The technique of radical treatment of tuberculosis of the spine at various levels is described. Eighty cases out of 158 of spinal caries underwent radical surgery over a period of 7 years. Caseous pus, sequestra, granulation tissue and discs were excised. Rib grafts were used in the dorsal spine and iliac strut grafts in the cervical and lumbar spine, through a direct anterior approach. Sound bony fusion was achieved in 90% of cases. The procedure is technically difficult but recovery is fast and the majority were fit to resume work within 6 months.

In this series of nine patients with diastrophic dwarfism, multiple problems with the spine were noted. Scoliosis was present in all patients and usually became rapidly progressive in the preadolescent years. The scoliosis had a strong tendency to become severe and rigid, with a large rotatory component. Early brace treatment appeared to have merit. Two cases of aneurysmal bone cyst of the spine are reported. These rather rare tumours may pose initial diagnostic problems. If, however, the clinical and radiological features are studied, diagnosis should be easy and the appropriate treatment will follow. Computed tomography provides an additional diagnostic aid. Management appears to be primarily surgical, but the role of radiotherapy has not yet been clearly defined.

Long term follow up of three patients with aneurysmal bone cysts has allowed us to draw attention to features which facilitate diagnosis and determine adequate treatment. The lesion occurs usually under the age of twenty with a slight female predominance. Treatment consists in complete local excision. Failing this, curettage should be carried out, and this gives good results. The exact cause of tennis elbow, a common condition, is still obscure. While the condition may well be entirely due to a local disorder at the elbow, the results of a study of 50 patients whose condition was resistant to 4 weeks of treatment directed to the elbow suggest that the underlying condition may have been (at least in these patients) a reflex localization of pain from radiculopathy at the cervical spine. A patient with fracture-dislocation of D9 on D8 had considerable posterolateral displacement. The neurologic injolvement of his left leg proved to be progressive. Treatment consisted of posterior decompression, exploration, open reduction and stabilization by means of Harrington rods, and fusion. All neurologic findings resolved completely. Final evaluation 18 months later showed a good and serviceable spine with no residual neurologic deficits. In my opinion, the treatment used in this case under given circumstances represented a reasonable modality of therapy.

A study of 60 children with thoracic and lumbar spine injuries indicates that the child's response to trauma is unique. It is concluded that the potential for growth and development and the presence of healthy tissues separate these injuries from comparable injuries in adults. There are three factors which determine the volume of the spinal canal, its lateral recesses and the intervertebral canals; the developmental size; the level cephalad and caudad of the vertebral column; the multiple acquired conditions which may cause encroachment. The causes of narrowing in spondylolisthesis are characteristic of each type. Three common groups are mentioned, congenital, isthmic and degenerative and the nature of obstruction and its appropriate treatment is described in each case.

The authors have treated two hydatic cysts, three plasmocytomas, one chondrosarcoma, one osteogenic sarcoma and four metastasis at the level of the spine. The anterior approach to surgical treatment of disorders of the cervical spine is described, and the technique and advantages are outlined. A series of 66 patients who underwent 75 procedures is reported. The follow-up period ranged from one to five years. Good to excellent results were obtained with 91% of the operations for disc disease, and good results in all cases of fracture-dislocation.
