Image-Guided Biopsies and Interventions of Mediastinal Lesions.

Optimal assessment of the mediastinal masses is performed by a combination of clinical, radiological and often histological assessments. Image-guided transthoracic biopsy of mediastinal lesions is a minimally invasive and reliable procedure to obtain tissue samples, establish a diagnosis and provide a treatment plan. Biopsy can be performed under Computed Tomography, MRI, or ultrasound guidance, using a fine needle aspiration or a core-needle. In this paper, we review the image-guided strategies and techniques for histologic sampling of mediastinal lesions, along with the related clinical scen

Ultrasound-guided microwave ablation for breast tumors: current status and future perspectives.

Ultrasound (US)-guided microwave ablation (MWA) has emerged as a promising minimally invasive therapy for both benign and malignant breast tumors. This review comprehensively examines the current clinical status, technical principles, and therapeutic outcomes of US-guided MWA in breast tumor management. We discuss the biophysical mechanisms of MWA, its advantages over other ablation techniques-such as rapid temperature elevation, the ability to create more extensive coagulation areas, and diminished impact from heat sink phenomena-and the critical role of real-time US guidance in enhancing pro

Updates in ultrasound-guided musculoskeletal interventions.

Musculoskeletal ultrasound (MSK-US) has evolved from a diagnostic tool into a key modality for guiding minimally invasive interventions across the musculoskeletal system. Its real-time imaging, portability, and absence of ionizing radiation make it a cost-effective and safe alternative to fluoroscopy and CT for numerous procedures, including biopsies, aspirations, injections, and percutaneous releases. Recent technological advances in ultrasound, including high-frequency probes, microvascular imaging, needle-tracking systems, and robotic probe steering, have improved precision and expanded its

Endoscopic management of recurrent obstructive jaundice after endoscopic ultrasound-guided biliary drainage.

EUS-GUIDED biliary drainage (EUS-BD) has recently gained widespread acceptance as a minimally invasive alternative method for biliary drainage. However, the risks of encountering recurrent biliary obstruction (RBO) after EUS-BD have increased due to the growing clinical experience of EUS-BD and prolonged prognosis of the underlying disease. Previous studies have shown that the incidence of RBO following EUS-BD ranges from 11% to 25%. Nevertheless, literature on the efficacy of reintervention of RBO after EUS-GUIDED hepaticogastrostomy (EUS-HGS) and case reports describing the procedural detail

A Novel Device for Training and Evaluating Ultrasound-Guided Procedures in Anesthesia.

To improve dynamic needle-tip visualization, nurse anesthesiology students were evaluated during simulated ultrasound-guided procedures. This quasiexperimental study utilized two computed tomography 3D printed models. Thirty-two students performed each procedure twice, once without and once with needle guidance. Measures focused on accuracy and procedural performance to determine the impact that guidance versus no guidance had on attempts. Students evaluated their experiences, self-confidence, feasibility, and usability using needle guidance technology. Needle guidance improved the distance to

Spinal Sonography for Ultrasound-Guided Lumbar Neuraxial Anesthesia.

Neuraxial anesthesia is one of the few remaining forms of regional anesthesia that relies on palpation and tactile feedback techniques to facilitate catheterization into the epidural space. Over two decades ago, spine ultrasonography was demonstrated to provide reliable guidance for locating the epidural space. Compared to the palpation technique, preprocedural ultrasonography has been shown to result in fewer needle punctures and fewer traumatic procedures, particularly in patients with abnormal or distorted spine anatomy (e.g., scoliosis, obesity). Despite its utility, the ultrasound-guided

Comprehensive Review of the Management of Patients With Acute Cholecystitis Who Are Ineligible for Surgery.

OBJECTIVE: Review the current literature for available treatments for acute cholecystitis (AC) in nonsurgical candidates and provide guidelines for the management of these patients. BACKGROUND: Cholecystectomy is the gold-standard treatment modality for AC. A considerable number of patients who are not eligible for surgery are managed by percutaneous and endoscopic techniques. There is recent data regarding endoscopic ultrasound-guided gallbladder drainage and emerging percutaneous approaches to address cholelithiasis and remove drains. METHODS: An expert panel of surgeons, gastroenterologis

Ultrasound-Guided Nerve Hydrodissection for Peripheral Entrapment Neuropathies.

