Surgical fasciectomy of the trapezius muscle combined with neurolysis of the Spinal accessory nerve; results and long-term follow-up in 30 consecutive cases of refractory chronic whiplash syndrome.

BACKGROUND: Chronic problems from whiplash trauma generally include headache, pain and neck stiffness that may prove refractory to conservative treatment modalities. As has previously been reported, such afflicted patients may experience significant temporary relief with injections of local anesthetic to painful trigger points in muscles of the shoulder and neck, or lasting symptomatic improvement through surgical excision of myofascial trigger points. In a subset of patients who present with chronic whiplash syndrome, the clinical findings suggest an affliction of the spinal accessory nerve (…

[Anaysis on acupoint selection rule of acupuncture for trigeminal neuralgia].

The characteristics and rules of acupoint selection of acupuncture for trigeminal neuralgia were analyzed. By searching CNKI, VIP, WF, literature regarding acupuncture for trigeminal neuralgia from 1980 to 2013 was collected to establish an acupuncture prescription database. The data mining technology was applied to analyze the characteristics and rules of the acupoint selection. As a result, a total of 180 papers were included, involving 148 acupoints. It was found that the acupoints that had high frequency of selection included Hegu (LI 4), Xiaguan (ST 7), Fengchi (GB 20) and trigger points.…

[Treatment of patients with chronic nonspecific lumbalgia with various pain triggers].

OBJECTIVE: To evaluate the effectiveness of complex therapy in patients with chronic nonspecific lumbalgia associated with various pain triggers. MATERIAL AND METHODS: There were 121 patients with chronic nonspecific lumbalgia (average duration of pain 8.0±5.0 months) aged 22 to 59 years (average age 42.1±10.5). The lesion of the facet joints (24.8%), sacroiliac joint (23.2%), muscles (16.5%) or their combined lesion (35.5%) was established as pain triggers of lumbalgia. The patients underwent complex therapy, including medications, kinesiotherapy and cognitive therapy. Before and after the c

Parvovirus Infection Triggering Still's Disease.

UNLABELLED: We present the case of a 59-year-old man with acute B19 parvovirus infection who developed a systemic inflammatory reaction similar to adult-onset Still's disease (AOSD). We discuss the clinical challenge due to overlapping symptoms to distinguish between a primary B19 viral infection and the autoimmune disease it can trigger. LEARNING POINTS: Distinguishing between primary B19 parvovirus infection and autoimmune diseases can be difficult in view of the significant symptom overlap. In our patient, recurrence of symptoms during follow-up and response to treatment were in favour of a

[Mechanisms underlying chronic fatigue, a symptom too often overlooked II- From deregulated immunity to neuroinflammation and its consequences].

Hypothalamus stimulation by inflammatory and / or stress signals can trigger activation of the HPA (hypothalamic-pituitary-adrenal) axis, which includes the hypothalamus, pituitary and adrenal gland. Acute activation of the HPA axis is fundamental for the fight or flight response. It allows a maximal energy mobilization available for an effort, whilst erasing fatigue. On the contrary, the chronic activation of this axis decreases muscle efficiency and leads to chronic fatigue. In this second part of our review will be discussed several strategic points that need to be considered for attempting

Trigeminal neuralgia during sleep.

Pain paroxysms in trigeminal neuralgia (TN) are sudden and extremely intense. Nonetheless, many clinicians who treat TN report that patients are rarely if ever awakened at night by pain attacks. If true, this observation is important as it implies the presence of a powerful sleep protective mechanism. We queried TN patients and their habitual sleep partners about painful awakenings and discovered that such awakenings are in fact quite common. As during the day, pain paroxysms during sleep are typically induced by natural stimuli at TN trigger points. Brief attacks sometimes occur without frank…

The relationship of neck injury and post-traumatic headache.

