transplantation repair of complete spinal cord injury (SCI). Here, we develop a millimeter-scale macroscopic complex scaffold assembled by Matrigel-filled channel-patterned type II collagen (CPCol II-M), in which a spheroid derived from human motor neural progenitors extends within the channel to generate the human spinal cord organoids in CPCol II-M (HSCO-CPCol II-M). The CPCol II-M scaffold, with enhanced mechani…

Timing-dependent synergies between noninvasive motor cortex and spinal cord stimulation in chronic cervical spinal cord injury.

OBJECTIVE: Spinal cord sensory networks strongly interact with descending motor circuits. We targeted this interaction by pairing motor cortex stimulation with coordinated cervical spinal cord stimulation. Using separate non-invasive and epidural experiments, we tested the hypothesis that the strongest muscle response would occur when paired brain and spinal cord stimuli simultaneously converge within the spinal cord. METHODS: For non-invasive experiments, we measured motor evoked potentials in response to transcranial magnetic stimulation (TMS) and transcutaneous spinal cord stimulation (TSC…

Multi-system benefits of non-invasive spinal cord stimulation following cervical spinal cord injury: a case study.

Cervical spinal cord injury (SCI) impairs sensorimotor and autonomic functions. We investigated the effects of lumbosacral transcutaneous spinal cord stimulation (tSCS) on urinary bladder, bowel, and sexual function as well as cardiovascular and sensorimotor functions in one individual with chronic clinically motor-complete cervical SCI, 11 years post-injury. Following 30 sessions of lumbosacral tSCS, the individual presented with improved urinary bladder compliance as well as anorectal function in parallel with mitigation of the severity of autonomic dysreflexia during filling cystometry and …

Psychosexual well-being following spinal cord injury: Perspectives of adults with spinal cord injury and rehabilitation clinicians.

OBJECTIVES: This study aimed to identify important mental health topics for people with spinal cord injuries (PwSCI) and garner insights from health care clinicians working with this population. In doing so, we identified psychosocial adaptations in the context of sexuality, intimacy, and reproductive health. METHOD: A qualitative, phenomenological approach was used, with 60-90-min virtual focus groups involving PwSCI, rehabilitation clinicians, and psychologists. Themes were identified through deductive and inductive transcript analysis. Eligibility for PwSCI included: (a) SCI for ≥ 1 year, …

Pain-induced autonomic dysreflexia secondary to spinal cord injury with significant improvement after spinal cord stimulator implantation.

INTRODUCTION: Spinal cord stimulation (SCS) has been established as a safe and effective alternative treatment for many conditions. This is a unique case involving SCS in spinal cord injury (SCI) patients with recurrent episodes of autonomic dysreflexia (AD). AD is a sympathetically driven reflexive hypertension in response to a noxious stimuli below the neurological level of spinal cord injury. There is currently limited research and literature regarding SCS application for AD. We present a unique case where pain-induced AD was successfully treated with implantation of SCS, in order to avoid …

Slow development of bladder malfunction parallels spinal cord fiber sprouting and interneurons' loss after spinal cord transection.

Neurogenic lower urinary tract dysfunction typically develops after spinal cord injury. We investigated the time course and the anatomical changes in the spinal cord that may be causing lower urinary tract symptoms following injury. Rats were implanted with a bladder catheter and external urethral sphincter electromyography electrodes. Animals underwent a large, incomplete spinal transection at the T8/9 spinal level. At 1, 2-3, and 4 weeks after injury, the animals underwent urodynamic investigations. Urodynamic investigations showed detrusor overactivity and detrusor-sphincter-dyssynergia app…

Congenital exercise ability ameliorates muscle atrophy but not spinal cord recovery in spinal cord injury mouse model.

Spinal cord injury (SCI) can cause loss of mobility in the limbs, and no drugs, surgical procedures, or rehabilitation strategies provide a complete cure. Exercise capacity is thought to be associated with the causes of many diseases. However, no studies to date have assessed whether congenital exercise ability is related to the recovery of spinal cord injury. High congenital exercise ability (HE) and low congenital exercise ability (LE) mice were artificially bred from the same founder ICR mice. The HE and LE groups still exhibited differences in exercise ability after 13 generations of breed…

Methylprednisolone Therapy in Acute Traumatic Spinal Cord Injury: Analysis of a Regional Spinal Cord Model Systems Database.

BACKGROUND: The objective of this study was to assess the relationship between exposure to methylprednisolone (MP) and improvements in motor function among patients with acute traumatic spinal cord injury (TSCI). MP therapy for patients with TSCI is controversial because of the current conflicting evidence documenting its benefits and risks. METHODS: We conducted a retrospective cohort study from September 2007 to November 2014 of 311 patients with acute TSCI who were enrolled into a model systems database of a regional, level I trauma center. We linked outcomes and covariate data from the mo…

[Spinal cord diffusion tensor tractography as a diagnostic tool in difficult cases of spinal cord intramedullary tumours].

Diffusion tensor-based spinal cord tractography is a technically complicated but rapidly evolving diagnostic method. The difficulties result from the volume of the spinal cord, the constant pulsing of the cerebrospinal fluid and respiratory movements. The method is being used more and more frequently to examine long spinal tracts in patients with intramedullary tumours. The method can be especially useful for ambiguous cases investigation. The presented case with coincidental intramedullary tumour and severe cervical spondylosis has been described by the authors to show their own experience wi…

Tetramethylpyrazine protects spinal cord and reduces inflammation in a rat model of spinal cord ischemia-reperfusion injury.

OBJECTIVE: Inflammation, which is known to be detrimental to the neurologic outcome during the acute phase after an ischemic stroke, provides a potential target for preventive or therapeutic approach for spinal cord ischemia-reperfusion injury. Tetramethylpyrazine (TMP), a pure compound derived from Ligusticum chuanxiong, is widely used in the treatment of ischemic stroke. The present study aimed to gain a deeper insight into the mechanism underlying the anti-inflammatory effects of TMP on spinal cord ischemia-reperfusion injury. METHODS: Spinal cord ischemia was induced in male Sprague-Dawle…

The isolated sympathetic spinal cord: Cardiovascular and sudomotor assessment in spinal cord injury patients: A literature survey.

OBJECTIVES: To present a comprehensive approach to the assessment of the severity of the autonomic lesion in spinal cord injury (SCI) patients, with regard to the level of lesion. To discuss how to assess an isolated sympathetic spinal cord that has lost supraspinal control (sympathetically complete lesion). METHOD: PubMed was searched for articles related to cardiovascular (mainly cold pressor test, respiratory and postural challenges) and sudomotor (sympathetic skin responses) tests that have been used. The results of these evaluations are analysed with regard to the site of stimulation (ab…
