Large mediastinal mass in a 15-year-old boy.

Hyperimmunoglobulin E syndrome is a rare multisystem inherited disorder characterised by high serum IgE levels, skin disorder causing eczema, dermatitis, recurrent staphylococcal infections and pulmonary infections and various skeletal and connective tissue abnormalities. Common presentation is with recurrent skin and sinopulmonary infections. Several features unrelated to immune system such as characteristic facial features, hyperextensibility of joints, multiple bone fractures and craniosynostosis have been described in the literature. We describe a rare presentation of this disease with inv…

α-Toxin Induces Platelet Aggregation and Liver Injury during Staphylococcus aureus Sepsis.

During sepsis, small blood vessels can become occluded by large platelet aggregates of poorly understood etiology. During Staphylococcal aureus infection, sepsis severity is linked to the bacterial α-toxin (α-hemolysin, AT) through unclear mechanisms. In this study, we visualized intravascular events in the microcirculation and found that intravenous AT injection induces rapid platelet aggregation, forming dynamic micro-thrombi in the microcirculation. These aggregates are retained in the liver sinusoids and kidney glomeruli, causing multi-organ dysfunction. Acute staphylococcal infection resu

Protecting against post-influenza bacterial pneumonia by increasing phagocyte recruitment and ROS production.

Seasonal and especially pandemic influenza predispose patients to secondary bacterial pneumonias, which are a major cause of deaths and morbidity. Staphylococcus aureus is a particularly common and deadly form of post-influenza pneumonia, and increasing staphylococcal drug resistance makes the development of new therapies urgent. We explored an innate immune-mediated model of the lung to define novel mechanisms by which the host can be protected against secondary staphylococcal pneumonia after sub-lethal influenza infection. We found that stimulating the innate immunity in the lung by overexpr

Citral, a monoterpenoid aldehyde interacts synergistically with norfloxacin against methicillin resistant Staphylococcus aureus.

BACKGROUND: Staphylococcus aureus (SA), is a major human pathogen causing wide range of clinical infections, which has been further complicated by drug resistance like methicillin resistant S. aureus (MRSA), vancomycin intermediate S. aureus (VISA)/vancomycin resistant S. aureus (VRSA), etc. The present study was aimed at determining anti-staphylococcal potential of citral against drug resistant clinical isolates alone and in combination with antibiotics. PURPOSE: To assess the potential of citral in combination with norfloxacin in treating drug resistant infections of SA. STUDY DESIGN: In t

Emerging resistance in staphylococci following long-term dalbavancin treatment for prosthetic joint infections.

BACKGROUND AND OBJECTIVES: Dalbavancin, a lipoglycopeptide with a half-life of 150-200 h, is a promising treatment option for prosthetic joint infections (PJIs) and other orthopaedic implant-associated infections (IAIs) caused by multidrug-resistant staphylococci. However, in vitro studies have shown that when exposed to low concentrations of dalbavancin, staphylococcal strains resistant to dalbavancin can emerge, potentially affecting its effectiveness in cases of recurrent infection. To investigate whether dalbavancin-resistant staphylococci emerge in vivo following long-term dalbavancin tre

Functional diversity of staphylococcal surface proteins at the host-microbe interface.

Surface proteins of Gram-positive pathogens are key determinants of virulence that substantially shape host-microbe interactions. Specifically, these proteins mediate host invasion and pathogen transmission, drive the acquisition of heme-iron from hemoproteins, and subvert innate and adaptive immune cell responses to push bacterial survival and pathogenesis in a hostile environment. Herein, we briefly review and highlight the multi-facetted roles of cell wall-anchored proteins of multidrug-resistant Staphylococcus aureus, a common etiological agent of purulent skin and soft tissue infections a

Antibacterial photodynamic therapy using water-soluble formulations of hypericin or mTHPC is effective in inactivation of Staphylococcus aureus.

Antibacterial photodynamic treatment (APDT) might prove valuable as an alternative to the application of chemoantibiotics for the treatment of staphylococcal infections. The rapid uptake of the photosensitizing agent into bacteria allows for selective killing of microorganisms whilst sparing the eukaryotic host tissue, but also requires removal of excessive dye after the incubation period. We tested water-soluble formulations of hypericin (PVP-hypericin) and m-tetrahydroxyphenylchlorin (Fospeg), which could be applied as aqueous sprays and removed easily by flushing with buffers, for their eff…

Virulence factors of Staphylococcus aureus strains causing infective endocarditis--a comparison with strains from skin infections.

