Paediatric palliative care and paediatric euthanasia: clarifying a moral continuum.

This Viewpoint explores the complex relationship between two approaches to end-of-life care for children-paediatric palliative care and paediatric euthanasia. Both approaches share the goal of alleviating suffering in children with life-limiting conditions. They differ about which interventions are permissible or forbidden in pursuit of that goal. Palliative care permits the withdrawal of life-sustaining treatments and even the administration of medications that might hasten death, as long as the goal of administering the medications is to relieve suffering and not to hasten death. With euthan

Challenging Case: Navigating End-of-life in Neuro-inclusive Cancer Care.

Roy is a 13-year-old boy with autism spectrum disorder who has a recurrent glioblastoma. He initially underwent a surgical resection followed by 5 months of intensive systematic chemotherapy that required multiple hospitalizations. Although a clinical trial was considered, Roy was excluded from enrollment because of concerns around his ability to consent and adhere to a complex protocol. Imaging soon revealed significant disease progression and his care plan shifted toward palliative therapies.Roy was the only child of his aging parents. Roy's mother was navigating her own chronic medical cond

Bridging urology and palliative care: a narrative review of current practice and evolving priorities.

PURPOSE OF REVIEW: This narrative review summarizes recent evidence (2023-2025) on the role of palliative care in urology. It focuses on utilization trends, access disparities, symptom burden, procedural palliative needs, and end-of-life (EOL) care in advanced genitourinary malignancies. RECENT FINDINGS: PC utilization in genitourinary cancers is increasing. Inpatient palliative care use rose from 4.9 to 31.5% in renal cell carcinoma and from 5.9 to 19.0% in testicular cancer. Yet, integration into routine care remains inconsistent. Disparities persist, including lower palliative care utiliza

Developing service models for adult palliative and end of life care for people with a learning disability (The DAPPLE Project): protocol for a mixed-method study.

INTRODUCTION: People with a learning disability face significant health and mortality inequalities as well as wider systemic inequities. Challenges in palliative and end of life care (PEOLC) include communication difficulties, lack of involvement in decision-making and multimorbidity. Early identification of PEOLC needs is challenging, impacting timely care planning. The study aims to (1) understand barriers and enablers to providing high-quality, accessible PEOLC for people with a learning disability, and identify effective service delivery models and interventions and (2) improve PEOLC quali

Mapping palliative care for people living with advanced cancer in phase 1 clinical trials: A scoping review.

BACKGROUND: Phase 1 cancer clinical trials offer potential disease modifying treatments for patients with advanced cancer who have not responded to standard treatments. These patients also experience palliative care needs that should be addressed. However, misunderstandings may arise between the goals of experimental treatments and the goals of palliative care, especially when the latter focuses on quality of life. AIM: This study aimed to map the existing literature on the provision and integration of palliative care within phase 1 clinical trials, identifying knowledge gaps and informing fu

Reflections on Christmas in palliative practice.

This article explores the significance of providing compassionate end-of-life care around Christmas. Patients and their families may find this time particularly hard and community nurses have an important role to play in minimising their strain and stress. Sensitive and engaging communication about death and dying can make a difference.

Instability in End-of-Life Goals and Preferences of Patients Who Are Seriously Ill: A Systematic Review.

IMPORTANCE: Advance care planning (ACP) aims to ensure goal-concordant end-of-life (EOL) care based on patients' prerecorded goals and preferences, assuming these remain stable. However, this assumption has not been systematically examined. OBJECTIVE: To assess evidence on stability of EOL care goals and preferences over time among patients who are seriously ill, and identify factors associated with changes over time. EVIDENCE REVIEW: PubMed, EMBASE, and Scopus were searched for English-language longitudinal studies published between 2014 and September 2024 assessing outcomes at least twice

'Saying goodbye'. . . A systematic integrative review of palliative caregiving in intergenerational living contexts.

BACKGROUND: Intergenerational co-residence has historically been prevalent. Recent financial pressures, increasing caregiving responsibilities and ageing populations have led to a resurgence of this arrangement, particularly in end-of-life care. However, understanding of its influence on care quality across cultures remains limited. AIM: To explore how intergenerational co-residence affects emotional, practical, and cultural dimensions of palliative and end-of-life care across different settings, and to identify key themes shaping caregiving roles, decision-making, and support needs in these

[Ethical and clinical recommendations concerning palliative sedation].

The utility of wild-type outer membrane vesicle (wtOMV) vaccines against serogroup B (MenB) meningococcal disease has been explored since the 1970s. Public health interventions in Cuba, Norway and New Zealand have demonstrated that these protein-based vaccines can prevent MenB disease. Data from large clinical studies and retrospective statistical analyses in New Zealand give effectiveness estimates of at least 70%. A consistent pattern of moderately reactogenic and safe vaccines has been seen with the use of approximately 60 million doses of three different wtOMV vaccine formulations. The key

Impact of palliative care at end-of-life Covid-19 patients - a small-scale pioneering experience.