Although there may be several causes of post-traumatic headache, neck injury is perhaps the most common. This paper primarily reviews the relationship of neck injury, whiplash, and post-traumatic headache. Mechanisms may include structural damage from acceleration or extension of the neck, development of myofascial pain and trigger points, interaction of the trigeminal nociceptive system with the upper cervical (occipital) nerves, and psychologic and emotional factors. Although some healing will occur, the outcome may depend on a number of human factors (awareness of an impending collision) an…

Atlantoaxial (C1-C2) facet joint osteoarthritis: a distinctive clinical syndrome.

Osteoarthritis of the atlantoaxial facet joints was identified radiologically in 27 patients, and these comprised 4% of all outpatients with osteoarthritis or degenerative disease of the spine seen during a 36-month period. These patients had a clinical syndrome which differed from those seen in patients who have subaxial degenerative disease of the spine or myofascial cervical pain. Occipital pain, occipital trigger points, crepitus in the occipital region, and a rotational head tilt deformity (in 13 patients, usually associated with collapse of 1 of the lateral masses [facets]), were the maj…

A comparison of the clinical manifestation and pathophysiology of myofascial pain syndrome and fibromyalgia: implications for differential diagnosis and management.

Two prominent forms of chronic musculoskeletal pain disorders are fibromyalgia (FM) and myofascial pain syndrome (MPS). Inconsistent diagnosis of chronic musculoskeletal pain is an important clinical issue, as MPS is often mistaken for FM. Distinction between the two diagnoses depends largely on identification of either tender points or myofascial trigger points in FM and MPS, respectively. However, there currently is no standard diagnostic protocol for MPS. Consequently, this results in a lack of consistency across health care practitioners diagnosing both FM and MPS. Therefore, developing se…

[Fibromyalgia].

The classification of fibromyalgia is based on the criteria of the American College of Rheumatology. For diagnostic reasons autonomic disturbances and mental features have to be considered. The distinction between fibromyalgia (tender points) and myofascial pain syndrome (trigger points) is essential. Internal and neurological disorders as a primary cause of fibromyalgia have to be excluded. The aetiology and pathogenesis of fibromyalgia still remain uncertain. The myopathological patterns in fibromyalgia are non-specific: type-II-fiber-atrophy, a slight increase in lipid droplets, a prolifera…

The Impact of Critical Life Points and Events for Survivors of Institutional Child Sexual Abuse: Understanding Trauma and the Role of Compensation in Recovery.

Although the impact of child sexual abuse (CSA) is well documented in the international literature, little is known about the critical points of recovery across the life course for survivors of institutional CSA. The aim of this study was to identify critical points, or events across the life course that may increase, or decrease, a survivor's vulnerability to the complex traumatization of institutional CSA (ICSA). The sample consisted of two hundred and forty-eight witness statements extracted from the 56 publicly available case studies presented to the Australian Royal Commission into Instit

Myofascial pain and treatment: Editorial.

The first article featured in this quarter's overview deserves special attention. Margalef and colleagues developed the first viable animal model of trigger points (TrPs). They also provided evidence of glycosaminoglycans (GAGs) near TrPs, which is a new finding that deserves further scientific inquiry (Margalef et al 2019). In 2011, Stecco et al. already mentioned a possible role of hyaluronan, which constitutes a subgroup of GAGs, in the etiology of myofascial pain (Stecco et al 2011). Mayoral Del Moral and colleagues published an excellent study that showed very good inter-examiner reliabil…

Early-stage myocardial changes are not aggravated by Western diet or ageing in Mybpc3 c.2373insG heterozygous mice.

Hypertrophic cardiomyopathy (HCM) caused by heterozygous pathogenic gene variants is characterized by highly variable disease phenotypes. Ageing and systemic metabolic disruption are thought to act as additional 'second hits' that modulate this genotype-phenotype relationship. To define mechanisms through which metabolic and ageing-related stressors may trigger development of HCM, we investigated whether Western diet (WD) feeding or ageing (3, 8.5 and 18 months) could trigger and/or aggravate cardiac hypertrophy and dysfunction in targeted knock-in mice harbouring the Dutch heterozygous (HET)

A randomised study comparing the effectiveness of acupuncture or morphine versus the combination for the relief of dyspnoea in patients with advanced non-small cell lung cancer and mesothelioma.

