Strabismus surgery in a patient with a Boston K-pro keratoprosthesis.

We report the case of a 33-year-old woman with sensory exotropia who developed diplopia following implantation of a Boston K-pro keratoprosthesis. She underwent uneventful, successful strabismus surgery with recovery of stereopsis. To the best of our knowledge, surgical treatment of diplopia related to sensory exotropia in a K-pro patient has not been previously reported.

The association of refractive error, strabismus, and amblyopia with congenital ptosis.

PURPOSE: To determine the frequency of amblyopia and its associations among children with congenital ptosis. METHODS: A retrospective chart review of 92 active patients with congenital ptosis in the practices of 2 of the authors. The main outcome measures were relative eyelid ptosis severity, presence and type of strabismus, refractive error, and amblyopia. RESULTS: Amblyopia was identified in 22 of 92 patients (23.9%), in almost every case occurring in the context of coexisting anisometropia or strabismus and affecting the eye with the more severe ptosis (P = 0.0001). Amblyopia was more lik…

Variable angle of strabismus related to timing of opiate ingestion.

Heroin (diamorphine) is a highly addictive opiate with potential for misuse. A small number of reports have linked the commencement of heroin misuse to acute exotropia with diplopia and subsequent withdrawal to esotropia in individuals without previous symptoms.(1-5) We describe a young adult who sought strabismus surgery to correct a large-angle exotropia. Detailed patient history and orthoptic measurements at different times of the day revealed a fluctuating angle of divergence relating to the timing of opiate ingestion, rendering surgery inappropriate. We suggest that opiate misuse, which m…

Strabismus surgery for internuclear ophthalmoplegia with exotropia in multiple sclerosis.

BACKGROUND: Internuclear ophthalmoplegia (INO) is a disabling condition affecting up to 40% of patients with multiple sclerosis (MS). Management of bilateral internuclear ophthalmoplegia (BINO) with exotropia in MS has been controversial because of the uncertain course of MS. Diplopia associated with INO severely impacts the patient's quality of life and, therefore, treatment should be considered. METHODS: Three patients (ages 28, 62, and 82 years) who had BINO with exotropia and disabling diplopia secondary to MS underwent bilateral medial rectus resections with either unilateral or bilatera

The application of statistical process control to horizontal strabismus surgery.

PURPOSE: Although originally developed to evaluate quality in manufacturing, statistical process control (SPC) techniques may be applicable to other frequently performed standardized processes. SPC analysis has rarely been used in medicine. We employed SPC charts to analyze the outcomes of one- and two-muscle horizontal strabismus surgery for esotropia and exotropia. METHODS: During a 33-month period between October 1, 1998 and July 1, 2001, we evaluated 95 patients undergoing strabismus surgery on either one or two previously unoperated eye muscles in one or both eyes. SPC charts were used t…

Development of refractive error and strabismus in children with Down syndrome.

PURPOSE: To investigate the development of refractive errors and strabismus in a cohort of children with Down syndrome. METHOD: Data for 55 children with Down syndrome who are participating in this longitudinal study of visual development, first examined by us when aged less than 2 years and on at least two other occasions, were analyzed. Mohindra retinoscopy was used to measure refractive error. Ocular alignment was assessed using the Hirschberg test and, when possible, the cover test. RESULTS: Despite the high prevalence of large refractive errors in children with Down syndrome, longitudin…

Effect of ADCON-L on adjustable strabismus surgery in rabbits.

BACKGROUND/AIMS: In search of a way to prevent postoperative adhesion after strabismus surgery, an animal study was performed to assess the effect of a gel consisting of a polyglycan ester in a gelatin matrix (ADCON-L). METHODS: Bilateral recessions of superior rectus muscle (SR) were performed on 16 rabbits. ADCON-L was applied beneath and over the SR in the right eyes of all rabbits, while the operative fields in the left eyes were irrigated with a balanced salt solution (BSS). The adjustment was performed on each SR at 4 and 7 days postoperatively on the same eye. The length and force of t…

Vertical location of the corneal light reflex in strabismus photography.

OBJECTIVE: To improve the clinical documentation of strabismus by mathematically predicting and clinically verifying the location of a fixation target that produces a vertically centered corneal light reflex (first Purkinje image) in clinical photographs of the eye using a standard photographic flash unit. MATERIALS AND METHODS: Mathematical modeling of the corneal light reflex during clinical photography was based on the schematic eye. Clinical photographs were taken using a range of fixation targets located between the center of the camera lens and the center of the flash. Image quality was…

Deficiency of adaptive control of the binocular coordination of saccades in strabismus.

Disconjugate (different in the two eyes) oculomotor adaptation is driven by the need to maintain binocular vision. Since binocular vision is deficient in strabismus, we wondered whether oculomotor disconjugate adaptive capabilities are deficient in such subjects. We studied eight adult subjects with constant, long-standing convergent strabismus of variable angles (4-30 prism D). No subject had severe amblyopia. Binocular vision was evaluated with stereoacuity tests. Two subjects had peripheral binocular vision and gross stereopsis; two other subjects had abnormal retinal correspondence and abn…
