Spontaneous resolution of a thoracic spinal epidural arteriovenous fistula caused by stabbing injury.

STUDY DESIGN: A single case is presented of a spontaneously resolving epidural arteriovenous fistula caused by a stab wound. OBJECTIVE: To demonstrate a unique case of a spontaneously resolving epidural arteriovenous fistula caused by a stab wound, as well as to present a brief review of pathology. SUMMARY OF BACKGROUND DATA: Spinal epidural arteriovenous fistulas (SEAVF) are high-flow vascular malformations characterized by an arteriovenous shunt involving the internal vertebral venous plexus (IVVP). SEAVFs can present with intramedullary, subarachnoid or epidural hemorrhages, spinal cord o

Simultaneously diagnosed pulmonary thromboembolism and hemopericardium in a man with thoracic spinal cord injury.

BACKGROUND: Simultaneous pulmonary thromboembolism (PTE) and hemopericardium is a rare but life-threatening condition. As hemopericardium is a contraindication to anticoagulation treatment, it is challenging to handle both conditions together. OBJECTIVE: The objective of the study was to report a rare case of a man with thoracic spinal cord injury presenting with simultaneous PTE and hemopericardium. DESIGN: Case report. SUBJECT: A 43-year-old man with incomplete T9 paraplegia (American Spinal Injury Association Impairment Scale D) complained of fever one and a half months after spinal cord

Open repair of blunt thoracic aortic injury remains relevant in the endovascular era.

BACKGROUND: Thoracic endovascular aneurysm repair (TEVAR) has been a major advance in the treatment of blunt thoracic aortic injury (BTAI), although many patients still undergo open repair. This study was undertaken to evaluate outcomes with open repair and TEVAR for BTAI. STUDY DESIGN: A retrospective review of all patients with BTAI at a single Level I trauma center from 2001 through 2009 was performed. Patients were grouped according to treatment modality, ie, open repair, TEVAR, or medical management. Direct comparison using standard statistical methods was made between patients undergoin

Natural history of minimal aortic injury following blunt thoracic aortic trauma.

BACKGROUND: Endovascular repair of blunt traumatic thoracic aortic injuries (BTAI) is common at most trauma centres, with excellent results. However, little is known regarding which injuries do not require intervention. We reviewed the natural history of untreated patients with minimal aortic injury (MAI) at our centre. METHODS: We conducted a retrospective database review to identify all patients with a BTAI between October 2008 and March 2010. The cohort comprised patients initially untreated because of the lesser degree of injury of an MAI. We reviewed initial and follow-up computed tomogr

Outcome after thoracic aortic injury: experience in a level-1 trauma center.

Traumatic rupture of the thoracic aorta is a potentially fatal injury that leads to death in 75-90% of cases at the time of injury. In this report, we present our experience with traumatic thoracic aortic injury and compare the outcome in patients with respect to their hemodynamic stability at presentation and the timing of surgical repair. We performed a retrospective data analysis of the medical records of 30 patients who had sustained a traumatic rupture of the thoracic aorta during the period from January 1, 2000 to October 30, 2005. The demographic data, mechanism of injury, modality of d

[Case study of puncturing thoracic injury with right ventricle perforation and cardiac tamponade].

We present a case of open thoracic traumatism with heart wound and pericardial tamponade. The echocardiographic findings were inconclusive and contrast-enhanced computed tomography (CT) with fine reconstructions was performed. We discuss the role of CT with emphasis on the radiological signs related to myocardial perforation, hematic pericardial effusion, and cardiac tamponade, as well as ruling out other associated lesions. CT was useful in the evaluation of hemodynamically stable patients with open cardiac wounds and inconclusive echocardiographic findings.

[Nonpenetrating traumatic injury of the thoracic aorta and left subclavian artery; report of a case].

Nonpenetrating traumatic injury of the thoracic aorta and/or its major branch is usually fatal and the treatment of this condition carries extremely high risk because of associated visceral organ injuries. Accurate diagnosis have been difficult. However, recently developed multi-slice helical computed tomography (CT) is highly sensitive in early detection of precise location of injury and associating injuries of other organs. Here we report our case with combined thoracic aortic and left subclavian artery injuries, diagnosed by 3-dimensional (3-D) CT and treated successfully.

Delayed repair for blunt thoracic aortic injury: is it really equivalent to early repair?