BACKGROUND: In March 2020, the outbreak caused by the SARS-CoV-2 virus was declared a pandemic, resulting in numerous fatalities worldwide. To effectively combat the virus, it would be beneficial to involve professionals who specialize in symptom control for advanced illnesses, working closely with other specialties throughout the illness process. This approach can help manage a range of symptoms, from mild to severe and potentially life-threatening. No studies have been conducted in Portugal to analyse the intervention of Palliative Medicine at the end of life of Covid-19 patients and how it

Influence of social interactions, professional supports and fear of death on adults' preferences for life-sustaining treatments and palliative care.

AIM: To explore the significance of culture, professional support in the community, social interactions and intrapersonal determinants of adults' preferences for life-sustaining treatments and palliative care. METHODS: A cross-sectional design with a Social Ecological Model was used. Between 1 October 2012 and 31 December 2012, 474 adults aged ≥20 years living in a city of Southern Taiwan completed the survey. Data were analysed using hierarchical multiple regression. RESULTS: The life-sustaining measures model was significant with 15.3% (p < 0.0001) of the variance in the Modified Emmanuel

Appropriateness of End-of-Life Care in People Dying With Dementia: Applying Quality Indicators on Linked Administrative Databases.

OBJECTIVES: Dementia is a progressive incurable life-limiting illness. Previous research suggests end-of-life care for people with dementia should have a symptomatic focus with an effort to avoid burdensome interventions that would not improve quality of life. This study aims to assess the appropriateness of end-of-life care in people who died with dementia in Belgium and to establish relative performance standards by measuring validated population-level quality indicators. DESIGN: We conducted a retrospective observational study. SETTING AND PARTICIPANTS: We included all persons deceased wi

Can Orthodox Jewish Patients Undergo Palliative Extubation? A Challenging Ethics Case Study.

According to Jewish law/ethics, continuous life-sustaining therapy may not be withdrawn after its introduction, unless the patient has improved and no longer has a medical indication for the treatment. We report the case of an 88-year-old Orthodox Jewish patient, on invasive mechanical ventilation, with severe anoxic brain injury after multiple cardiac arrests. Although the patient's son informed the palliative care team that his father did not want to be in pain or to linger in a vegetative state when terminally ill, the mechanical ventilation was keeping him alive with a poor neurological pr

Combining regional expertise to form a bereavement support alliance.

Providing compassionate bereavement care for families experiencing perinatal loss is a standard of care in most healthcare organizations. In this article, we describe the development of The Alliance of Perinatal Bereavement Support Facilitators, begun over 25 years ago in Chicago by staff who identified the need to reach out to colleagues at other area institutions for advice and support in this work. This collaboration created a regional support network that has resulted in a long-lasting, active, sustainable organization of excellence focused on enhancing practice, education, and perinatal b

Pressure ulceration and palliative care: prevention, treatment, policy and outcomes.

Pressure ulcer development and management have particular significance in palliative and end-of-life care owing to the prevalence of mobility issues and the skin changes that can occur with ageing, chronic illness, and at the end of life. Hence, it is important for nurses working in palliative care to be vigilant for pressure ulcer development and to possess knowledge relating to the prevention and management of pressure ulceration. This article considers current UK policy regarding pressure ulcers, highlighting contradictions of relevance to patients requiring palliative care. It then provide

Rational use of sublingual opioids in palliative medicine.

The sublingual administration of opioid analgesics has been a mainstay in the pain management of homebound dying hospice patients who are no longer able to swallow. It is also a potentially useful route of administration in other situations in which the oral route is not available and other routes are impractical or inappropriate. Potential advantages of the sublingual route include rapid analgesic onset and avoidance of hepatic first-pass metabolism. Pharmacokinetic and pharmacodynamic studies have yielded widely disparate data on sublingual morphine. Other opioids have been less studied. Ava

"The death rattle" in the intensive care unit after withdrawal of mechanical ventilation in neurological patients.

The noise produced by oscillatory movements of secretions in the oropharynx, hypopharynx, and trachea during inspiration and expiration in unconscious terminal patients is often described as "the death rattle." The secretions are produced by the salivary glands and bronchial mucosa. These patients are usually too weak to expectorate or swallow the migrating secretions. Sputum usually only accumulates in these areas if there is a significant impairment of the cough reflex, as in deep coma or near death. Reported incidence of death rattle in terminal patients varied between 6 and 92%. Death ratt
