Proximity of uterosacral ligament suspension sutures and S3 sacral nerve to pelvic landmarks.

INTRODUCTION AND HYPOTHESIS: To describe the relationships between pelvic bony landmarks to points along the third sacral nerve and to uterosacral ligament suspension sutures. METHODS: Three transvaginal uterosacral ligament suspension sutures were placed bilaterally in unembalmed female human cadavers. The third sacral nerve was marked at the foramen (S3a) and at two additional points at 1-cm intervals along its course caudally (S3b, S3c). Three bony pelvic landmarks were identified and marked, including the ischial spine, pubic symphysis, and coccyx. Distances from each landmark to each sut…

Lipoabdominoplasty without drains or progressive tension sutures: an analysis of 100 consecutive patients.

BACKGROUND: Subcutaneous surgical drains are commonly used in abdominoplasties to prevent seromas but are not tolerated well by patients and add additional discomfort after the procedure. The lipoabdominoplasty modification may create a more favorable surgical field to reduce the need for surgical drains without increasing seroma formation. OBJECTIVES: The goal of this review was to determine if surgical drains can be completely eliminated in lipoabdominoplasty procedures without an increased risk of seromas. METHODS: The authors conducted a retrospective chart review of 100 consecutive stan

Is frosting effective? The role of retention sutures in posttraumatic orbital reconstruction surgery.

BACKGROUND: Plastic surgeons, otolaryngologists, and maxillofacial surgeons routinely perform periorbital fracture fixation in the setting of acute trauma. One of the most common complications of periorbital surgery continues to be lower-lid retraction. The Frost suture has been widely adopted as one of the methods of reducing the frequency of this complication, although there are no compelling data on its efficacy. MATERIALS AND METHODS: A retrospective chart review was performed on trauma patients who had undergone periorbital fracture fixation by one of three reconstructive surgeons at a s

Vascularization of conjunctival autografts in pterygium surgery: comparison of fibrin glue with sutures.

PURPOSE: To monitor the development of graft vascularization after pterygium excision with conjunctival autograft transplantation (CAT) using indocyanine green angiography (ICGA) and to compare the graft vascularization between 2 different fixation techniques (fibrin glue and sutures). METHODS: A total of 26 eyes of 26 patients with primary pterygium were randomly assigned after pterygium excision as having either fibrin glue (13 eyes) or Vicryl sutures (13 eyes) for CAT. Anterior segment ICGA findings were evaluated postoperatively at 1, 7, and 15 days and the percentages of graft vasculariz…

Vesical calculus formation on non-absorbable sutures used for open inguinal hernia repair.

INTRODUCTION: Iatrogenic injuries to the urogenital tract are rare, with the bladder being the organ most affected. We describe a case of a vesical calculus that formed on non-absorbable sutures that were used to repair an inguinal hernia. PRESENTATION OF CASE: A 45-year-old male presented with frank haematuria and dysuria 2 years following an open left inguinal hernia repair. A CT urography showed a vesical calculus adherent to the left anterio-lateral wall of the bladder. Cystoscopy revealed that the calculus formed on non-absorbable sutures. Cystolapaxy was performed followed by cystoscopi…

Performing simple and safe dunking pancreaticojejunostomy using mattress sutures in pure laparoscopic pancreaticoduodenectomy.

BACKGROUND: Although recent technological developments and improved endoscopic procedures have further spread the application of laparoscopic pancreatic resection, laparoscopic pancreaticoduodenectomy still presents major technical difficulties, such as when performing pancreatic-enteric anastomosis. METHODS: Laparoscopic dunking pancreaticojejunostomy using mattress sutures was performed in 15 consecutive patients with a soft pancreas and a nondilated pancreatic duct between October 2011 and December 2012. RESULTS: According to the International Study Group on Pancreatic Fistula criteria, 3…

Femoral venous hemostasis in children using the technique of "figure-of-eight" sutures.

BACKGROUND: Structural heart interventions require the use of relatively large-diameter delivery sheaths or latex nylon net balloon catheters, which results in a relatively large-diameter venous puncture point. At present, femoral venous hemostasis is achieved by manual compression. A temporary figure-of-eight subcutaneous suture has been introduced to achieve immediate postprocedural femoral venous hemostasis after using a ≥24Fr sheath in an adult. This method is not well evaluated in children. METHOD: We report our experience using this technique in children treated with 7-14Fr sheaths. Fro

Comparison of cosmetic outcomes of absorbable versus nonabsorbable sutures in pediatric facial lacerations.