The objective was to study potential bacterial virulence factors in S. aureus endocarditis. S. aureus strains isolated from patients with well-classified episodes of infective endocarditis (IE) (n=26) were compared with control S. aureus strains from consecutive patients with skin infections (n=30). The potential virulence factors studied were Staphylococcal enterotoxin A-D (SEA, SEB, SEC, SED) and toxic shock syndrome toxin-1 (TSST-1) production and binding capacity to the extracellular matrix proteins: fibronectin, collagen type I, collagen type II and bone sialoprotein (BSP). [Therapeutic effectiveness of neomycin in staphylococcal and Pseudomonas aeruginosa corneal lesions].

Ophthalmic films (OPHF) with neomycin were used in treatment of 15 patients with staphylococcal infections of the cornea. OPHF were effective in treatment of 5 patients with affection of the cornea surface layers. These patients were discharged from the hospital on days 4-5 of the treatment. In the other 10 patients deeper layers of the cornea were affected by the infiltrates and their treatment with OPHF required longer periods (10-15 days). Another 5 patients developed severe keratitis caused by Ps. aeruginosa after removal of foreign bodies from the eyes. These patients were treated with ne…

Acute bacterial infections of the eye: their aetiology and treatment.

The aerobic microbial flora of 823 eyes with acute bacterial conjunctivitis, corneal ulceration, dacryocystitis, discharging sockets, blepharitis, and postoperative infections has been investigated. Staphylococci, Streptococcus pneumoniae, and nonfermenting Gram-negative bacilli including Pseudomonas aeruginosa were the main pathogens. Fermenting coliforms were also infrequently isolated. The least overall resistance of 30.9% was to chloramphenicol, but no one antibiotic was effective against all pathogens. Cloxacillin is the best drug for staphylococcal and polymyxin B for pseudomonas infecti

Staphylococcal scalded skin syndrome in a woman with chronic renal failure exposed to human immunodeficiency virus.

A twenty-eight-year-old woman seropositive for the human immunodeficiency virus and undergoing hemodialysis for end-stage renal disease sustained an infection of her arteriovenous graft. Multiple erythematous erosions appeared and a diagnosis of staphylococcal scalded skin syndrome was made when a frozen section of a fresh skin peel revealed an intraepidermal split through the granular layer and Staphylococcus aureus group II, phage type 71 was cultured from the wound and blood. Twenty-six cases of adult patients with staphylococcal scalded skin syndrome have been reported. Although this inclu

[Osteoarticular infections. Role of immune deficiences. Value of sero-immunology in the diagnosis].

Although bone and joint infections do not seem to be more frequent in patients with immune deficiences than in normal subjects, it seems paradoxically that an immune deficiency is relatively frequent during fully diagnosed bone and joint infections: the discrepancy between the two types of data is not easy to explain. Serology and immunology laboratories give little information in the etiological diagnosis of bone and joint infections, with the exception of perhaps gonococcal infections (search for anti-gonococcal antibodies by immunofluorescence), staphylococcal infections (pasteurella, yersi…

Panhemispheric infarction: a complication of cuffed catheter.

The need for reliable vascular access remains the Achilles heel of hemodialysis. Complications of vascular access are a leading cause of morbidity and mortality in patients who undergo hemodialysis, especially in those patients with end-stage renal disease. Among methods of vascular access, arteriovenous fistulae have the lowest rate of infection and should be the access of choice when vascular anatomy permits. Also, the incidence of staphylococcal infections in patients infected with human immunodeficiency virus is increasing. To emphasize the need to use arteriovenous fistula access for hemo…

Pulmonary manifestations in hyper IgE syndrome: A case series and review of Indian literature.

Recurrent pulmonary infections starting from childhood often prompt evaluation for primary immunodeficiency disorders (PIDs). Hyper IgE syndrome (HIES) is a less common PID characterised by recurrent skin and pulmonary infections associated with elevated IgE levels. Staphylococcal infections are more commonly seen in these individuals, resulting in structural lung abnormalities such as pneumatoceles and bronchiectasis. The associated non-immunologic features (characteristic facies, retained primary dentition, scoliosis, osteopenia, and hyperextensible joints) should raise suspicion of this syn…

Prevention and control of neonatal pyoderma with chlorhexidine.

The extent of staphylococcal colonisation and neonatal pyoderma was studied in 5 220 infants in two large Swedish hospitals in 1979-1982. A method for routine cord care with 4% chlorhexidine detergent solution was evaluated in one hospital. Nursery colonisation with Staph. aureus and streptococci decreased significantly. The method proved to be clinically efficient for prevention of pyoderma in the nursery as well as for controlling staphylococcal infection after discharge from hospital. The combined use of a quantitative sampling method and phage-typing showed that most post-discharge lesions
