Coronary CT findings of coronary to bronchial arterial communication in chronic pulmonary disease.

To describe the coronary CT findings of coronary-to-bronchial artery communication (CBAC) in chronic pulmonary disease. Coronary CT was performed in 15 patients with chronic pulmonary disease using 64-channel or greater multidetector CT. Among those patients, one or two CBACs were identified. A retrospective analysis of the CT findings was done to determine the originating artery, arterial course of the communications and other associated results. The main underlying pulmonary disease was bronchiectasis (n = 12). The origin of the CBAC was from the left atrial (n = 7) or sinoatrial (SA) nodal

Supracerebellar infratentorial approach to cavernous malformations of the brainstem: surgical variants and clinical experience with 45 patients.

OBJECTIVE: The supracerebellar infratentorial (SCIT) approach can be performed at the midline (median variant), lateral to the midline (paramedian variant), or at the level of the angle formed by the transverse and sigmoid sinuses (extreme lateral variant). We analyzed our experience with SCIT approaches for the surgical treatment of cavernous malformations of the brainstem (CMBs). METHODS: Demographic, clinical, radiologic, and surgical data from 45 patients (20 males and 25 females; mean age, 36.2 years) with CMBs surgically removed through SCIT approaches were reviewed retrospectively. Endovascular mechanical thrombectomy for the treatment of extensive postpartum cerebral venous sinus thrombosis: a case report.

BACKGROUND: Cerebral venous sinus thrombosis (CVST) is a rare but potentially life-threatening complication of pregnancy and the postpartum period. Although systemic anticoagulation remains the standard first-line treatment, evidence supporting endovascular mechanical thrombectomy in postpartum patients is limited. CASE PRESENTATION: A 22-year-old woman presented 12 days after an uncomplicated vaginal delivery with progressive headache and vomiting. Neuroimaging confirmed extensive CVST involving the superior sagittal, cortical, left transverse, and sigmoid sinuses, extending to the left jugu

Idiopathic hypertrophic pachymeningitis masquerading as CNS neoplasm: case report and literature review.

IHP is a rare inflammatory disorder characterized by dural thickening. Its nonspecific presentation often leads to diagnostic challenges and potential misdiagnosis as a neoplasm. Literature review and illustrative case example. PubMed search using terms related to IHP yielded 272 candidate citations, 50 of which met study criteria and were included. A 40-year-old woman presented with headache, dizziness, and blurred vision. Surgical intervention via right craniotomy was recommended due to diagnostic uncertainty, symptomatic mass effect, and the potential for a malignant diagnosis. A near-total

A Rare Case of First-Time Seizure Induced by Cerebral Venous Sinus Thrombosis Following the Use of Tranexamic Acid for Menorrhagia.

Background and clinical significance: Tranexamic acid (TXA) is commonly used for menorrhagia. Common side effects include diarrhoea, nausea, and vomiting. However, more serious and rare side effects, including embolism, thrombosis, and seizures, are less commonly considered. Case presentation: We report the case of a 39-year-old woman of Asian origin who presented after a first-time seizure while driving, following starting tranexamic acid for menorrhagia seven days prior. She complained of a headache, nausea, neck stiffness, floaters, and blurred vision. Her lactate was elevated on presentati

Persistent Oscillating Vertigo From Extracranial Venous Compression.

BACKGROUND: Persistent oscillating vertigo (POV) can be triggered by motion, when it is called mal de débarquement syndrome (MdDS) but can also occur from nonmotion triggers such as neck injury, inflammation, or homeostatic derangements (nonmotion POV [nmPOV]). The pathology underlying MdDS and nmPOV is unknown but shared symptoms include rocking/bobbing/swaying vertigo, headache, cognitive slowing, and fatigue. METHODS: We present a case series of patients with MdDS and nmPOV whose symptoms were relieved after treatment of extracranial venous compression in the neck and thoracic outlet. RES

New Horizons for Diagnostic Pitfalls of Cerebral Venous Thrombosis: Clinical Utility of a Newly Developed Cerebral Venous Thrombosis Diagnostic Score: A Case Report and Literature Review.

BACKGROUND Diagnosing cerebral venous thrombosis (CVT) poses significant challenges owing to a nonspecific clinical presentation, poorly correlated laboratory biomarkers, and low sensitivity of non-contrast head computed tomography (CT). We describe a case of missed CVT diagnosis, due to low clinical suspicion and nonrecognition of anemia as a prothrombotic factor, especially during an ulcerative colitis (UC) flare. A recently proposed CVT clinical probability score can guide clinicians in pursuing further neurovascular imaging. CASE REPORT A 35-year-old man, with treatment-naive UC, presented

Use of a Minimally Invasive Retractor System for Retrieval of Intracranial Fragments in Wartime Trauma.

OBJECTIVE: Wartime penetrating brain injury can result in deep-seated parenchymal and intraventicular shrapnel, bullets, and bone. Large fragments pose a risk of secondary injury from migration, infection, and metal toxicity. It has been recommended that aggressive removal of fragments be avoided. The goal of this study is to report our technique of minimally invasive removal of select deep-seated fragments using a tubular retractor system. METHODS: A retrospective review of our database of service members presenting with penetrating traumatic brain injuries incurred during Operations Iraqi F

Box lesion in the open left atrium for surgical ablation of atrial fibrillation.

OBJECTIVE: Cut-and-sew maze with a box lesion around the pulmonary veins is currently the criterion standard procedure for surgical ablation of atrial fibrillation. Recently, we changed our technique from standard bilateral epicardial pulmonary vein isolation with interconnecting lesions to a box lesion procedure with a bipolar radiofrequency ablation device. Our study describes this technique. METHODS: Between March 2009 and June 2012, we performed 90 ablations by the box technique with a bipolar radiofrequency device. Fifty-five patients (61%) had persistent atrial fibrillation, and 21 (23%