Ultrasound-guided nerve hydrodissection is a technique whereby fluid is used to separate a nerve from surrounding tissue as a means to alleviate symptoms of peripheral entrapment neuropathies. Pre-procedure scans are necessary to plan a safe procedure. Typically, the operator will target the site of nerve entrapment, which is just distal to the point of maximum cross-sectional area enlargement. Possible mechanisms for improvement in symptoms include improvement in the function of the nervi nervorum and vasa nervorum. These procedures offer an option after conservative measures (such as splinti

Technical note: durable resolution of hydrocephalus after ultrasound-guided percutaneous fenestration of giant suprasellar arachnoid cyst in a neonate.

Arachnoid cysts are relatively common, but rarely require intervention. While most arachnoid cysts in typical middle or posterior cranial fossa locations are seldom symptomatic, suprasellar cysts may become symptomatic due to the potential for ventricular outflow obstruction and hydrocephalus. Typical standard of care for the treatment of these lesions is endoscopic fenestration with third ventriculostomy, or the placement of ventriculoperitoneal or cystoperitoneal shunts. The surgical and anaesthetic risks of traditional interventions may be higher in the early neonatal period, including leak

Ultrasound-guided spinal anaesthesia for a patient with severe scoliosis.

Ultrasound (US) guided spinal anaesthesia is gaining recognition for its ability to minimise technical difficulties encountered in patients with atypical spinal anatomy. Spinal anaesthesia is administered during a surgical procedure lithotripsy in patients with respiratory complications that can result from severe scoliosis or cerebral palsy. Here, we report a case in which US-guided spinal anaesthesia proved feasible and advantageous in a patient with cerebral paralysis and severe scoliosis. A man in his 60s with cerebral paralysis and severe scoliosis was scheduled for spinal anaesthesia bec

Efficacy of Ultrasound-Guided Tendon Release for Trigger Finger Compared With Open Surgery: A Systematic Review and Meta-Analysis.

BACKGROUND: Trigger finger (also known as stenosing tenosynovitis) is a chronic inflammatory disorder that affects the fingers and causes discomfort and functional impairment. It is estimated to affect 2-3.6% of the population and is more common in manual laborers and individuals engaged in repetitive hand activities. This study comprehensively compares the efficacy of ultrasound-guided release versus traditional open surgery in treating trigger fingers. MATERIALS: We systematically searched PubMed, Scopus, EMBASE, and the Cochrane Library to identify relevant studies. Inclusion criteria were

Ultrasound-Guided Glossopharyngeal Nerve Block for an Awake Intubation in a Patient Predicted to Have a Difficult Airway: A Case Report.

A peripheral nerve block may be used to improve patient tolerance of awake intubation. During an awake intubation, the glossopharyngeal, superior laryngeal, and recurrent laryngeal nerves can mediate discomfort, pain, cough, glottic closure, and gag reflexes. We describe the use of ultrasound-guided superior laryngeal, recurrent laryngeal, and glossopharyngeal nerve blocks to facilitate awake intubation in a patient predicted to have a difficult airway. The glossopharyngeal nerve block was performed via the parapharyngeal space approach targeting the distal glossopharyngeal nerve. This procedu

Endoscopic Management of Gallbladder Disease.

PURPOSE OF REVIEW: Diseases of the gallbladder can be increasingly managed through endoscopic interventions, either serving as an alternative to or obviating the need for cholecystectomy. In this review, we aim to review the most recent data on endoscopic management of the most common gallbladder diseases. RECENT FINDINGS: The development of lumen-opposing metal stents (LAMS) marked a major shift in gallbladder management, with transmural techniques now well studied for management of cholecystitis. Endoscopic ultrasound (EUS) is also a well-developed technique for gallbladder imaging, compara
