Nasopharyngeal masses in adults.

Nasopharyngeal masses in adults present a perplexing problem because of the concern for malignancy. A retrospective review of the records, radiographs and biopsy histology of 57 patients presenting in one year with nasopharyngeal masses at the Massachusetts Eye and Ear Infirmary suggests that persistent pain, epistaxis, trismus, or cervical mass are uncommon in patients with benign histology and should alert the clinician to the probability of malignancy. A high index of suspicion on the part of the clinician is essential for the successful recognition of early lesions. The roles of convention…

[Disorders of the coordination of masticatory muscles in lesions of the brain stem].

Observations of 10 patients were reported. In addition to other neurologic deficits, coordination of the trigeminus, a motor nerve, may also be disturbed with brain lesions. Disturbance of bilateral synergism can be demonstrated with the EMG. The muscles closing the jaw on the affected side are paradoxically innervated when the jaw is opened, i.e., they function like antagonists to the muscles responsible for jaw closure on the opposite side. This condition may produce trismus. Similar central disturbances in bilateral coordination of the ocular muscles have also been established. Bilateral sy…

The Role of Kinesio Tape Application on Blood Flow and Morbidity After Third Molar Tooth Extraction.

BACKGROUND: The most common side effects of mandibular third molar operations are pain, edema, and trismus. Postoperative application of the kinesio tape (KT) may improve these side effects more rapidly by increasing blood flow. PURPOSE: The aim of this study was to measure the effect of KT application on blood flow after mandibular third molar operation. STUDY DESIGN, SETTING, SAMPLE: This study was conducted as a single-center, parallel-arm, randomized study at Gaziantep University Faculty of Dentistry in 2023. Healthy patients who underwent mandibular third molar surgery were included in …

Diagnosis and management of temporomandibular joint dysfunction before surgery: a case report.

We present a case of colorectal cancer with temporomandibular joint dysfunction and discuss the management of the case. Type IIIb temporomandibular disorder involves anterior dislocation of the articular disk, trismus and difficult intubation. A 68-year-old woman was scheduled for colectomy. The day before surgery, the patient had temporomandibular pain. On examination, the mouth opening was 13 mm. We diagnosed type IIIb temporomandibular disorder. A simple splint was fabricated with gauze and she bit it. The mouth opening was 55 mm on the day of surgery. The pain disappeared, and intubation w…

Airway management and outcomes in surgical drainage of severe odontogenic infections: a retrospective cohort study.

BACKGROUND: Severe odontogenic infections requiring surgical drainage pose significant airway management challenges. This study aimed to com … were used. RESULTS: Of 85 patients, 60% were managed with AFOI. Multivariable analysis identified trismus (OR = 6.67, 95% CI 1.53, 39.53, p = 0.010) and higher BMI (OR = 1.15, 95% CI 1.03, 1.33, p = 0.013) as independent predictors of AFOI selection. AFOI patients experienced more complex postoperative courses, with intraoperative tracheostomy, ICU admi …

Non-Wound Tetanus Mimicking Botulism in a Low-Resource Setting: Early Recognition and Successful Intensive Care Management.

INTRODUCTION: Tetanus is a vaccine-preventable, toxin-mediated neurologic emergency caused by Clostridium tetani. Although classically associated with contaminated wounds, the portal of entry may be trivial or unrecognized, resulting in so-called non-wound presentations that can delay diagnosis. Early tetanus may also mimic other toxin-mediated syndromes, including botulism, particularly when cranial or bulbar symptoms predominate. CASE PRESENTATION: A 41-year-old man in Somalia presented with rapidly progressive trismus, neck stiffness, dysarthria, perioral paresthesia, and marked masseter r…

Comparison of Postoperative Outcomes Between Envelope and Triangular Flaps After Mandibular Third Molar Surgery: A Systematic Review and Meta-Analysis.

PURPOSE: The objective of the present systematic review was to compare the postoperative outcomes between envelope and triangular flaps after mandibular third molar surgery. MATERIALS AND METHODS: The PubMed, Embase, Web of Science, and Cochrane Library databases were systematically searched from the inception date to November 2018. Randomized and nonrandomized controlled trials were included if they had met certain inclusion and exclusion criteria. The predictor variable was the flap design, envelope or triangular. The outcome variables were operation time, pain, trismus, alveolar osteitis (…

Successful management of generalized tetanus in a 12-year old girl without anti-tetanus immunoglobulins: a case report.

