Congenital Diaphragmatic Hernia: State of the Art in Translating Experimental Research to the Bedside.

Congenital diaphragmatic hernia (CDH) is a devastating disease that still carries a high mortality and morbidity rate. Poor outcomes for fetuses and infants with CDH are mainly related to pulmonary hypoplasia (PH) and pulmonary vascular remodeling that leads to pulmonary hypertension (PHTN). Over the last five decades, research efforts have focused on modeling CDH not only to study the pathophysiology of the diaphragmatic defect, pulmonary hypoplasia, and pulmonary hypertension, but also to identify therapies that would promote lung growth and maturation, and correct vascular remodeling. As CD

The Great Harmony in Translational Medicine: Biomaterials and Stem Cells.

Thanks to novel approaches and emerging technologies, tissue engineering and regenerative medicine have made a great effort to regenerate damaged tissue or organ with no donor needed. The approaches involve two fundamental components: bioengineered scaffolds and stem cells. Bioengineered scaffolds which can also be enriched with bioactive molecules such as cytokines, growth factors, and so on have been fabricated using a wide range of synthetically or naturally derived biodegradable and biocompatible polymers. These scaffolds should support cell attachment, migration, proliferation, and/or dif

Role and translational implication of galectins in arthritis pathophysiology and treatment: A systematic literature review.

Galectins are members of the animal lectin family that bind to the β-galactoside-containing carbohydrate moieties of glycoconjugates. They seem to have an important role in the pathophysiology of several diseases, including arthritis. Osteoarthritis (OA) and rheumatoid arthritis (RA) are chronic conditions with few or no available therapies. In this context, galectins could provide a novel opportunity, but the precise role and mechanism of their involvement in arthritis are still not fully understood. This descriptive systematic literature review summarizes in vitro, in vivo, and clinical stud

Discovery and progress of direct cardiac reprogramming.

Cardiac disease remains a major cause of death worldwide. Direct cardiac reprogramming has emerged as a promising approach for cardiac regenerative therapy. After the discovery of MyoD, a master regulator for skeletal muscle, other single cardiac reprogramming factors (master regulators) have been sought. Discovery of cardiac reprogramming factors was inspired by the finding that multiple, but not single, transcription factors were needed to generate induced pluripotent stem cells (iPSCs) from fibroblasts. We first reported a combination of cardiac-specific transcription factors, Gata4, Mef2c,

New therapeutic targets in alcoholic hepatitis.

Alcoholic liver disease (ALD) is a leading cause of liver-related morbidity and mortality worldwide. ALD encompasses a spectrum of disorders including asymptomatic steatosis, steatohepatitis, fibrosis, cirrhosis and its related complications, and the acute-on-chronic state of alcoholic hepatitis. While multidisciplinary efforts continue to be aimed at curbing progression of this spectrum of disorders, there is an urgent need to focus our efforts on effective therapeutic interventions for alcoholic hepatitis (AH), the most severe form of ALD. AH is characterized by an abrupt development of jaun

Animal models of ischemia-reperfusion-induced intestinal injury: progress and promise for translational research.

Research in the field of ischemia-reperfusion injury continues to be plagued by the inability to translate research findings to clinically useful therapies. This may in part relate to the complexity of disease processes that result in intestinal ischemia but may also result from inappropriate research model selection. Research animal models have been integral to the study of ischemia-reperfusion-induced intestinal injury. However, the clinical conditions that compromise intestinal blood flow in clinical patients ranges widely from primary intestinal disease to processes secondary to distant or

Preclinical models: needed in translation? A Pro/Con debate.

The discovery of the causative mutations and many of the predisposing risk factors for neurodegenerative disorders such as Amyotrophic Lateral Sclerosis, Alzheimer's, Parkinson's, and Huntington's disease (HD), has led to the development of a large number of genetic animal models of disease. In the case of HD, for example, over 20 different transgenic rodent models have been generated. These models have been of immense value in providing novel insights into mechanisms of disease, with the promise of accelerating the development of therapies that can delay the onset or slow the progression of t

Translational research on ocular surface reconstruction using oral mucosal epithelial cell sheets.

Ocular surface reconstruction using autologous oral mucosal epithelial cell sheets has drastically changed the treatment of limbal stem-cell deficiency. The morphological and functional characteristics of oral mucosal epithelial cell sheets are similar to those of normal corneal epithelium. Ocular surface reconstruction can prevent potential problems associated with limbal transplantation, including immune rejection and donor tissue shortages. Thus far, ocular reconstruction using epithelial cell sheets has been limited to clinical research. Although the effectiveness and safety of this surgic

Batten disease: clinical aspects, molecular mechanisms, translational science, and future directions.

The neuronal ceroid lipofuscinoses, collectively the most common neurodegenerative disorders of childhood, are primarily caused by an autosomal recessive genetic mutation leading to a lysosomal storage disease. Clinically, these diseases manifest at varying ages of onset, and associated symptoms include cognitive decline, movement disorders, seizures, and retinopathy. The underlying cell biology and biochemistry that cause the clinical phenotypes of neuronal ceroid lipofuscinoses are still being elaborated. The 2012 Neurobiology of Disease in Children Symposium, held in conjunction with the 41

Nausea and the quest for the perfect anti-emetic.

