Differentiation of Mycobacterium kansasii infection from Mycobacterium tuberculosis infection: comparison of clinical features, radiological appearance, and outcome.

This retrospective study sought to systematically identify clinical and radiological features differentiating Mycobacterium kansasii from Mycobacterium tuberculosis infection. The sample included matched patients with a culture-positive diagnosis of M. tuberculosis infection (n = 121) or M. kansasii infection (n = 62) derived from the databases of two tuberculosis centers. Data on patient background and clinical features were collected, and chest radiographs were analyzed. Sixty percent of the M. kansasii group were native Israelis compared to 15% of the M. tuberculosis group (p = 0.0001). M.

A case of pleural paragonimiasis confused with tuberculous pleurisy.

Here, we report a case of pleural paragonimiasis that was confused with tuberculous pleurisy. A 38-year-old man complained of a mild febrile sensation and pleuritic chest pain. Radiologic findings showed right pleural effusion with pleural thickening and subpleural consolidation. Adenosine deaminase (ADA) activity in the pleural effusion was elevated (85.3 IU/L), whereas other examinations for tuberculosis were negative. At this time, the patient started empirical anti-tuberculous treatment. Despite 2 months of treatment, the pleural effusion persisted, and video-assisted thoracoscopic surgery

Comparison of the diagnostic values of biopsies of the pleura and cytologic evaluation of pleural fluids.

Results of a comparison of the diagnostic efficacy of percutaneous needle biopsy of the parietal pleura with that of cytologic examination of pleural fluid sediment obtained concurrently from 166 patients with benign or cancerous pleural disorders are reported. Of 44 patients with confirmatory evidence of cancer involving the pleural surfaces, 43 had positive cytologic findings, whereas pleural biopsies were diagnostic in only 16 cases. Of a total of 122 patients with benign pleural diseases, cytology provided the diagnosis in two cases of eosinophilic pleurisy, and pleural biopsy contributed

Thoracoscopy: technique and results in eighteen patients with pleural effusion.

Eighteen patients with pleural effusion were examined. Nine patients had malignant neoplasm and in one the pleura was diffusely involved. Nine patients had pleural effusion of inflammatory origin. One of these was due to tuberculosis, one to trichinosis and the other seven were nonspecific. Thoracotomy in three of these seven failed to add additional information. All patients tolerated pneumothorax well with equilibration of pleural space and atmospheric pressure.

[False recurrences in pulmonary tuberculosis].

A number of 170 relapses have been recorded between 1966 and 1975 in the sector nr. 1 of Bucharest. Of a total of 119 files that were examined a number of 11 false relapses have been selected. The authors stress the difficulties of a definite diagnosis when bacteriological confirmation in lacking, as well as previous radiological documents, and when sputum cannot be collected for bK examination. With regard to pulmonary tuberculosis involving the pleural cavity, repeated examinations of the pleural fluid are necessary, and especially pleural bioptic puncture. Relapses in pulmonary tuberculosis

Bronchial tuberculosis with recurrent spontaneous pneumothorax: A case report.

BACKGROUND: Spontaneous pneumothorax associated with tuberculosis due to clinical manifestations, imaging findings and negative pleural biopsy is rare. CASE REPORT: A 43-year-old young woman went to the hospital several times because of recurrent dyspnea and was diagnosed with a right spontaneous pneumothorax. She underwent multiple closed thoracic drainage procedures, but the pneumothorax was not completely resolved. Pleural biopsy pathology was chronic inflammation; there was no evidence of tuberculosis. A small amount of pneumothorax persisted, intermittent dyspnea became more severe, and

Long-term spontaneous remission with active surveillance in IgG4-related pleuritis: A case report and literature review.

Pleural effusion is a relatively rare feature of IgG4-related disease (IgG4-RD). Here, we report a case of a 72-year-old woman who presented with pleural effusion. Although the pleural adenosine deaminase level was increased, surgical biopsy of the pleura and left inguinal lymph node indicated that the effusion was due to IgG4-RD. Active surveillance was initiated because serum IgG4 and pleural effusion naturally decreased and then completely disappeared. The patient has shown no recurrence for >4 years. This case suggests that pleural biopsy can be used to distinguish IgG4-RD from tuberculosi

Demonstration of extracellular traps in tuberculous pleural effusion from adult patients.