In low-income countries tetanus is a major public health concern owing to a low immunization coverage and unclean birth practices. Although it is a vaccine-preventable disease, tetanus can be contracted across the life course. The disease is prevalent and harmful in newborn babies and their mothers when the mothers` are unvaccinated against tetanus. We report on a case of a 12-year-old girl who presented with general malaise, anorexia, dysphagia, trismus and dehydration, which rapidly developed into severe generalized tetanus and was successfully managed in a low-resource setting without the a…

Skull Base Osteomyelitis With Facial and Trigeminal Nerve Involvement Secondary to Otitis Media.

Skull base osteomyelitis (SBO) is a rare condition that is associated with high morbidity rates. It is most commonly caused by an infection in the external part of the ear that eventually spreads to the skull base and related structures. To our knowledge, reports in the literature concerning atypical causes of SBO (e.g., otitis media) have been scarce. In this report, we present a rare case of an elderly diabetic patient who presented with unilateral SBO originating from an infection in the middle ear. This patient also had ipsilateral facial weakness and trismus, indicating a spread of the in…

Technology advancements in the diagnosis and treatment of peritonsillar abscess.

A 17 year-old man presented to the emergency department with signs and symptoms of a peritonsillar abscess. His trismus was sopronounced that it was too difficult to drain the abscess under dynamic ultrasound guidance. It was suggested that localization of the abscess with ultrasound be used concurrently with video laryngoscopy. The ultrasound was used to localize the abscess and visualize its depth. The laryngoscope was then used to visualize the exact spot, where the ultrasound probe characterized the abscess. The probe was then removed, and a needle attached to a syringe was used in its pla…

[Zygomatic arch fracture extended to the glenoid fossa: a cause of TMJ pain].

INTRODUCTION: Fractures of the zygomatic arch extended to the glenoid fossa are rare and seldom reported. OBSERVATION: A 26-year-old man with a slightly dislocated fracture of the zygoma associated with trismus, and localized pain in the right Temporo-Mandibular Joint (TMJ) on palpation, but without any condylar fracture. The craniofacial CT scan revealed a fracture of the zygoma extended to the glenoid fossa. The treatment was an intraoral reduction of the zygoma and screw fixation of the glenoid fossa lateral region. DISCUSSION: The risk of delayed healing by mobilization of the bone fragm…

Myositis ossificans in medial, lateral pterygoid, and contralateral temporalis muscles: a rare case report.

Myositis ossificans is ossification and bone formation within muscle. It is a rare and unusual pathologic entity that has defied most efforts to establish definite etiology, pathogenesis, and satisfactory treatment. Myositis ossificans is mainly of 2 types: myositis ossificans progressiva (MOP) and myositis ossificans traumatica (MOT). Myositis ossificans idiopathic is also reported in the literature. When it affects the muscles of mastication, it causes severe trismus. Computed tomography scans and magnetic resonance imaging can effectively delineate the calcified mass. In this article, we pr…

[Diagnosis and therapy of a retropharyngeal abscess].

A 53-year-old female patient developed a phlegmon of the floor of the mouth after external extraction of 38 acutely inflammed teeth. The phlegmon tracked retropharyngeally and settled on the opposite side. It was difficult to estimate the amount of tissue area affected by the abscessed phlegmon because of severe trismus. The fact that the inflammation had basically tracked retropharyngeally and inferiorly was ascertained via a lateral radiograph of the neck which showed that the lower part of the prevertebral soft-part shadow was considerably wider than normal. The life-threatening infection h…

[Hyperplasia of the coronoid processes of the mandible--case-history].

Hyperplasia of the coronoid processes of the mandible in a 36-year-old man was associated, in addition to difficult opening of the mouth, with noises of apparently articular origin. The noise was due to the shifting of the coronoid processes along the inner surfaces of the temporal processes of the facial bones. On the X-ray picture also the elongated styloid processes were striking as well as symmetrial exostoses at the lower borderline of the mandible and condylar processes deformed without any apparent cause.
