Study of the outcome of patients transferred to a level I hospital after stabilization at an outlying hospital in a rural setting.

OBJECTIVE: To determine the characteristics and outcome of transferred trauma patients in a rural setting. METHODS: We conducted a case-control study of all trauma admissions to a rural Level I trauma center to examine a 3.5-year (1993-1996) comparison of trauma patients admitted directly with those transferred (RTTP) after being initially stabilized at an outlying hospital. We used prehospital times, Injury Severity Score (ISS), LD50ISS (the ISS at which 50% of patients died), Revised Trauma Score, probability of survival, Acute Physiology and Chronic Health Evaluation II, and observed survi

Disclosure of sexual victimization: the effects of Pennebaker's emotional disclosure paradigm on physical and psychological distress.

Research suggests that many sexual assault survivors do not disclose their experience, which may increase associated distress. Pennebaker's emotional disclosure paradigm has been shown to ameliorate psychological and physical distress in individuals exposed to stressful events. The current study assessed the effectiveness of this paradigm with sexual assault survivors (N = 74). College women with a history of sexual assault wrote about their most severe victimization or about how they spend their time (control). Then 73 women (98.6%) completed a 1-month follow-up assessment. Results indicated

Population-based analysis of severe injuries from nonmotorized wheeled vehicles.

BACKGROUND: Bicycling, skateboarding, and inline skating are popular recreational activities, with the potential of causing severe injury. METHODS: A retrospective, population-based, trauma registry analysis of severe injuries and deaths from nonmotorized wheeled vehicles (NMWV) over a 10-year period was performed. RESULTS: During the study period, 1475 cyclists, 141 skateboarders, and 112 inline skaters sustained injury meeting registry criteria (length of stay > or = 3 days or death). The majority were male. Lone crashes were the most common mechanism of injury, although collisions with mo

Evaluation of Severity Score in Patients with Lower Limb and Pelvic Fractures Injured in Motor Vehicle Front-Impact Collisions.

BACKGROUND Lower limb and pelvic injuries and fractures occur at a very high incidence in motor vehicle accidents. In this study, the characteristics (e.g., body side, bone location, and fracture severity) of lower limb and pelvic fractures that occurred during front-impact collisions were correlated with the injured patients' sex, age, and position in the vehicle. MATERIAL AND METHODS We retrospectively evaluated 191 patients (136 males, 55 females) who were injured in motor vehicle accidents, specifically in frontal collisions. RESULTS This study revealed that most of lower limb and pelvic f

Evaluating the global psychotrauma screen total score using item response theory in an Italian community sample.

Background: The Global Psychotrauma Screen (GPS) is a brief transdiagnostic screener for trauma-related symptoms. Although the Italian version has previously been examined using factor-analytic methods, evidence on item-level functioning and on the interpretability of the symptom total remains limited. Objective: To determine whether the 17 GPS symptom items support the use of a single total score as an efficient indicator of global trauma-related burden.Method: Data from an age- and gender-balanced analytic sample of 6930 Italian adults recruited online during the COVID-19 outbreak were analys

The prevalence of chronic deep venous thrombosis in trauma: Implications for hospitals and patients.

INTRODUCTION: Deep venous thrombosis (DVT) is considered a preventable complication in trauma patients. Hospitals risk financial penalties for DVT rates above accepted benchmarks. These penalties do not apply to chronic DVT, which develops before admission. Lower-extremity duplex ultrasound (LEDUS) can detect characteristics of thrombus chronicity, allowing differentiation of chronic from acute DVT. The objective of this study was to determine the prevalence of chronic DVT in hospitalized trauma patients. METHODS: We performed a retrospective review of trauma patients admitted to our Level I

Progressive Mobility Protocol Reduces Venous Thromboembolism Rate in Trauma Intensive Care Patients: A Quality Improvement Project.

The intensive care unit (ICU) trauma population is at high risk for complications associated with immobility. The purpose of this project was to compare ICU trauma patient outcomes before and after implementation of a structured progressive mobility (PM) protocol. Outcomes included hospital and ICU stays, ventilator days, falls, respiratory failure, pneumonia, or venous thromboembolism (VTE). In the preintervention cohort, physical therapy (PT) consults were placed 53% of the time. This rose to more than 90% during the postintervention period. PT consults seen within 24 hr rose from a baseline

The role of primary plastic surgery in the management of open fractures.

A study was undertaken to determine the requirement for primary plastic surgery in the treatment of open fractures. We reviewed 3297 consecutive open fractures in a 22-year period in a defined population. Analysis showed that 12.6% of patients required primary plastic surgery with 5.6% being treated with split skin grafting and 7.2% with a flap. Only 3.5% of open upper limb fractures required primary plastic surgery compared to 27.9% of open lower limb fractures. The fractures that required most primary plastic surgery were those of the femoral diaphysis and all fractures between the proximal

Pediatric sports injuries: an age comparison of children versus adolescents.

BACKGROUND: Significant knowledge deficits exist regarding sports injuries in the young child. Children continue to engage in physically demanding, organized sports to a greater extent despite the lack of physical readiness, predisposing themselves to injury. PURPOSE: To evaluate sports injuries sustained in very young children (5-12 years) versus their older counterparts (13-17 years) with regard to the type and location of injuries, severity, and diagnosis. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: A retrospective chart review was performed on a 5% random probabi

Joint analysis of crash injury severities for autonomous and conventional vehicles in mixed traffic environments: Application of random parameter bivariate probit model.

Autonomous vehicles (AVs) are expected to significantly enhance road safety in the future. However, until fully autonomous driving systems are widely adopted, mixed traffic with AVs and conventional vehicles (CVs) will remain a typical feature of roadways. Consequently, it is crucial to understand how roadway and built environment factors impact traffic safety in mixed traffic settings. This study proposes a joint model to analyze crash injury severity for both autonomous and conventional vehicles within a unified framework. A random parameter bivariate probit model (RBP) is used as the method
