Prognostic factors of thymectomy in patients with myasthenia gravis: a cohort of 132 patients.

OBJECTIVE: To identify the response to thymectomy and the factors associated with a poor response, a nested case-control study was performed on 132 patients with an established diagnosis of myasthenia gravis who had had a thymectomy between 1987 and 1997 and had been followed up for at least 3 years. METHODS: In order to assess the response to thymectomy, the following two points were taken into account: (a) the dose of pyridostigmine and other drugs (steroids, azathioprine) that the patient took before and after thymectomy, and (b) the Osserman classification before and after thymectomy. The…

Technique of transcervical--subxiphoid-vats "maximal" thymectomy in treatment of myasthenia gravis.

Operative technique of transcervical-subxiphoid-VATS thymectomy is described. Total sternotomy is avoided which makes the procedure less invasive without compromising its radicality. The operation is performed with aid of special device with two sternal retractors elevating the sternum from above and below and facilitating access to the mediastinum. The mediastinal pleura is incised bilaterally and exenteration of the mediastinal fat is done under control of video camera inserted through the thoracoscopic ports. Two team approach is proposed: one team, called "the cervical" team performs disse…

Prognostic factors for myasthenia gravis treated by thymectomy: review of 61 cases.

BACKGROUND: Medical treatment for myasthenia gravis (MG) involves the use of anticholinesterase agents, immunosuppressive drugs, plasmapheresis, and gamma-globulin. However, these agents result in a complete clinical remission rate as low as 15%. As a consequence, thymectomy, preferably by transsternal approach, has become increasingly accepted as an efficacious procedure for MG, with reported complete clinical remission rates as high as 80%. METHODS: We have the clinical records of 61 patients diagnosed with MG at La Paz University Hospital, Madrid, Spain, from January 1977 to December 1994.…

Induction of autoimmune hepatitis and autoantibodies to liver antigens by neonatal thymectomy in mice.

We examined development of autoimmune hepatitis in neonatally thymectomized C3H/HeN mice and tried to characterize the nature of liver antig … indicate that hepatitis and autoantibodies to liver proteins are induced spontaneously by neonatal thymectomy in mice, and the candidates of autoantigen in this hepatitis model are 52-kD protein in LSP and 150-kD liver proteins different from LSP. Still more, we regard the 150-kD molecule as a new autoantigen related to hepatitis.

Long-term outcome and quality of life after thymectomy for myasthenia gravis.

OBJECTIVE: The authors identify criteria suitable to predict long-term clinical improvement and evaluate quality of life after thymectomy for myasthenia. DESIGN: Retrospective analysis with long-term follow-up (mean 92 months) was conducted for 86 patients and questionnaire interviews were performed for 65 patients who underwent thymectomy between 1976 and 1993. MAIN OUTCOME MEASURES: The authors used the Osserman Score and the European Organization for Research and Treatment of Cancer quality-of-life questionnaire. RESULTS: After thymectomy, lasting benefits were achieved predominantly by …

The effect of adult thymectomy on immune response to infection with Listeria monocytogenes in mice.

The role of adult thymus in in vivo immune response to infection with Listeria monocytogenes was examined using euthymic mice and adult-thymectomized (ATx) mice which had been thymectomized 2 weeks before. Numbers of T cells in peritoneal cavities and spleens were increased at 2 weeks after inoculation of L. monocytogenes, whereas such increases of T cells were several times higher in euthymic mice than in ATx mice. In flow-cytometric analysis of peritoneal exudate cells, a significant increase of CD3+CD4-CD8- cells bearing gamma/delta type T cell receptor was noted in euthymic mouse after inf

[The morphological characteristics of the parotid and submandibular salivary glands following neonatal thymectomy].

The time course of structural and functional changes in the parotid and submaxillary salivary glands after neonatal thymectomy was considered. A combination of various methods including histological, histochemical, cytophotometric ones and transmission electron microscopy, was employed. They showed that neonatal thymectomy resulted in an increase in secretory activity of the salivary glands and was followed by hemolymphodynamic disorders and lymphoplasmocytic infiltration.

[Clinical examinations of thymic abnormalities and significance of thymectomy in the patients with autoimmune disease].

Results and significance of thymectomy in 427 patients with autoimmune disease, which include myasthenia gravis, ulcerative colitis. Behçet's syndrome and others, were studied. Preoperative pneumomediastinography was useful to delineate the outline of the nontumorous thymus and small "latent thymoma". The thymus was almost always successfully removed by our procedure "thymectomy via the suprasternal notch" (Method I), but the procedure combined with dissection through a parasternal incision (Method II) was frequently performed. Method II has been routinely adopted since 1976. Thymectomy was pe…

[Surgical treatment of myasthenia by thymectomy. A report on 248 cases (author's transl)].

Good results were obtained in 80 p. cent of 248 patients seen after more than 15 months following the operation. Results are grouped as a function of the different characteristic features of the patients and three groups can be defined: failures, improvement, remissions. These results contradict those who assert that thymectomy is of no value for severe cases. Cervicotomy is performed for non-tumoral forms or for small tumors; sternotomy is reserved for large median thymomas; anterolateral thoracotomy for laterally located thymomas and tracheotomized patients. Postoperative tracheotomy is very…

Effect of neonatal thymectomy on experimental subacute sclerosing panencephalitis in adult hamsters.

