Medial temporal lobotomy in the treatment of medial temporal lobe epilepsy: a case series of 12 patients.

PURPOSE: This case series aimed to investigate the safety and efficacy of medial temporal lobotomy (MTL) in the treatment of medial temporal lobe epilepsy (MTLE). METHODS: We presented our experience with 12 MTLE patients who underwent MTL in our center from January 2018 to July 2021. The MTL technique involves suction-based removal of the amygdala, effectively severing the fibrous connections between the medial temporal lobe structures and the surrounding cortex while maintaining the integrity of the medial temporal lobe structures without removing the hippocampus as in selective amygdalohip

A neural computational framework for face processing in the human temporal lobe.

A key question in cognitive neuroscience is how unified identity representations emerge from visual inputs. Here, we recorded intracranial electroencephalography (iEEG) from the human ventral temporal cortex (VTC) and medial temporal lobe (MTL), as well as single-neuron activity in the MTL, to demonstrate how dense feature-based representations in the VTC are translated into sparse identity-based representations in the MTL. First, we characterized the spatiotemporal neural dynamics of face coding in the VTC and MTL. The VTC, particularly the fusiform gyrus, exhibits robust axis-based feature c

Strain analysis of cardiac chambers in individuals with temporal lobe epilepsy.

Cardiovascular impairment in people with epilepsy (PWE) has been attracting medical interest. This study investigated the myocardial strain of the four cardiac chambers of individuals with temporal lobe epilepsy, without known heart disease, compared to an age-, sex-, and body mass index-matched control group. Eighty-six individuals (43 with epilepsy and 43 controls) underwent clinical evaluation, electrocardiography, and transthoracic echocardiography. Individuals with epilepsy had reduced left ventricular ejection fraction (p = .001), left ventricular longitudinal strain (p = .011), right ve

The Progress of Cognitive Dysfunction Impairment Caused by Temporal Lobe Epilepsy.

Epilepsy is a chronic disease that is characterized primarily by seizures. One of the most common and detrimental comorbidities of epilepsy is cognitive impairment. Approximately, 30-40% of patients with chronic epilepsy experience cognitive impairment, which significantly impacts their daily functioning and quality of life and imposes a heavy burden on both patients and society. Numerous studies have established a correlation between epilepsy and cognitive impairment, showing that epilepsy patients exhibit varying degrees of cognitive deficits influenced by various factors. However, the clini

Shared and language-specific phonological processing in the human temporal lobe.

All spoken languages are produced by the human vocal tract, which defines the limited set of possible speech sounds. Despite this constraint, however, there exists incredible diversity in the world's 7,000 spoken languages, each of which is learned through extensive experience hearing speech in language-specific contexts1. It remains unknown which elements of speech processing in the brain depend on daily language experience and which do not. In this study, we recorded high-density cortical activity from adult participants with diverse language backgrounds as they listened to speech in their n

Chronic Visual Abnormality in an Elderly Patient With Temporal Lobe Epilepsy.

A 79-year-old woman visited our department for chronic visual field abnormalities with a floating sensation for two months. Neurological and ophthalmologic examinations yielded normal results, except for brain MRI indicating left hippocampal atrophy. Cognitive function tests were normal. EEG revealed frequent spikes and slow waves in the left frontotemporal region, corroborated by reduced accumulation in 123I-iomazenil single photon emission computed tomography. A diagnosis of temporal lobe epilepsy was established, and treatment with lacosamide resulted in a remarkable improvement in symptoms

An exploration of anomia rehabilitation in drug-resistant temporal lobe epilepsy.

Around 40% of patients who undergo a left temporal lobe epilepsy (LTLE) surgery suffer from anomia (word-finding difficulties), a condition that negatively impacts quality of life. Despite these observations, language rehabilitation is still understudied in LTLE. We assessed the effect of a four-week rehabilitation on four drug-resistant LTLE patients after their surgery. The anomia rehabilitation was based on cognitive descriptions of word finding deficits in LTLE. Its primary ingredients were psycholinguistic tasks and a psychoeducation approach to help patients cope with daily communication

Reorganization of pragmatic language networks in patients with temporal lobe epilepsy.

OBJECTIVE: To investigate the neural networks involved in idiomatic expressions (IE) comprehension in healthy controls and patients with drug-resistant temporal lobe epilepsy (TLE), with a functional magnetic resonance imaging (fMRI) task. METHODS: Thirty-two patients with TLE (left or right) and seventeen healthy controls were evaluated. Activated nodes in the fMRI task were defined as Regions of Interest (ROIs) for a posterior functional connectivity analysis. RESULTS: All participants completed the task successfully. We found a bilateral fronto-temporal network, lateralized to the right,

Addressing the Medicolegal Aspects of Temporal Lobe Epilepsy: A Literature Review.

