The authors present the newest data of a minimal thoracic technique: the pleurotomy. In our times this technique requires the modem chest tubes, the complete pleurotomy sets for different emergent situations. The authors presents the indications, the possible complications, their own technique of pleurotomy and the actual management of pleural drainage.

We describe a case of giant pulmonary aspergilloma in a 79-year old man. He had undergone an operation for pulmonary tuberculosis of the right lung at the age of 49 years. His chest ragiograph showed a large fungus ball in the right upper lung field. Taking his age and pulmonary condition into consideration, we performed a cavernostomy and fungus ball resection to prevent life-threatening hemoptysis. The postoperative course was satisfactory and without complication. Cavernostomy may be an alternative choice in high-risk aspergilloma patients.

Dirofilariasis was diagnosed in 2 cats with spontaneous pneumothorax. One cat had a 3-week history of a cough, and the other had an 11-month history of vomiting and tachypnea. Pneumothorax was managed in cats by thoracocentesis and supportive care. Diagnosis of dirofilariasis was made on the basis of heartworm antigen and antibody test results and radiographic findings. Clinical signs of heartworm infection improved after treatment with corticosteroids. Cats had good extended outcomes. Postoperative subcutaneous emphysema (SE) can sometimes lead to clinical complications, mainly due to abnormal air leakage (AL) into the chest wall through the surgically breached parietal pleura. In this report, we describe a patient who, following a right upper lobectomy and upper mediastinal dissection, developed progressive mediastinal and SE on postoperative day 6, which persisted despite thoracostomy. A 69-year-old man had experienced right upper lobectomy for inflammatory granuloma. Three months after surgery, he was diagnosed with pleural empyema due to bronchopleural fistula and open window thoracostomy was performed. BACKGROUND: There is an increasing need for objective and validated educational concepts. This holds especially true for surgical procedures like chest tube insertion (CTI). Thus, we developed an instrument for objectification of learning successes: the assessment scale based on Objective Structured Assessment of Technical Skill (OSATS) for chest tube insertion, which is evaluated in this study. After prenatal diagnosis of bilateral fetal hydrothorax, ascites, and polyhydramnios, bilateral thoracoamniotic shunts were placed at 29 weeks gestation using an ultrasound-guided, minimally invasive technique. Thoracic disease is common in cattle and is a significant cause for economic losses in the stocker and feedlot industries. In most cases, economic constraints limit diagnostic investigation and affect treatment options. Although medical management is, by far, the most appropriate therapeutic intervention in such cases, surgical management of some respiratory diseases can allow for profitable return to productivity. Surgical procedures of the thorax most often involve thoracotomy or pericardiotomy. Tracheal reconstruction and invasion of the mediastinum are rarely indicated in cattle.