The discovery of anti-emetic agents is reviewed to illustrate the large database (>129,000 papers in PubMed) available for potential data mining and to provide a background to the shift in interest to nausea from vomiting. Research on nausea extends to identification of biomarkers for diagnosis/clinical trials and to understanding why nausea is such a common dose-limiting toxicity of diverse therapeutic agents. The lessons learned for translation from animals to humans, from the discovery of the anti-vomiting effects of 5-HT3 and NK1 receptor antagonists, is discussed in terms of the similarit

Translation of genetic findings to clinical practice in juvenile myoclonic epilepsy.

It has been estimated that JME (juvenile myoclonic epilepsy), when compared to other adult epilepsy syndromes, is most likely to have a genetic cause. However, decades of research have not brought us closer to finding a single 'JME gene' that is important on a population basis. Is this due in part to the genetic complexity of the syndrome, the cryptic nature of the genes of effect, or perhaps because JME is not one condition at all but many? Before we can begin to harness the power of next-generation sequencing techniques, we must first reduce JME down to lacunae of homogeneity--using increasi

A perinatal signature of light on chronobiology? If so, numerous questions arise and experimental animal research must provide more information.

That light and melatonin rhythms provide both clock and calendar information in humans and numerous other species is beyond dispute; this holds true for all stages of life, including the very early ones. Experimental evidence elucidates that exposure to light and melatonin titres are keys for the very development of circadian and seasonal rhythms. As evinced by a 2011 publication in Nature Neuroscience such awareness could impact considerably on the design and conduct of experimental studies as well as their subsequent analyses, interpretations and comparisons. Therefore "when and how experime

Translational implications of novel findings in cancer cachexia: the use of metabolomics and the potential of cardiac malfunction.

PURPOSE OF REVIEW: Cancer cachexia is a complex metabolic syndrome that manifests as a progressive wasting disorder. Recent findings have substantially improved our understanding of this unique entity shedding light on a potentially significant and overlooked morbidity in the form of cardiac functional changes that may occur in tandem with cachexia. Moreover, recent technological advances, such as with the application of metabolomic analysis, have yielded a better understanding of the metabolic changes that accompany this condition. RECENT FINDINGS: Recent evidence from murine studies indicat

Recent advances in the genetics of cerebellar ataxias.

The hereditary cerebellar ataxias are a clinically and genetically heterogeneous group of disorders that primarily affect the cerebellum; often there are additional features such as neuropathy, cognitive decline, or maculopathy that help define the clinical subtype of ataxia. They are commonly classified according to their mode of inheritance into autosomal dominant, autosomal recessive, X-linked, and mitochondrial forms. Great advances have been made in understanding the genetics of cerebellar ataxias in the last 15 years. At least 36 different forms of ADCA are known, 20 autosomal-recessive,

Vaccine innovation, translational research and the management of knowledge accumulation.

What does it take to translate research into socially beneficial technologies like vaccines? Current policy that focuses on expanding research or strengthening incentives overlooks how the supply and demand of innovation is mediated by problem-solving processes that generate knowledge which is often fragmented and only locally valid. This paper details some of the conditions that allow fragmented, local knowledge to accumulate through a series of structured steps from the artificial simplicity of the laboratory to the complexity of real world application. Poliomyelitis is used as an illustrati

Translational musculoskeletal science: is sarcopenia the next clinical target after osteoporosis?

Translational medicine must increasingly turn its attention to the aging population and the musculoskeletal deterioration that it entails. The latter involves the integrated function of both muscle and bone. Musculoskeletal science has an established interest in such problems in relationship to osteoporosis of bone. The introductory concepts in this paper consider the extent to which loss of muscle mass and function, or sarcopenia, will be the next major translational target. Its epidemiology shows parallels with that of osteoporosis, and the two tissues have a close functional relationship. I

A translational view of the genetics of lipodystrophy and ectopic fat deposition.

A wide range of lipodystrophy syndromes exist, each with varying clinical presentations, and yet cumulatively they underscore the importance of adipocyte biology in human metabolism. Loss of the ability to retain excess lipids in "classical" adipose tissue stores can lead to the overdevelopment of ectopic fat stores, often creating severe perturbations of both glucose and lipid homeostasis. Linkage analysis and candidate sequencing efforts have successfully identified responsible mutations for multiple forms of lipodystrophy. Recently, the reduction in the cost of DNA sequencing has resulted i

Monoacetyldiacylglycerols as emerging immunomodulatory lipids in chronic inflammation: mechanisms, biology, and translational potential.

Chronic inflammatory diseases remain a major global health burden, with approximately 30-40% of patients responding inadequately to current cytokine-targeted biologics, Janus kinase (JAK) inhibitors, or small-molecule immunomodulators. Beyond suboptimal efficacy, these therapies carry well-documented adverse effects, including infection and malignancy risks associated with JAK inhibitors, and act on single molecular targets, leaving upstream, membrane-proximal drivers of chronic inflammation largely unaddressed. Monoacetyldiacylglycerols (MacDGs), a structurally distinct class of partially ace

Preclinical models of Wilson's disease: From pathogenesis to clinical translation.

Wilson's disease (WD) is an autosomal recessive disorder of copper homeostasis caused by pathogenic ATP7B variants, leading predominantly to hepatic and neurological injury. Extensive heterogeneity in genotype, age at onset, clinical presentation, and treatment response complicates both diagnosis and therapeutic development, highlighting the need for reliable experimental models that reproduce distinct features of WD pathogenesis. This review critically evaluates animal models, conventional cell systems, induced pluripotent stem cell models, and organoids of WD, with an emphasis on the phenoty