Temporal lobe epilepsy (TLE) represents a prevalent form of focal epilepsy that often requires surgical intervention and can be resistant to antiseizure medications. Its epidemiology varies across regions due to diagnostic challenges and underestimation of individual neurological traits. Despite these complexities, TLE accounts for a significant proportion of total epilepsies worldwide. Early discussions on TLE's forensic implications date back to the mid-20th century, highlighting its unique behavioral and mental disturbances during seizures. Depersonalization and derealization as sequelae of a temporal lobe lesion: a case report.

BACKGROUND: Depersonalization and derealization can occur not just from psychiatric causes but also from various organic etiologies, such as seizures and intracerebral structural abnormalities. However, there have been no previous reported cases to the authors' knowledge detailing isolated depersonalization and derealization in the absence of clinical seizure activity or other psychiatric pathology, as sequelae of structural intracerebral lesions. CASE PRESENTATION: In this case report, we detail the unique presentation of a 68-year-old woman under the care of palliative medicine who experien

Evolution of interictal activity in models of mesial temporal lobe epilepsy.

Interictal activity and seizures are the hallmarks of focal epileptic disorders (which include mesial temporal lobe epilepsy, MTLE) in humans and in animal models. Interictal activity, which is recorded with cortical and intracerebral EEG recordings, comprises spikes, sharp waves and high-frequency oscillations, and has been used in clinical practice to identify the epileptic zone. However, its relation with seizures remains debated. Moreover, it is unclear whether specific EEG changes in interictal activity occur during the time preceding the appearance of spontaneous seizures. This period, w

Intraparenchymal Schwannoma of Temporal Lobe: A Case Report and Review of the Literature.

Intracranial schwannomas (ISs) account for approximately 8% of intracranial tumors, while IS, a rare entity, is responsible for roughly 1% of IS. A 33-year-old man with a 3-month headache and sudden onset seizure was referred to our clinic. Preoperative magnetic resonance imaging revealed a contrast-enhancing mass accompanied by cystic components in the right temporal lobe. Ganglioglioma, metastasis, or glioblastoma multiforme was suspected, and surgery was advised. During surgery, gross total resection of a noninvasive tumor was conducted. Postoperative recovery was uneventful. Based on histo

Single-neuron mechanisms of neural adaptation in the human temporal lobe.

A central function of the human brain is to adapt to new situations based on past experience. Adaptation is reflected behaviorally by shorter reaction times to repeating or similar stimuli, and neurophysiologically by reduced neural activity in bulk-tissue measurements with fMRI or EEG. Several potential single-neuron mechanisms have been hypothesized to cause this reduction of activity at the macroscopic level. We here explore these mechanisms using an adaptation paradigm with visual stimuli bearing abstract semantic similarity. We recorded intracranial EEG (iEEG) simultaneously with spiking

Temporal lobe hemorrhage as a complication of HSV encephalitis: a case report.

KEY CLINICAL MESSAGE: This case underlined the importance of having high suspicion for lobar hemorrhage as a rare but deadly complication of herpes simplex virus encephalitis and shone light upon the added complexity it poses on management on an already deadly disease. ABSTRACT: Herpes simplex virus (HSV) encephalitis is the most common type of sporadic encephalitis that inflicts high rates of morbidity and mortality. Differentiating a progressing encephalitis syndrome from a lobar hemorrhage as a complication presents a challenge and requires great vigilance and insight on part of the treati

Extent of piriform cortex resection in children with temporal lobe epilepsy.

OBJECTIVE: A greater extent of resection of the temporal portion of the piriform cortex (PC) has been shown to be associated with higher likelihood of seizure freedom in adults undergoing anterior temporal lobe resection (ATLR) for drug-resistant temporal lobe epilepsy (TLE). There have been no such studies in children, therefore this study aimed to investigate this association in a pediatric cohort. METHODS: A retrospective, neuroimaging cohort study of children with TLE who underwent ATLR between 2012 and 2021 was undertaken. The PC, hippocampal and amygdala volumes were measured on the pre

Localization of a Medial Temporal Lobe-Precuneus Network for Time Orientation.

OBJECTIVE: Time orientation is a fundamental cognitive process in which one's personal sense of time is matched with a universal reference. Time orientation is commonly assessed through mental status examination, yet its neural correlates remain unclear. Large lesions have been associated with deficits in time orientation, but the regional anatomy implicated in time disorientation is not well established. The current study investigates the anatomy of time disorientation and its network correlates in patients with focal brain lesions. METHODS: Time orientation was assessed 3 months or more aft