In pleural tuberculosis (TB), pleural effusion is typically characterized by a lymphocytic-rich exudate; however, neutrophils and macrophages predominate during the initial phase of infection. These immune cells are known to combat pathogens via phagocytosis, degranulation, and formation of extracellular traps (ETs)-a recently recognized defence mechanism. Although in-vitro and in-vivo studies have demonstrated ETs formation during Mycobacterium tuberculosis infection, their presence in clinical samples of patients with pleural TB has not been previously demonstrated. In this study, we employe

Comparative efficacy of methods for early diagnosis of tuberculosis pleuritis with rifampicin-resistance.

The purpose of the article was to explore the effectiveness of a complex morphological study of pleural biopsies and molecular genetic study (GeneXpert MBT/Rif) of pleural effusion in verifying the diagnosis of pleurisy of tuberculous aetiology. The participants of the study were 120 patients with a exudative pleurisy who were admitted to the department of extrapulmonary tuberculosis of the Regional Phthisiopulmonology Center (RPPC) in Aktobe (Republic of Kazakhstan) in the period from 2018 to 2020. Significant differences were obtained in the compared groups (p<0.05), which testifies to the r

Tuberculous pleural effusions: advances and controversies.

On a global scale, tuberculosis (TB) remains one of the most frequent causes of pleural effusions. Our understanding of the pathogenesis of the disease has evolved and what was once thought to be an effusion as a result of a pure delayed hypersensitivity reaction is now believed to be the consequence of direct infection of the pleural space with a cascade of events including an immunological response. Pulmonary involvement is more common than previously believed and induced sputum, which is grossly underutilised, can be diagnostic in approximately 50%. The gold standard for the diagnosis of tu

In-hospital airborne tuberculous infection from a lesion of calcified pleural thickening during thoracic surgery in a patient with lung cancer.

A 75-year-old Japanese man underwent thoracic surgery to treat a large lung cancer lesion in the left upper lobe with calcified pleural thickening. Postoperatively, viable Mycobacterium tuberculosis was detected in the margin of the resected thickened calcified pleural lesion. Therefore, an infection control investigation of medical staff who had come in contact with the patient was conducted. Consequently, two of the 14 healthcare professionals who had been in the operating room were diagnosed with latent tuberculous infections. Therefore, strict precautions against airborne infections are re

Spontaneous Chest Wall Interstitial Emphysema Secondary to Chronic Empyema Thoracis.

Spontaneous extension of air from the pleural space to the chest wall, also referred to as pneumothorax necessitans (PN), is a rare occurrence. Few of such cases have been reported in the literature, some appear to have a pattern of extension from pleural cavity to chest wall. Clinical conditions known to predispose to this complication are pneumothorax, empyema thoracis and tuberculosis of the pleural space or rib. We report a case of PN arising as complication of postpneumonectomy empyema (PPE) secondary to broncho-pleural fistula. Imaging features are highlighted emphasizing the likely patt

Pulmonologist-performed Ultrasound-guided Transthoracic Biopsy of Pleural-based Lung Masses: Diagnostic Yield and Safety, a Retrospective Study.

OBJECTIVE: Ultrasound-guided transthoracic lung biopsy is a well-established, minimally invasive technique for evaluating peripheral and pleural-based lung lesions. Its use by trained pulmonologists has expanded in recent years; however, evidence from tuberculosis-endemic regions remains limited. This study aimed to assess the diagnostic yield and safety profile of pulmonologist-performed ultrasound-guided transthoracic biopsy of pleural-based lung masses. MATERIAL AND METHODS: This retrospective study included patients who underwent ultrasound-guided transthoracic biopsy of pleural-based lun

[Axonal neuropathy revealing pleural lymphoma as a long-term outcome of therapeutic pneumothorax].

Peripheral neuropathy is a rare presenting feature of malignant lymphoma, and commonly associated with diagnostic delay. We report a patient with axonal neuropathy revealing primary pleural lymphoma as a late outcome of pulmonary tuberculosis. A 72-year-old-man with a past medical history of pulmonary tuberculosis presented with a 5-month history of axonal neuropathy. The patient complained of chest pain, altered general status. Chest computed tomography (CT) showed pleural tumour invading the chest wall and CT-guided pleural biopsy revealed a B-cell lymphoma. Chemotherapy was not started in c
