The geriatric trauma hospitalist service: An analysis of a management strategy for injured older adults.

BACKGROUND: Management of geriatric trauma patients requires balancing chronic comorbidities with acute injuries. We developed a care model in which patients are managed by hospitalists with trauma-centered education and hypothesized that clinical outcomes would be similar to outcomes in patients primarily managed by trauma surgeons. METHODS: This was a retrospective study of trauma patients aged ≥65 from January 2020 to December 2021. Groups were defined by admitting service: trauma surgery service (TSS) or geriatric trauma hospitalist service (GTHS). The primary outcome was in-hospital mort

A ten year analysis of the traumatic maxillofacial and brain injury patient in Amsterdam: complications and treatment.

Maxillofacial trauma is often associated with injuries to the cranium, especially in high-energy trauma. The management of such cases can be challenging and requires close cooperation between oral and maxillofacial surgery and neurosurgical teams. There are few reports in the current literature describing the complications that develop in patients with maxillofacial trauma and traumatic brain injury (TBI). Complications can be categorized as early or late and/or minor and major. The exact definition of complications and their categorization remains a matter of current debate. We present a 10 y

Dural Sinus (Cerebral Venous) Thrombosis in a Pediatric Trauma Patient: A Rare Complication After Closed Head Injury.

OBJECTIVE: Closed head injury rarely predisposes patients, particularly children, to the development of dural sinus thrombosis. In addition, most cases of sinus thrombosis are subacute in nature. The following is a case report of a precipitous course of dural sinus thrombosis after closed head injury in a pediatric trauma patient. DESCRIPTION: A 14-year-old African American girl presented to the emergency department as a trauma activation. She was an unrestrained rear-seat passenger involved in a motor vehicle collision during which she was ejected. She experienced loss of consciousness and w

Financial impact of emergency hand trauma on the health care system.

BACKGROUND: The US has a crisis of insufficient emergency coverage for hand trauma. One of the problems is the perceived financial loss associated with caring for this population. We evaluated the financial impact of treating emergency hand trauma patients on an academic medical practice and health care system. STUDY DESIGN: We examined billing records for 2,632 hand patients seen in the emergency department in 2005 at the University of Michigan. Financial data were separated into inpatient professional and facility revenues and costs. Professional net revenue was calculated by applying actua

Pediatric Trauma Readiness: A Trauma-Specific Assessment to Complement the National Pediatric Readiness Project.

OBJECTIVES: The National Pediatric Readiness Project of the Emergency Medical Services for Children surveyed emergency departments in the United States in 2013 for readiness to provide emergency care to children. However, that survey did not query for many elements considered essential to Advanced Trauma Life Support (ATLS). METHODS: Our pediatric trauma center and state department of health collaborated to develop a survey reflecting ATLS principles regarding pediatric-specific trauma stabilization, clinical/administrative resources, and interfacility transfer to complement the 2017 PedsRead

Strategic Principles and Historical Lessons Learned: The Angels of Bataan and Corregidor Case Study as an Example for Contemporary Military Nurses.

Whether a large urban trauma center facing varied healthcare crises, an understaffed hospital during a pandemic, or a remote and austere setting in a combat zone during active war, the lines between United States (US) military nurses serving in civilian or military settings continue to blur. Health care systems across America face uncharted challenges, including imminent closure related to financial factors, threats of future pandemics with equal or worse devastation than COVID-19, and outbreaks of war across the globe that require the specialty skills of medical professionals working in the U

Reducing costs and improving patient recovery through a nurse-driven centralized spinal orthoses program on a post-surgical unit: A quality improvement initiative.

BACKGROUND: An external vendor providing off-the-shelf spinal orthoses to inpatients created significant costs and barriers to quality care for spinal surgery patients. A nursing leadership team initiated a quality improvement project to reduce the cost of providing off-the-shelf spinal orthoses and improve the care provided to spinal patients. OBJECTIVE: To develop and evaluate a nursing-led process for providing off-the-shelf orthoses to spinal surgery patients and eliminate high costs. DESIGN: Quality improvement project evaluated as a retrospective interrupted time-series. SETTING: Post

Trauma Exposure and Posttraumatic Stress Disorder Symptoms Predict Onset of Cardiovascular Events in Women.

BACKGROUND: Psychological stress is a proposed risk factor for cardiovascular disease (CVD), and posttraumatic stress disorder (PTSD), the sentinel stress-related mental disorder, occurs twice as frequently in women as men. However, whether PTSD contributes to CVD risk in women is not established. METHODS AND RESULTS: We examined trauma exposure and PTSD symptoms in relation to incident CVD over a 20-year period in 49 978 women in the Nurses' Health Study II. Proportional hazards models estimated hazard ratios and 95% confidence intervals for CVD events confirmed by additional information or

[Therapy of para-articular hip fractures in the elderly].