OBJECTIVE: We sought to compare cosmetic outcomes, complication rates, and patient/caregiver satisfaction of absorbable versus nonabsorbable sutures in children. METHODS: Healthy patients, 1 to 18 years old, with facial lacerations 1 to 5 cm, were randomized to repair with fast-absorbing catgut (FAC) or nylon (NYL) sutures. Patients returned in 4 to 7 days and in 3 to 4 months, at which time photographs and caregiver surveys were completed. Unlike part I, all FAC sutures were permitted to absorb rather than be removed. Using a 100-mm visual analog scale (VAS), a noninferiority (NI) design was…

Effectiveness of ultrasonographic evaluation of the cranial sutures in children with suspected craniosynostosis.

Computed tomography (CT) is the 'gold standard' for evaluation of the cranial sutures. While prenatal cranial suture evaluation with ultrasound (US) is common, US has not been established as a postnatal screening tool. We evaluated the effectiveness of US for diagnosis of craniosynostosis (CS). During 2006, 24 infants with questionable CS were assessed with US of the sagittal, metopic, and bilateral coronal and lambdoid sutures. US findings and clinical records were reviewed retrospectively. Sixteen boys and eight girls (ages 1-11 months, mean 4.3) underwent US. The correct diagnosis was provi…

Laparoscopic incisional hernia repair in a porcine model: what do transfixion sutures add?

BACKGROUND: The aim of this study was to evaluate the need for transfixion sutures during laparoscopic ventral hernia repair with mesh. METHODS: Incisional hernias were created in 14 Yucatan mini-pigs. Animals were randomized to undergo laparoscopic hernia repair either with spiral tacks alone (Tacks) or with tacks and 4 Prolene transfixion sutures (Sutured) using Composix E/X mesh (Davol Inc.). At 4 weeks, exploratory laparoscopy was performed to assess the repair and score adhesions. The abdominal wall was harvested for tensile strength analysis and histologic evaluation. Continuous variabl…

Photochemical keratodesmos as an adjunct to sutures for bonding penetrating keratoplasty corneal incisions.

PURPOSE: To evaluate the benefit of photochemical keratodesmos (PKD) in acute wound closure of penetrating keratoplasty (PKP) corneal incisi … icant by paired t test (P<.05). CONCLUSIONS: Photochemical keratodesmos may be a useful adjunct to sutures for approximating PKP corneal incisions in the immediate postoperative period. This process does not induce high temperature, and thus denaturation can be avoided and structural integrity restored.

Necrotizing scleritis after intraocular surgery associated with the use of polyester nonabsorbable sutures.

We present 2 elderly patients who developed necrotizing scleritis after cataract extraction in which the wound was sutured with polyester. In the first case, a 73-year-old woman who had right phacoemulsification via a limbal incision developed necrotizing scleritis 8 months after surgery. The eye became progressively painful and phthisical, necessitating enucleation. Microscopic examination of the enucleated globe showed a predominantly lymphocytic infiltration of the ocular tissues with no evidence of an infectious agent. In the second case, a 78-year-old woman had bilateral extracapsular cat

Comparison of 3 Different Surgical Techniques for Rotator Cuff Repair in a Rabbit Model: Direct Suture, Inlay Suture, and Polyether Ether Ketone (PEEK) Suture Anchor.

BACKGROUND: Rotator cuff tears have been repaired using the transosseous method for decades. The direct suture (DS) technique has been widely used for rotator cuff tears; however, the retear rate is relatively high. Suture anchors are now used frequently for rotator cuff repair (RCR) in accordance with recent developments in materials. However, polyether ether ketone (PEEK) may still cause complications such as the formation of cysts and osteophytes. Some studies have developed the inlay suture (IS) technique for RCR. PURPOSE/HYPOTHESIS: To compare how 3 different surgical techniques-namely,

Obstetric Sutures Gone Astray: A Bladder Erosion of a Cerclage Suture Leading to Recurrent Urinary Tract Infections.

Cervical cerclage is generally considered a safe procedure; however, rare complications, such as suture erosion and migration into adjacent organs, can occur. We present a case of a 31-year-old multiparous female who presented with a three-year history of recurrent urinary tract infections (UTIs), dysuria, urinary frequency, intermittent gross hematuria, and pelvic pain. She had a history of two prior cervical cerclages. Multiple courses of antibiotics failed to resolve her symptoms. CT imaging revealed a curvilinear hyperdense foreign body within the bladder. A cystoscopy was performed. Intra

Optimization of lesser tuberosity osteotomy repair: a biomechanical assessment of suture tensioning, repair configuration, and type of suture.

BACKGROUND: Lesser tuberosity osteotomy (LTO) is an effective method to manage the subscapularis for total shoulder arthroplasty but requires adequate compression and stability for healing after repair. The optimal configuration and type of sutures for LTO repair are unknown. The effect of variable surgeon strength on repair is similarly an unknown factor and may play a role in construct repair security. This study measured, in vitro, the effect of suture number and the effect of surgeon manual strength on repair. We hypothesized that increasing the number of sutures or selecting suture tapes …