Syrian golden hamsters were thymectomized in the first 48 h of life, raised, and then inoculated intracerebrally with the HBS strain of hamster-adapted subacute sclerosing panencephalitis virus when over 12 weeks old. Two groups soon become apparent; one group showed progressive neurological susmptoms and the other group remained healthy. The sick hamsters became moribund between days 8 and 17 postinoculation, and they had large amounts of complete virus in their brains, no antibodies to measles virus in their sera, and a demonstrated lack of thymus tissue. Pathological changes in the central

Neonatal thymectomy increases the incidence of spontaneous and methylcholanthrene-enhanced thyroiditis in rats.

At 16 weeks of age, 13 percent of untreated Buffalo strain rats showed evidence of autoimmune thyroiditis. Feeding methylcholanthrene increased the incidence to 42 percent. Neonatal thymectomy significantly raised the incidence of disease so that almost all (87 percent) untreated and all methylcholanthrene-treated animals developed severe disease. It is proposed that the thymus exerts a regulatory effect on autosensitization.

Surgical outcomes of thoracoscopic extended thymectomy via subxyphoid and left intercostal approach.

BACKGROUND: Minimally invasive thymectomy is widely adopted for the management of thymic diseases and myasthenia gravis (MG). Among thoracoscopic techniques, the intercostal approach (ICA) is the most commonly used, whereas the subxiphoid approach (SA) has been proposed to reduce postoperative pain. However, comparative evidence remains heterogeneous. METHODS: This single-center retrospective cohort study included consecutive patients who underwent minimally invasive thymectomy between January 2019 and December 2023. Patients were assigned to the ICA or SA group according to the surgical appr…

Improved Neuromuscular Transmission Following Thymectomy in Juvenile Myasthenia Gravis: A Case Series.

INTRODUCTION/AIMS: Juvenile myasthenia gravis (JMG) patients report symptomatic improvement following thymectomy; however, there is a lack of published literature on biomarkers for interval improvement. This study aimed to investigate the utility of single-fiber electromyography to quantify changes post-thymectomy. METHODS: This was a retrospective medical record review of JMG patients who underwent thymectomy at Children's Healthcare of Atlanta between 2014 and 2024. Pre- and post-thymectomy orbicularis oculi jitter values and Myasthenia Gravis Activities of Daily Living (MG-ADL) scores were…

Long-Term Immune Alterations After Thymectomy in Early Childhood: A Case Series.

The thymus plays a central role in T-lymphocyte maturation and in maintaining immune defense against infections and autoimmunity. We describe three boys (mean age 7.3 years) who underwent total thymectomy during surgical correction of congenital heart disease, two in infancy and one in the neonatal period. Patient A presented (14 years old) with recurrent cervical lymphadenopathy, generalized Epstein-Barr virus (EBV) infection, and hepatic fibrosis. Patient B (two-year-old) developed splenomegaly and had a history of frequent lower respiratory tract infections and pseudohypoaldosteronism type …

Post-Thymectomy Autoimmune Flare-Up With New-Onset Type 1 Diabetes Mellitus.

The thymus gland aids in the maturation of the immune system. An overactive or malfunctioning thymus gland, as seen in thymomas, can lead to disrupted immune systems. Thymectomy, the usual treatment, can paradoxically lead to further derangements in the immune system, leading to new autoimmune disorders. Most of these reported disorders are rheumatological. Except preclinical studies, there are no reported cases of autoimmune diabetes post-thymectomy. A 25-year-old woman who had malignant thymoma underwent chemotherapy, followed by thymectomy and radiotherapy. She developed autoimmune diabetes…

Early Postoperative Results after Thymectomy for Thymic Cancer: A Single-Institution Experience.

BACKGROUND: Surgery for thymic cancers is considered the key of curative treatment. Preoperative patients' characteristics and intraoperative features might influence postoperative outcome. We aim to verify short-term outcomes and possible risk factors for complications after thymectomy. METHODS: We retrospectively investigated patients undergoing surgery for thymoma or thymic carcinoma in the period between January 1, 2008, and December 31, 2021, in our department. Preoperative features, surgical technique (open, bilateral VATS, RATS), intraoperative characteristics and incidence of postoper…

Thymectomy in Juvenile Myasthenia Gravis Is Safe Regarding Long Term Immunological Effects.

Thymectomy is an established treatment in adult MG and also recommended for the treatment of post-pubertal onset juvenile MG. Whether the youngest children should be thymectomized is still debated. Signs of premature aging of the immune system have been shown in studies on early perioperative thymectomy in children with congenital heart defect. In this retrospective cohort study the objective was to investigate the long-term effects of treatment related thymectomy on T cell subsets and T cell receptor rearrangement excision circles (TRECs) in peripheral blood of juvenile myasthenia gravis (MG)…

Severe Aplastic Anemia Developed after Thymectomy: A Case Report and Literature Review.

Thymus neoplasms are frequently related to paraneoplastic autoimmune manifestations. Its most common associations are myasthenia gravis and pure red cell aplasia. Aplastic anemia has been increasingly documented as an initial presentation of thymoma. Nevertheless, its development after successful surgical resection of thymoma is a rare condition. We report a case of a 53-year-old man with severe aplastic anemia preceded by amegakaryocytic thrombocytopenia three years after thymectomy with no signs of disease recurrence. He underwent immunosuppressive therapy with cyclosporine 5 mg/kg/day and p…