UNLABELLED: Fractures of the hip in old patients are life-threatening events. A steady increase of that fracture type is likely in the near future. Surgical therapies and strategies have to consider the special requirements and problems of geriatric patients to achieve better results. They have to be designed together with new concepts of geriatric rehabilitation programmes. MATERIAL AND METHODS: In 1992 148 patients with hip fractures were treated at the department of traumatology, University Ulm. Follow-up parameters were daily activities in life pre- and postoperatively as well as mortalit

Adrenal gland trauma is associated with high injury severity and mortality.

PURPOSE: To review a statewide experience of adrenal gland trauma (AGT), incidence, demographics, associated injuries, Injury Severity Score (ISS), Glasgow Coma Scale (GCS), mechanisms of injury, and complications, associated with AGT. METHODS: A retrospective analysis of patients admitted to accredited trauma centers in the Commonwealth of Pennsylvania who sustained AGT from January 1, 1989 to December 31, 2000. RESULTS: Adrenal trauma was found in 322 of 210,508 cases (0.15%). There were 76.4% men and 23.6% women. Seventy-one percent of patients had an ISS greater than 20. The overall mort

Trauma Prehospital Hypothermia Prevention and Treatment: An Observational Study.

BACKGROUND: Hypothermia in trauma patients causes increased morbidity and mortality. Swift recognition and treatment are important to prevent any further heat loss. In addition, patient discomfort from cold decreases satisfaction with care. The administration of active and passive rewarming measures is important in the prevention and treatment of hypothermia, but their use in prehospital trauma patients in Portugal has not been previously reported. OBJECTIVE: To assess the prevalence of hypothermia, the impact of rewarming measures, and the management of the discomfort caused by cold. METHOD

Assessing Burnout and Secondary Traumatic Stress Among Sexual Assault Nurse Examiners in the Upstate of South Carolina.

BACKGROUND: Sexual assault nurse examiners (SANEs) provide trauma-informed care to victims of sexual assault. Repeated exposure to trauma places SANEs at risk for burnout and secondary traumatic stress (STS), which may negatively impact SANE availability. AIM: To examine burnout and STS among SANEs and their potential impact on workforce sustainability. METHODS: SANEs in the Upstate of South Carolina completed a cross-sectional survey including a demographic and occupational questionnaire, the Maslach Burnout Inventory-Human Services Survey for Medical Personnel, and the Secondary Traumatic

Clinical report of delayed recovery after general anaesthesia in elderly patients with cervical spine surgery: A case report.

This report describes a case of a 70-year-old male that underwent decompression laminectomy and internal fixation under general anaesthesia. After extubation, the patient gradually developed no response to instructions and the disturbance of consciousness persisted with unequal pupils in size, but clinical neurological findings and a brain computed tomography scan showed no organic abnormalities. A careful medical history undertaken by anaesthesiologists revealed that the patient had a history of trauma to his left eye, resulting in blindness in this eye, but the surgeons, anaesthesiologists a

Social admissions in older trauma patients, not just a one night stay.

PURPOSE: Older trauma patients (65 years or older), who suffer minor or moderate injury for which immediate hospitalization is not strictly indicated, are often admitted to the hospital due to a self-sufficiency problem. Although hospitalization is expensive and associated with risks, little is known about the course of such a social admission. Therefore, the aim of this study was to clarify the course and outcome of social admissions. METHODS: A single centre retrospective cohort study was performed in a level II trauma centre. All hospitalized trauma patients aged 65 or older between 2015 a

Feasibility and inter-rater reliability of the ICU Mobility Scale.

OBJECTIVES: The objectives of this study were to develop a scale for measuring the highest level of mobility in adult ICU patients and to assess its feasibility and inter-rater reliability. BACKGROUND: Growing evidence supports the feasibility, safety and efficacy of early mobilization in the intensive care unit (ICU). However, there are no adequately validated tools to quickly, easily, and reliably describe the mobility milestones of adult patients in ICU. Identifying or developing such a tool is a priority for evaluating mobility and rehabilitation activities for research and clinical care

[New parenting education in maternal child nursing].

Taiwan society is today typified by low birth rates amongst Taiwanese and a rising number of children born to immigrant and trans-cultural marriage families. Unhealthy behavior and anxiety on the part of pregnant women increase postpartum depression and complications and negatively affect neonatal heath. Such may further negatively impact upon romantic feelings between the new parents and the nascent parent-child relationship. New parenting education is a proactive and innovative strategy that may be used to improve maternal and child health. Therefore, it is worthy to explore how best to achi