BACKGROUND: Dyspnoea is one of the commonest symptoms of lung cancer. Opioids can reduce dyspnoea. This study investigates acupuncture for relief of breathlessness in lung cancer. METHODS: We performed a single-centre, randomised phase II study of 173 patients with non-small cell lung cancer or mesothelioma with dyspnoea score of ≥4 on visual analogue scale (VAS). Randomisation was to acupuncture alone (A), morphine alone (M) or both (AM). Acupuncture was administered at upper sternal, thoracic paravertebral, trapezius trigger points and LI4. Manubrial semi-permanent acupuncture studs were in…

EMG analysis of the thenar muscles as a model for EMG-triggered larynx stimulation.

Paralysis of one or both sides of the larynx musculature compromises breathing and speech function. Currently there is no surgical remedy to restore adequate function of the larynx. A plausible alternative solution is triggered electrical stimulation of the paralysed larynx site using a laryngeal pacemaker. Triggering of the pacemaker succeeds via constant EMG measurement of the muscle activity of the healthy larynx side. The EMG data analysis described in this work is one possible approach for regulating pacemaker triggering. In this study we used EMG data from the thenar muscles as a model t

Evaluation of the effects of occlusal splint and masseter muscle injection in patients with myofascial pain: a randomised controlled trial.

Myofascial pain is one of the common symptoms in patients with temporomandibular joint disorders (TMD). Occlusal splint (OS) and masticatory muscle trigger point (TP) local injections are primary treatment options. We aimed to investigate the effects of these treatments using clinical and elastography measures. Patients who were diagnosed with myofascial pain according to Diagnostic Criteria for Temporomandibular Disorders (DC/TMD) were included. There were 16 patients in each group. Group 1 was treated with occlusal splint, Group 2 was treated with occlusal splint and masseter muscle lidocain

Kinesio taping versus dry needling in the treatment of myofascial pain of the upper trapezius muscle: A randomized, single blind (evaluator), prospective study.

BACKGROUND: Although a number of therapeutic interventions for trigger-point inactivation have been studied, it remains controversial which intervention is better. OBJECTIVE: To compare the effectiveness of the kinesio taping and dry needling methods in patients with trigger-point related myofascial pain syndrome of the upper trapezius muscle. METHODS: A total of 42 patients were randomly allocated to the kinesio taping group (n= 23) and dry needling group (n= 19). The patients were assessed for pain, pressure pain threshold (PPT), cervical range of motion (CROM), and function at baseline, a

Effects of cassava wax immersion on pain, pressure pain threshold, and upper extremity function among patients with trigger finger.

BACKGROUND: Trigger finger causes pain and impaired hand function. Ultrasound and paraffin wax are commonly used but have cost and accessibility limitations; cassava wax may be a low-cost alternative, though its effectiveness remains unclear. OBJECTIVE: To evaluate the immediate, short-term, and post-intervention effects of cassava wax immersion combined with ultrasound therapy on pain, pressure pain threshold, and upper extremity function in patients with trigger finger. METHODS: In this single-blind, randomised controlled trial, 38 participants were randomly assigned to two groups ( n = 19

Alcoholic drinks as triggers in primary headaches.

OBJECTIVE: This project aims to investigate the role of alcoholic drinks (ADs) as triggers for primary headaches. METHODS: Patients followed in the Headache Centre and presenting with migraine without aura, migraine with aura (MA), chronic migraine (CM), and tension-type headache (TH) were asked if their headache was precipitated by AD and also about their alcohol habits. Individual characteristics and drink habits were evaluated within two binary logistic models. RESULTS: About one half (49.7%) of patients were abstainers, 17.6% were habitual consumers, and 32.5% were occasional consumers.
