Conventional surgical technique of subcutaneous venous port placement describes dissection of the port pocket to the pectoralis fascia and suture fixation of the port to the fascia to prevent inversion of the device within the pocket. This investigation addresses the necessity of that step. Between October 8, 2004 and October 19, 2007, 558 subcutaneous chest ports were placed at our institution; 24 cases were excluded from this study. We performed a retrospective review of the remaining 534 ports, which were placed using standard surgical technique with the exception that none were sutured int

BACKGROUND: Treatment of acute pediatric Monteggia fractures requires ulnar length stability to maintain reduction of the radiocapitellar joint. When operative care is indicated, intramedullary ulna fixation can be buried or left temporarily exposed through the skin while under a cast. The authors hypothesized that treatment with exposed fixation yields equivalent results to buried fixation for Monteggia fractures while avoiding secondary surgery for hardware removal.
METHODS: A retrospective review of children with acute Monteggia fractures at our Level 1 pediatric trauma center was performe

Patients with craniofacial microsomia (CFM) may present with severely malformed mandibles resulting in secondary tongue-based airway obstruction. While surgical management of clinically significant airway obstruction in these patients often involves mandibular distraction osteogenesis (MDO), patients with Pruzansky III CFM typically require additional costochondral bone grafting. In this report, the authors present the successful surgical management of airway obstruction and severe obstructive sleep apnea using a cranio-mandibular fixator (Matthews device) combined with MDO in a 4-year-old fem

BACKGROUND AND OBJECTIVE: Lumbar spinal stenosis (LSS), or the narrowing of the spinal canal, continues to be the leading preoperative diagnosis for adults older than 65 years who undergo spine surgery. Although the treatment of LSS depends on its severity, the optimal surgical technique towards decreasing the risk of adjacent segment disease (ASD) remains elusive. This study aimed to comparatively analyze spinal biomechanics with rigid and flexible fixation devices (i.e., rigid and dynamic posterolateral fusion (PLF) and interspinous process (ISP) devices) during daily activities.

Surgical mesh is a metallic or polymeric screen intended to be implanted to reinforce soft tissue or bone where weakness exists. Surgical mesh has been used since the 1950s to repair abdominal hernias. In the 1970s, gynecologists began using surgical mesh products to indicate the repair of pelvic organ prolapse (POP), and in the 1990s, gynecologists began using surgical mesh for POP. Then the U.S. Food and Drug Administration (FDA) approved the first surgical mesh product specifically for use in POP. Surgical mesh materials can be divided into several categories. Most surgical mesh devices cle

In spite of the recent achievements derived from modern protocols of prophylaxis, orthopedic surgical infections still remain unacceptably frequent, especially in light of the often devastating outcomes of septic complications. The spectrum and the prevalence of the bacteria most frequently involved in orthopedic infections are here explored, with particular reference to those infections associated to implant biomaterials, which were grouped based on device typology. During a 30 months period (from September 2000 to April 2003), 1027 microbial strains were consecutively isolated from 699 patie

INTRODUCTION: Unstable fractures of the distal forearm often require surgical treatment to restore the normal anatomy and function. We have used a relatively new technique, nonbridging external fixation, in the treatment of these fractures in our hospital during the past few years. Our results are presented here.
MATERIALS AND METHODS: Fifty-two patients (41 female, 11 male) with an unstable fracture of the distal forearm were treated using nonbridging external fixation at Oulu University Hospital during 1996-1999. The patients' mean age was 57 years. There were 45 Colles-type fractures, and

Osteoporotic spine fractures are among the most prevalent fractures in patients with osteoporosis and pose significant challenges to surgical treatment. Achieving stable fixation of osteoporotic bone is technically challenging and has become an increasing concern in spinal neurosurgery in the aging global population. Numerous studies have described the biomechanical benefits of fixation devices and techniques; however, no standard of care has been identified to provide distinct spinal stabilization in these patients. This study reviews the existing evidence and explores the complexities relate

OBJECT: The risk factors of halo fixation in elderly patients have never been analyzed. The authors therefore retrospectively reviewed data obtained in the treatment of such cases.
METHODS: A discharge database was searched for patients 70 years of age or older who had undergone placement of a halo device. In a search of cases managed between April 1999 and February 2005, data pertaining to 53 patients (mean age 79.9 years [range 70-97 years]) met these criteria. Forty-one patients were treated for traumatic injuries. Ten patients had deficits ranging from radiculopathy to quadriparesis, and

Surgical arthrodesis of the degenerative lumbar spine eliminates natural mobility across the motion segment and permanently alters the stress distribution resulting in accelerated degeneration at adjacent levels. This article and accompanying video describe the surgical details of an alternative motion preservation procedure: total joint replacement (TJR) of the lumbar spine. This procedure is indicated for patients who exhibit radiographic evidence of severe degeneration of the three-joint complex (intervertebral disc and facet joints). The TJR procedure involves a bilateral transforaminal lu

Major advances in the surgical treatment of fractures of long bones in children with Osteogenesis Imperfecta (O.I.) have been made in the last several years. The current mainstay of treatment is bracing and active rehabilitation. When this fails surgery is indicated. The type of fixation device used is generally some type of stainless steel rod. The rod stabilizes the fracture as well as accepts the load across the fracture site thus enabling the fracture to heal. However, the current rods in use have complications. One such complication is cortical atrophy felt to be secondary to the removal

The methods described are those currently employed by the authors for the injuries described. As surgical results accumulate, we hope to modify and improve our methods. Many injuries to the carpus and tarsus have been omitted. Many questions remain unanswered. How long must external coaptation be applied? When is the optimum time for removal of internal fixation devices? Are sclerosing agents more successful for the management of interphalangeal luxations than primary suturing? Should autogenous cancellous bone be used in all arthrodeses? What is the best method of handling sesamoid fractures,

BACKGROUND: Stemless total shoulder arthroplasty is a well-established and reliable surgical treatment option for glenohumeral osteoarthritis resulting in loss of pain and improvement of shoulder function. Currently the two methods for the fixation of the humeral component are either screw fixation or impaction. The purpose of this study is the clinical and radiological comparison of two different stemless designs (screw fixation vs impaction) for total shoulder arthroplasties in patients suffering from primary glenohumeral osteoarthritis.
METHODS: A retrospective cohort study including 39 pa