BACKGROUND: Blunt thoracic aortic injury (BTAI) is a severe injury that traditionally has mandated immediate surgical repair. Delaying operative intervention for BTAI can allow other life-threatening injuries to be managed first, but potentially increases the risk of aortic rupture and death. The objective of this study was to evaluate the outcome of delayed repair (DR) compared with early repair (ER) for BTAI and to assess the effectiveness of a protocol for medical control of systolic blood pressure and heart rate in those patients whose repairs were delayed. METHODS: This study is a retros

The effects of primary thoracic blast injury and morphine on the response to haemorrhage in the anaesthetised rat.

Primary thoracic blast injury causes a triad of bradycardia, hypotension and apnoea mediated in part via a vagal reflex. Blast casualties may also suffer blood loss, and the response to progressive simple haemorrhage is biphasic: an initial tachycardia followed by a vagally mediated reflex bradycardia which can be attenuated by micro opioid agonists. The aims of this study were to determine the effects of thoracic blast injury on the response to subsequent haemorrhage, and the effects of morphine, administered after blast, on the response to blood loss. Male Wistar rats, terminally anaesthetis

Reflex nature of the cardiorespiratory response to primary thoracic blast injury in the anaesthetised rat.

Blast injuries represent a problem for civilian and military populations. Primary thoracic blast injury causes a triad of bradycardia, hypotension and apnoea. The objective of this study was to investigate the reflex nature of this response and its modulation by vagotomy or administration of atropine. The study was conducted on terminally anaesthetised (alphadolone/alphaxalone, 18-24 mg x kg x h(-1), I.V.) male Wistar rats randomly allocated to the groups indicated below. Blast injuries were produced with compressed air while sham blast involved the sound of a blast only. Primary blast injury

[The clinical picture and pathogenesis of the late sequelae of birth injury to the thoracic spinal cord].

A study was made of the spinal cord of 28 neonates and of children aged up to 6 years with the anamnesis load, who died of different diseases. According to the morphological findings, massive hemorrhages were discovered in the thoracic part of the spinal cord in those who died at an age under 6 months. In the later periods one could find different stages of the organization of hemorrhages in the form of commissural process primarily around roots and vessels and dyscirculatory vascular disorders in the spinal cord. The lesions discovered were compared with the clinical manifestations in 40 pati

Blunt traumatic avulsion of an intercostal artery: an unusual case of thoracic aortic injury.

The aortic isthmus is the most commonly injured part of the thoracic aorta in patients who survive blunt deceleration injury to that vessel long enough to reach a hospital. Occasionally, avulsion of the brachiocephalic arteries from the aortic arch is seen. The authors describe an unusual form of intrathoracic vascular injury in which mediastinal hemorrhage occurred secondary to avulsion of an intercostal artery from the descending thoracic aorta as a result of a vertical deceleration injury.

Traumatic Aortic Disruption Index is Associated with Mortality and Urgency of Stent Grafting in Blunt Thoracic Aortic (Grade 3) Injuries.

BACKGROUND: Delayed stent grafting for blunt thoracic aortic injuries (BTAIs) is current standard of care. However, given the heterogeneity of pseudoaneurysm presentations, it is currently unclear which severe BTAIs require more urgent intervention. We hypothesize that a Traumatic Aortic Disruption Index (TADI) calculation based on sagittal computed tomography angiography imaging measurements would correlate with urgency of stent grafting. METHODS: All patients at a level-1 trauma center with BTAIs over a 12-year period were identified. A TADI score was then calculated using the length of pse

Thoracic responses and injuries to male postmortem human subjects (PMHS) in rear-facing seat configurations in high-speed frontal impacts.

Objective: One potential nonstandard seating configuration for vehicles with automated driving systems (ADS) is a reclined seat that is rear-facing when in a frontal collision. There are limited biomechanical response and injury data for this seating configuration during high-speed collisions. The main objective of this study was to investigate thoracic biomechanical responses and injuries to male postmortem human subjects (PMHS) in a rear-facing scenario with varying boundary conditions.Method: Fourteen rear-facing male PMHS tests (10 previously published and 4 newly tested) were conducted at

Management of blunt traumatic thoracic aorta injuries with endovascular stent-grafts in a tertiary hospital in an urban Asian city.

BACKGROUND: Thoracic aortic injury from blunt trauma is a life-threatening condition with significant mortality and morbidity with open surgical repair. Endovascular means of treatment is emerging as an attractive and less invasive option. We report our experience with endovascular stent-graft repair for blunt traumatic thoracic aorta injury. METHODS: Between January 2003 and August 2009, six patients underwent endovascular repair of blunt traumatic thoracic aorta injury. Data from the medical records of these patients were analyzed. RESULTS: All patients had deceleration injury resulting in
